Homocysteine sits at a junction: it is either remethylated back to methionine, which needs B12 and folate, or converted onward to cysteine, which needs B6. A raised level means one of those routes is congested, and the four common reasons are quite different — genuine B12 deficiency, genuine folate deficiency, reduced kidney clearance, and an underactive thyroid. Supplementing a B-complex without knowing which one is why so many people take methylfolate for years with a homocysteine that never moves.
What this panel can settle, and by what logic
Eight markers that turn one raised number into a named cause, because homocysteine sits at a junction with four common ways to congest it.
- Methylmalonic acid is what survives supplementation, and that is the reason to buy this panel rather than a B-complex. Homocysteine can be remethylated using folate or converted onward using B6, so folate alone can normalize the number while a B12 deficiency continues untreated. MMA rises only in the B12-dependent step and is unaffected by folate, which is what makes it the discriminator Harrington 2024. - Cystatin C with eGFR covers the cause nobody attributes. Reduced filtration raises homocysteine, and homocysteine has been examined explicitly as a marker in the progression of kidney as well as cardiac disorders Mohan 2023. Creatinine reads falsely poor in muscular people, and the discordance between the two filtration estimates is largest exactly there Wang 2023.
- Folate, RBC not serum folate. Red cell folate integrates status over the months the homocysteine took to rise, which is the matching timescale; the assay has its own overestimation problem worth knowing about Hunt 2020.
- TSH (Thyroid-Stimulating Hormone) with Free T4 (Thyroxine) is the cheapest of the four causes to exclude, and hypothyroidism raising homocysteine is part of the standing differential McCaddon 2023.
- Comprehensive Metabolic Panel (CMP) provides albumin, glucose and liver function, which is the background the rest is read against.
What it cannot settle, and what would
It cannot tell you that lowering the number will help you, and this is the honest headline. Homocysteine is a reliable marker of B-vitamin and renal status and a much less reliable treatment target: B-vitamin lowering trials have generally not reduced cardiovascular events even where the number fell McCaddon 2023. The clearest positive result was in a different domain, where B-vitamin lowering slowed brain atrophy in people with mild cognitive impairment and raised homocysteine Smith 2010. Treat the deficiency this panel finds, not the number.
It cannot measure B6 status, which is the third arm of the pathway and is not in this catalog McCaddon 2023.
MTHFR genotyping is not the missing piece, and this is worth saying because it is the test people buy next. Practice guidance from medical genetics recommends against routine MTHFR polymorphism testing, because it does not change management Hickey 2013. The actionable questions are what B12, folate, thyroid and kidney are doing, and all four are already on this requisition.
And it cannot tell you why a normal-looking Vitamin B12 is not working. Serum B12 is dominated by the haptocorrin-bound fraction that tissue cannot use, so the level and the function come apart Harrington 2024. Holotranscobalamin is the other functional marker and is not in this catalog either.
Draw conditions that decide whether the money is wasted
Sample handling matters here in a way it does not on most panels.
- Homocysteine rises in the tube if the sample sits. Red cells keep producing it after the draw, so the specimen has to be separated promptly. Use a proper draw station rather than a mobile collection with a long transport, and if the result is surprisingly high, ask how long the sample took to reach the analyzer McCaddon 2023.
- Stop B-complex, methylfolate and B12 for 5 to 7 days. Otherwise the panel measures the supplement rather than the status, and a recent B12 injection makes the serum level uninterpretable for months while Methylmalonic Acid (MMA) stays informative Harrington 2024.
- Fast 9 to 12 hours, and avoid a large protein load the evening before, because methionine intake feeds directly into this pathway.
- Not within 2 weeks of an illness, and not within 48 hours of a hard session, both of which move the kidney markers.
How you would know it answered your question, and what each pattern means next
Five patterns, each naming the marker that would move if the cause is right.
- Raised homocysteine with a raised Methylmalonic Acid (MMA): B12, whatever the serum Vitamin B12 says. Replace B12 and recheck MMA at 8 weeks; a flat MMA means B12 was not the problem Harrington 2024.
- Raised homocysteine with a low Folate, RBC and a normal Methylmalonic Acid (MMA): folate. Recheck red cell folate at 12 weeks, not at 2, because the marker follows the red cell population Hunt 2020.
- Raised homocysteine with a reduced Cystatin C with eGFR filtration rate and normal vitamins: the kidney, and B vitamins will not move it Mohan 2023.
- Raised homocysteine with a raised TSH (Thyroid-Stimulating Hormone) and low Free T4 (Thyroxine): the thyroid. Recheck homocysteine 12 weeks after the TSH normalizes.
- All four normal: common, largely genetic and dietary, and worth re-measuring rather than chasing McCaddon 2023.
