Folate, Serum
A B vitamin essential for DNA synthesis, red cell production and methylation.
Works in lockstep with B12. Deficiency causes macrocytic anemia, fatigue and elevated homocysteine — and it's critical preconception, since folate status in early pregnancy determines neural tube development.
The plan of attack
In this order. Most people start at step four, which is why they change five things at once and learn nothing.
- Confirm the number is real
What you ate this week. Serum folate reflects recent intake — literally your last few meals. A single leafy-green dinner can normalise it. RBC folate reflects months and is the better test. Use serum only for acute questions. - Read it with its partner
Serum folate reflects recent intake; RBC folate reflects long-term status. Draw it alongside: Vitamin B12, Homocysteine, MTHFR, DNA Analysis. - Work out which direction is yours
If it's high — Usually supplementation. Very high serum folate with low B12 can mask B12 deficiency while neurological damage progresses — always check both together.
If it's low — Fatigue, macrocytic anemia, glossitis, elevated homocysteine, and in pregnancy, neural tube defect risk. - Fix it in this order
Nutrition. Dark leafy greens, legumes, asparagus, avocado, liver. Cooking destroys a substantial portion — eat some raw.
Lifestyle. Reduce alcohol (a potent folate antagonist). Screen for celiac if levels stay low despite supplementation.
Supplements. Methylfolate (5-MTHF) — preferred over folic acid, especially with MTHFR variants. Always pair with B12 to avoid masking a B12 deficiency.
Hormones. No hormonal intervention. Preconception dosing belongs with your provider.
Compounds. Part of the methylation cluster — order with B12, homocysteine and MTHFR.
Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail. - Retest
8–12 weeks after supplementation. Change one thing at a time, or the retest can't tell you which thing worked.

Would you feel it? Symptoms Folate, Serum helps explain
People rarely search for a marker — they search for how they feel. These are the complaints where this marker is worth checking, and whether it is a first-line test or a follow-up once the obvious causes are ruled out.
Why your Folate, Serum might be wrong
Most abnormal results are interference, not disease. These are the things that measurably move this specific marker — check them before you change anything. Each one says whether the number is wrong and should be repeated, badly timed and should be redrawn, or real with a cause worth explaining.
Serum folate reflects recent intake — literally your last few meals. A single leafy-green dinner can normalise it.
RBC folate reflects months and is the better test. Use serum only for acute questions.
Methotrexate blocks folate metabolism directly; several anti-epileptics and chronic alcohol both deplete it over time.
Note any of these — and if you are on methotrexate, folate dosing and timing is your prescriber's decision.
What Folate, Serum means in combination
A single marker tells you a little. Combinations tell you the story — these are the named patterns this marker takes part in, and what each one points at.
Multiple unrelated nutrients low at once is rarely four separate dietary problems — it points at absorption. Coeliac, inflammatory bowel disease, chronic PPI use, gastric surgery or low stomach acid all produce this.
Stop supplementing harder and find the cause. Coeliac serology is cheap and routinely skipped. Review PPI and metformin use. The Gut Health panel is built for this picture, and fixing absorption fixes all of them at once.
What to test next
Markers rarely answer alone. These are the ones that put Folate, Serum in context — each with its own full breakdown.
