People rarely search for a marker — they search for how they feel. These are the complaints where this marker is worth checking, and whether it is a first-line test or a follow-up once the obvious causes are ruled out.
Why your Folate, RBC might be wrong
Most abnormal results are interference, not disease. These are the things that measurably move this specific marker — check them before you change anything. Each one says whether the number is wrong and should be repeated, badly timed and should be redrawn, or real with a cause worth explaining.
B12 deficiency traps folate in an unusable form, which can raise RBC folate while functional folate status is poor.

What Folate, RBC means in combination
A single marker tells you a little. Combinations tell you the story — these are the named patterns this marker takes part in, and what each one points at.
The pathway is working. MTHFR variants are extremely common — around a quarter of some populations carry two copies — and carrying one is close to unremarkable. Homocysteine is the functional readout, and a normal one says the genotype is not causing you a problem.
Treat the number, not the genotype. If homocysteine is raised, correct B12 and folate — with B12 first, always, because folate can normalise the anaemia while nerve damage continues.
That is a malabsorption pattern, not a diet pattern. Coeliac disease, atrophic gastritis, H. pylori, pancreatic insufficiency and inflammatory bowel disease all present this way, and correcting the individual nutrients without finding the cause is a treadmill.
Investigate why absorption is failing rather than supplementing indefinitely. Coeliac serology must be done before removing gluten. Any neurological symptom with low B12 or copper needs urgent attention.
RDW is how much your red cells VARY in size, and it is the most under-read number on a CBC. It rises before MCV moves, because new cells produced under a developing deficiency differ in size from the healthy ones still in circulation — so the average is still normal while the spread has already widened. That makes a raised RDW with a normal MCV and a normal haemoglobin an early signal rather than a curiosity: iron, B12 or folate starting to run short, often months before anything else on the panel is flagged, and long before you would be called anaemic. It also rises in mixed deficiency, where MCV can be deceptively perfect — small iron-deficient cells and large B12-deficient cells averaging out to a normal number that describes nobody.
Treat it as a prompt to look, not as a diagnosis. Ferritin with iron saturation, B12 with MMA, and folate — that combination explains most raised RDW with an otherwise clean panel. A normal MCV does not rule out a deficiency when RDW is up. That is the specific trap: two opposite deficiencies can produce a textbook average. If RDW is raised, the MCV has stopped being reassuring. If iron, B12 and folate all come back clean, RDW is worth repeating rather than dismissing — a rising trend means more than any single value.
