An amino acid intermediate that accumulates when B-vitamin-dependent methylation pathways run inefficiently.

Elevated homocysteine is associated with cardiovascular and cognitive risk — and it's one of the most easily corrected markers on any panel, usually with inexpensive B vitamins.

The plan of attack

In this order. Most people start at step four, which is why they change five things at once and learn nothing.

  1. Confirm the number is real
    Delay before separating the sample — the big one. Red cells keep producing homocysteine after the draw. If the sample isn't separated within about an hour, homocysteine rises measurably in the tube. This is the most common cause of a mildly raised result. Ask for prompt separation or a chilled sample. Repeat an isolated high value at a lab that handles it properly.
  2. Read it with its partner
    Pair with MTHFR genotyping plus B12 and folate — together they tell you exactly why it's high and which supplement form to use. Draw it alongside: Vitamin B12, Folate, Serum, MTHFR, DNA Analysis.
  3. Work out which direction is yours
    If it's high — Correctable in most cases within 8–12 weeks of appropriate B-vitamin repletion.
    If it's low — Generally fine.
  4. Fix it in this order
    Nutrition. Leafy greens and legumes (folate), animal proteins (B12), and adequate protein for glycine/betaine pathways.
    Lifestyle. Stop smoking, moderate alcohol, treat hypothyroidism and kidney disease.
    Supplements. Methylfolate + methylcobalamin (B12) + P5P (B6) is the standard stack. Add betaine/TMG and riboflavin if MTHFR variants are present. Use methylated forms if you carry MTHFR variants.
    Hormones. Address hypothyroidism, which raises homocysteine.
    Compounds. No direct peptide interaction.
    Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail.
  5. Retest
    8–12 weeks after starting B vitamins. Change one thing at a time, or the retest can't tell you which thing worked.

What Homocysteine means in combination

A single marker tells you a little. Combinations tell you the story — these are the named patterns this marker takes part in, and what each one points at.

Several independent contributors to clot risk stacking together. Each is unremarkable alone; the combination is not, particularly alongside androgen use, high haematocrit, oestrogen therapy or long-haul travel.

A raised D-dimer with leg swelling, chest pain or breathlessness is an emergency, not a retest. Otherwise: address homocysteine with B12/folate/B6, treat the inflammation driving fibrinogen, and review anything oestrogenic or androgenic you are taking with a clinician.

Serum B12 is an insensitive marker — you can be functionally deficient at the low end of 'normal.' Homocysteine and MMA reveal what B12 alone hides.

Methylfolate + methylcobalamin + P5P, add TMG/betaine. Test MTHFR to guide which forms to use. Check for metformin or PPI use as the cause.

MMA rises when B12 is short at the tissue level regardless of what the serum number says. This is the test that settles the argument, and neurological damage from prolonged deficiency is not always reversible.

Treat it, do not re-argue the serum number. Methylcobalamin or hydroxocobalamin; injections if absorption is the problem. Look for the cause: metformin, PPIs, gastric surgery, coeliac, pernicious anaemia, plant-based diet.
