The reason to take these together rather than separately is that correcting one input alone can shift the bottleneck rather than remove it — high-dose folate with inadequate B12, for instance, can mask a B12 deficiency while nerve damage continues. Most useful where homocysteine is genuinely raised, which is a cheap test and the right way to decide. A minority feel wired or irritable on methylated forms and do better on lower doses or the non-methylated versions.
How Methylation Support actually works
Combines the four inputs the one-carbon cycle needs simultaneously: methylfolate to carry the methyl group, methylcobalamin for methionine synthase to transfer it, P5P for the transsulfuration branch that clears homocysteine to glutathione, and betaine as an alternative folate-independent route through betaine-homocysteine methyltransferase. What Methylation Support actually does
This is the rare blend where every ingredient maps onto a named enzyme and the enzymes sit in a line. The filed panel per three capsules is riboflavin 90 mg as riboflavin 5'-phosphate sodium, vitamin B6 45 mg as pyridoxal 5'-phosphate, 3 mg of L-5-methyltetrahydrofolate declared as 5,000 micrograms of dietary folate equivalents, 3 mg of methylcobalamin, and 1.8 g of betaine anhydrous Office of Dietary Supplements. Read that as five cofactor positions rather than five nutrients.
Position one: MTHFR, and the cofactor is the riboflavin. Methylenetetrahydrofolate reductase reduces 5,10-methylene-tetrahydrofolate to 5-methyltetrahydrofolate, and it does it with a flavin adenine dinucleotide prosthetic group. The common 677C>T variant substitutes valine for alanine at residue 222 and destabilizes the binding of that FAD, which is why the enzyme is described as thermolabile and why homozygotes run higher homocysteine Frosst 1995. Ninety milligrams of riboflavin in a methylation formula is therefore not filler; it is the one ingredient aimed directly at the genotype the product is marketed for.
Position two: methionine synthase, and it needs both a substrate and a cofactor from this bottle. Methionine synthase transfers the methyl group from 5-methyltetrahydrofolate to homocysteine, producing methionine and regenerating tetrahydrofolate. Its cofactor is methylcobalamin, held as cob(I)alamin between cycles and periodically re-reduced by methionine synthase reductase. So 5-MTHF is the methyl donor and methyl-B12 is the shuttle, and neither works without the other — which is the biochemical reason folate and B12 are always sold together and the reason folate alone can normalize a blood count while a B12 deficiency continues to damage a spinal cord.
Position three: betaine-homocysteine methyltransferase, the second and separate road out. BHMT is a zinc enzyme expressed chiefly in liver and kidney that methylates homocysteine using betaine — trimethylglycine — and produces dimethylglycine. It is folate-independent and B12-independent. That is why a formula with 1.8 g of betaine in it can lower homocysteine in somebody whose folate cycle is already saturated: it is not a bigger dose of the same mechanism, it is a different mechanism Steenge 2003.
Position four: the exit, and the cofactor is the P5P. Cystathionine beta-synthase commits homocysteine irreversibly to cystathionine, and cystathionine gamma-lyase splits that to cysteine — the rate-limiting amino acid for glutathione synthesis. Both are pyridoxal 5'-phosphate enzymes. The transsulfuration arm is the only route that disposes of homocysteine rather than recycling it, so B6 is what converts a methylation product into an antioxidant precursor.
What the whole loop is for, stated once. Methionine is adenylylated to S-adenosylmethionine, which is the methyl donor for essentially every methyltransferase in the body — DNA methyltransferases, catechol-O-methyltransferase disposing of dopamine and noradrenaline, phosphatidylethanolamine N-methyltransferase making phosphatidylcholine, guanidinoacetate N-methyltransferase making creatine, nicotinamide N-methyltransferase clearing surplus B3. Homocysteine is what is left when SAM has given its methyl away. It is therefore a flux marker of the whole system, which is precisely why it is both the best thing to measure here and the most over-interpreted number in the field.
Cell, rodent, human — and where it stops
Genetics first, because the product is sold on it. The 677C>T variant was described as a candidate genetic risk factor for vascular disease, acting through a thermolabile enzyme and higher homocysteine Frosst 1995. Roughly a tenth of people of European descent are homozygous. This is a real biochemical phenotype with a real biochemical consequence; whether it is a disease is a separate question and the trials below answer it.
Pharmacokinetics in people, on form. In women homozygous for 677C>T, [6S]-5-methyltetrahydrofolate raised plasma folate more effectively than an equivalent dose of folic acid Prinz-Langenohl 2009. Underneath that sits a slower and more structural fact: dihydrofolate reductase, the enzyme that has to reduce folic acid before the body can use it, is extremely slow in human liver and varies about five-fold between individuals Bailey 2009. Folic acid is a synthetic pro-drug that human beings are unexpectedly bad at activating, which is the strongest argument the methylated forms have.
Betaine in people, at a dose the label does not reach. Betaine supplementation lowered plasma homocysteine in healthy men and women, with the clearest effects in the 6 g per day range Steenge 2003. The bottle delivers 1.8 g at its full three-capsule serving Office of Dietary Supplements — less than a third of that. The betaine here is a real ingredient at a partial dose.
And now the obstacle, which is the largest and least-stated fact in this entire category. Homocysteine-lowering works and does not deliver the outcome it was supposed to. Pooling eight randomized trials in 37,485 individuals, B vitamins lowered homocysteine by roughly a quarter and produced no significant effect on heart attack, stroke, cancer or all-cause mortality Clarke 2010. The fracture trial tells the same story from another direction: daily B12 and folic acid in elderly people with raised homocysteine did not reduce fracture incidence overall van Wijngaarden 2014. So the correct sentence is that this product reliably moves the number and the number has not been shown to be the cause of the thing you are worried about. A reader is entitled to take it for the biochemistry — but not to be told it prevents heart attacks.
The one place the population data and the individual case come apart. Those trials enrolled people with vascular disease at ordinary homocysteine levels. They say nothing about somebody with homocysteine above 30 micromoles per liter, about a pregnancy where the neural tube evidence for folate is unambiguous Samaniego-Vaesken 2024, or about a B12 deficiency caught before it becomes neurological Harrington 2024. Treating a genuinely abnormal number is not the same experiment as treating a normal one.
Methylation Support — which form, and does it matter
Folate: three different molecules sold under one word. Folic acid is fully oxidized, synthetic, and has to pass dihydrofolate reductase twice before it becomes usable — through an enzyme that is slow and variable in humans Bailey 2009. Folinic acid, 5-formyl-THF, enters below that block. L-5-methyltetrahydrofolate, which is what this bottle contains as the glucosamine salt Office of Dietary Supplements, is the circulating form itself and needs no reduction. For a 677TT homozygote the methylated form raises plasma folate better Prinz-Langenohl 2009; for neural tube defect prevention the evidence base is overwhelmingly folic acid, and a review of the two side by side is explicit that the substitution is reasonable but not equally evidenced Samaniego-Vaesken 2024. That distinction matters: pregnancy is the one setting where the older, cruder molecule has the better trial record.
B12: methylcobalamin against cyanocobalamin, and the honest size of the difference. Methylcobalamin is the cofactor form for methionine synthase, so it skips an intracellular processing step. Cyanocobalamin is more stable, cheaper, and has to shed a cyanide group that is trivial in quantity. Three milligrams of methylcobalamin per serving Office of Dietary Supplements is a pharmacological dose — over a thousand times the daily requirement — and at that quantity passive diffusion delivers plenty regardless of which form you chose. The form argument is strongest at small doses and weakest at this one.
B6: pyridoxal 5'-phosphate against pyridoxine, and the dose is the point. P5P is the active cofactor; pyridoxine hydrochloride has to be oxidized to it by pyridoxine 5'-phosphate oxidase, which is itself an FMN enzyme and therefore depends on the riboflavin in the same capsule. Forty-five milligrams per three capsules Office of Dietary Supplements is roughly twenty-five times the reference intake and sits in the range where the pharmacovigilance literature on sensory neuropathy lives van Hunsel 2018.
Riboflavin: the same 5'-phosphate question as this site's riboflavin page, and the same answer. Ninety milligrams per serving Office of Dietary Supplements is a serious dose of a nutrient whose reference intake is 1.3 mg, and it is the ingredient most specifically aimed at the MTHFR variant Frosst 1995.
And the dosing decision the label leaves to you. The panel declares a one-to-three-capsule serving Office of Dietary Supplements, which means the difference between the smallest and largest suggested intake is threefold on every ingredient in it. One capsule delivers 1 mg of 5-MTHF, 15 mg of B6 and 600 mg of betaine; three capsules delivers 3 mg, 45 mg and 1.8 g. Anyone who feels overstimulated on this product has a dial to turn before they have a decision to make.
What would have to be true, and how you would know it was not
1. Fasting total homocysteine at ten weeks, and the size of the fall depends entirely on where you start. Draw it fasting, before the first capsule and again after ten weeks. From a baseline above 12 micromoles per liter, predict a fall of a quarter or more — that is the pooled magnitude across 37,485 people Clarke 2010. From a baseline of 8, predict very little, because the remethylation machinery is already running at capacity and adding substrate to a saturated enzyme changes nothing. Measuring this before you start is the difference between an experiment and a subscription.
2. Red cell folate at four months, not at four weeks. Serum folate reflects what you swallowed this week; red cell folate is incorporated during erythropoiesis and therefore reports the average of the preceding 120 days. Predict red cell folate rises substantially on 3 mg of 5-MTHF daily, and predict that a four-week retest shows you nothing but serum. This is the marker that tells you whether the methylated form is actually reaching your tissues Prinz-Langenohl 2009.
3. Methylmalonic acid, because serum B12 will lie to you on this product. Three milligrams of methylcobalamin will push a serum B12 off the top of the reference range within days and that number then carries no information. Methylmalonic acid is the functional marker — it accumulates when the other B12-dependent enzyme, methylmalonyl-CoA mutase, is short of cofactor — and it is the one to use if the question is whether a deficiency is genuinely corrected Harrington 2024.
4. The prediction that cuts against the product: your cardiovascular risk will not change, and neither will your fracture risk. No change in ApoB, no change in blood pressure, no change in coronary calcium progression, and no reduction in fractures Clarke 2010van Wijngaarden 2014. The homocysteine will move. The outcome the homocysteine was a proxy for has not moved in any adequately-sized trial ever run. If you are taking this to prevent a heart attack, the evidence does not support the purchase and this page would rather say so plainly.
5. Labeled extrapolation, and worth one blood tube. Total cholesterol and LDL at twelve weeks. Betaine loading has been reported to raise LDL and total cholesterol in some studies while lowering homocysteine Steenge 2003, which would be the awkward case of a product improving one cardiovascular marker and worsening a better-evidenced one. At 1.8 g the risk is smaller than at 6 g, and nobody has measured it at this dose.
What will fool you: the stimulation. Feeling wired on the first day of a methyl-donor product is real and it is not a measure of anything — there is no marker of overmethylation you can order, and the sensation tracks the dose rather than the biochemistry. Turn the capsule count down before concluding anything.
What nobody has tested yet
Nobody has run methylfolate against folic acid with a clinical endpoint. The head-to-head evidence is plasma folate over weeks Prinz-Langenohl 2009. There has never been a randomized trial of 5-MTHF against folic acid, at matched folate equivalents, in 677TT homozygotes, with a hard endpoint — neural tube defects, vascular events, cognition, anything. The entire commercial premise of the methylated-folate category rests on a pharmacokinetic difference that has never been shown to become an outcome difference.
Nobody has tested riboflavin alone in 677TT. The variant's defect is FAD binding Frosst 1995 and this bottle contains 90 mg of riboflavin Office of Dietary Supplements. A three-arm trial — riboflavin alone, the full formula, placebo — in genotyped homozygotes with homocysteine as the endpoint would establish how much of this product's effect comes from its cheapest ingredient. It has never been done, and it is the experiment that would most change what people buy.
Nobody has measured the trimethylamine-N-oxide cost of the betaine. Betaine sits on the same gut-microbial pathway as choline and carnitine, and a 1.8 g daily dose of a trimethylammonium compound has a plausible route to raised TMAO. Plasma betaine, dimethylglycine and TMAO before and after eight weeks would answer whether this product improves one cardiovascular marker while raising another. Two assays, thirty people.
And nobody has defined the overmethylation phenotype. A large fraction of the people who stop this product stop because of irritability or insomnia in the first week. There is no published characterization of who they are, whether they share a catechol-O-methyltransferase genotype, or what if anything moves in their blood. A crossover study with a symptom instrument and COMT genotyping would turn the commonest clinical observation in this category into data.
Methylation Support — its own safety story, not its category's
The B6 dose is this product's own specific risk and it deserves a number. Forty-five milligrams of pyridoxal 5'-phosphate per three-capsule serving Office of Dietary Supplements is roughly twenty-five times the adult reference intake. Pyridoxine-associated sensory neuropathy was historically described at hundreds of milligrams a day, but pharmacovigilance case series of spontaneously reported neuropathy in supplement users include reports at doses in the tens of milligrams and after long durations van Hunsel 2018. The presentation is distal, symmetric and sensory — tingling in the feet and hands, sometimes unsteadiness in the dark — and it usually improves on stopping. The practical rule is that this is a product to cycle or to dose against a measured homocysteine, not one to take at three capsules a day indefinitely because the bottle allows it.
The folate-masking rule, stated precisely rather than as a slogan. Folate corrects the megaloblastic anemia of B12 deficiency without correcting the neurological damage, so a full blood count can normalize while a myelopathy progresses. This formula contains 3 mg of methylcobalamin alongside its folate Office of Dietary Supplements, which mostly disarms the classic trap — but it also makes a serum B12 uninterpretable, so anybody investigating unexplained neuropathy needs methylmalonic acid rather than a B12 level Harrington 2024.
Five thousand micrograms of folate equivalents, and what the upper limit does and does not cover. The 1,000 microgram tolerable upper intake level for folate applies to synthetic folic acid, and the reason it exists is the masking problem rather than direct toxicity. L-5-methyltetrahydrofolate is not folic acid and does not produce unmetabolized folic acid in serum Bailey 2009Samaniego-Vaesken 2024, so the limit is not straightforwardly transferable. That is a real distinction and it is not a license: it means the risk profile of 3 mg of 5-MTHF is different, not that it has been established as safe at that dose for years.
Betaine, cholesterol and fluid. At 1.8 g the tolerability is generally good; the reported issues at higher doses are gastrointestinal and a rise in LDL cholesterol Steenge 2003.
The double-dosing trap that is specific to this bottle. Every ingredient here also appears in a good multivitamin. This site's own Thorne multivitamin panels declare 1.7 mg of folate equivalents and 10 mg of B6 each Office of Dietary SupplementsOffice of Dietary Supplements; add three capsules of this and a person is at 6.7 mg of folate equivalents and 55 mg of B6 without having read a second label. B6 is the one where that arithmetic has a consequence van Hunsel 2018.
And the drug interaction most likely to matter. Methotrexate is a folate antagonist and its therapeutic effect in cancer depends on that antagonism; high-dose folate is a conversation with an oncologist rather than a supplement decision. Anticonvulsants including phenytoin, carbamazepine and phenobarbital lower folate and can have their own levels altered in return.
