The plan of attack

  1. Confirm the number is real
    Serum B12 is a poor test. It measures total B12, most of which is bound to haptocorrin and unavailable to cells. You can be tissue-deficient with a normal serum B12 — which is exactly why MMA exists. A low-normal B12 with symptoms should be confirmed with MMA, not dismissed.
  2. Read it with its partner

  3. Work out which direction is yours
    If it's high — Supplementation (harmless); rarely liver disease or myeloproliferative disorders.

  4. Fix it in this order
    Nutrition. Animal proteins, shellfish, eggs, dairy. Vegans require supplementation — there is no reliable plant source.
    Lifestyle. Review medications — metformin and long-term acid suppression are the two most common iatrogenic causes.
    Supplements. Methylcobalamin or hydroxocobalamin oral is effective for most; sublingual is fine. Injections if pernicious anemia or malabsorption is present. Pair with folate — treating B12 deficiency with folate alone can mask ongoing neurological damage.
    Hormones. No hormonal intervention; treat underlying malabsorption.
    Compounds. Anyone on metformin or long-term acid reducers should test B12 — both meaningfully impair absorption.
    Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail.
  5. Retest
    8–12 weeks after starting supplementation. Change one thing at a time, or the retest can't tell you which thing worked.

What Vitamin B12 means in combination

High homocysteine with 'normal' B12

Serum B12 is an insensitive marker — you can be functionally deficient at the low end of 'normal.' Homocysteine and MMA reveal what B12 alone hides.

Nothing is absorbing

Multiple unrelated nutrients low at once is rarely four separate dietary problems — it points at absorption. Coeliac, inflammatory bowel disease, chronic PPI use, gastric surgery or low stomach acid all produce this.

A well-run plant-based diet is fine, but four nutrients are genuinely harder to get and this combination shows a diet that has not been supplemented deliberately.

B12 is non-negotiable and non-optional — deficiency causes irreversible neurological damage. Add iron with vitamin C away from tea and coffee, plus vitamin D, zinc and an algal omega-3. Retest in 3 months.

RDW moved first — the earliest warning on a routine panel

RDW is how much your red cells VARY in size, and it is the most under-read number on a CBC. It rises before MCV moves, because new cells produced under a developing deficiency differ in size from the healthy ones still in circulation — so the average is still normal while the spread has already widened. That makes a raised RDW with a normal MCV and a normal haemoglobin an early signal rather than a curiosity: iron, B12 or folate starting to run short, often months before anything else on the panel is flagged, and long before you would be called anaemic. It also rises in mixed deficiency, where MCV can be deceptively perfect — small iron-deficient cells and large B12-deficient cells averaging out to a normal number that describes nobody.

Treat it as a prompt to look, not as a diagnosis. Ferritin with iron saturation, B12 with MMA, and folate — that combination explains most raised RDW with an otherwise clean panel. A normal MCV does not rule out a deficiency when RDW is up. That is the specific trap: two opposite deficiencies can produce a textbook average. If RDW is raised, the MCV has stopped being reassuring. If iron, B12 and folate all come back clean, RDW is worth repeating rather than dismissing — a rising trend means more than any single value.
