What Galantamine actually does

Read the label's mild cognitive impairment paragraph before anything else on this page. In two randomized placebo-controlled trials of two years' duration in people with mild cognitive impairment, 13 patients on galantamine (n=1026) and 1 patient on placebo (n=1022) died Galantamine label 2021. The label puts the rates at 10.2 per 1000 person-years against 0.7, and notes that about half the galantamine deaths appeared to result from vascular causes — myocardial infarction, stroke and sudden death. Mild cognitive impairment is not dementia. It is the closest studied population to the person who buys a nootropic because their memory feels a step slower than it did, and it is the population where this drug was tested and the deaths were counted.

That paragraph is a finding, not a warning label boilerplate, and it deserves a fair reading. Thirteen against one is a small absolute number in a two-year study of two thousand elderly people, the causes were mixed, and the label itself frames them as causes expected in an elderly population. It was still enough of a signal for a regulator to require it in the label of a drug approved for a different, sicker population. Both halves of that are true and this page states both.

The mechanism, in the label's own words and no further. Galantamine is a tertiary alkaloid and a competitive and reversible inhibitor of acetylcholinesterase; the label says the precise mechanism is unknown and that the therapeutic effect is postulated to come from enhanced cholinergic function Galantamine label 2021. The indication, and the price. Approved for mild to moderate dementia of the Alzheimer's type Galantamine label 2021. A partner lists 8 mg tablets at $112, the most expensive of the three dementia drugs in that catalog. It is a prescription medicine and this page carries no purchase route.

Cell, rodent, human — and where it stops

Two large syntheses reach opposite readings of this drug, and that is worth showing rather than resolving. Veroniki 2022 pooled 80 randomized trials and 21,138 adults, set 1.40 MMSE points as the threshold for clinical importance, and lists galantamine among the single-agent regimens that did not clear it. Li 2026 synthesized 125 trials in more than 30,000 people and ranked galantamine at a standardized mean difference of 0.40 (95% credible interval 0.22 to 0.58), the highest of any drug in that network — above donepezil at 0.21 and memantine at 0.24. Same drug, two defensible methods, two different answers.

How to hold both at once. The first analysis used raw MMSE change on an absolute-points scale; the second used standardized effect sizes across mixed instruments and reported ranking probabilities. A drug can be top-ranked on relative effect and still fall short of an absolute-points threshold, and both papers report wide intervals and substantial risk of bias — Veroniki 2022 states that two-thirds of its included trials were at high risk of bias for incomplete outcome data and that individual patient data were available for only 15% of them. The honest summary is that the effect is real, small, and measured imprecisely enough that the ranking depends on the method.

In the same network, two behaviors scored higher than every drug. Li 2026 put aerobic exercise at 0.55 and cognitive training at 0.45 against galantamine's 0.40, while cautioning that the non-drug arms are short-term and adjunctive rather than substitutes for medication. The 2025 guideline still recommends the cholinesterase inhibitors strongly for Alzheimer's dementia Kim 2025. Both statements can stand.

And every participant in all of it had dementia. The one large body of work in people without dementia is the pair of mild cognitive impairment trials described above, and that is the study set with the excess deaths in it Galantamine label 2021. This is the rare case where the evidence closest to the reader's own situation is also the evidence that argues hardest against use.

Galantamine pharmacokinetics — how much of it actually gets in

Fast in, fast out, and that shape explains the drug's unofficial use. Oral bioavailability is about 90%, peak concentration arrives at roughly 1 hour, and the elimination half-life is about 7 hours Galantamine label 2021. A molecule that is almost fully absorbed, peaks within an hour and is largely gone within a working day is exactly the profile behind the popular middle-of-the-night dosing practice for lucid dreaming. The pharmacokinetics support the practice. Nothing in the label or in either synthesis above studied it, so there is no safety dataset attached to a use pattern that deliberately puts peak concentration into the sleeping half of the day.

Eighteen percent bound is unusually low and it matters. Plasma protein binding is 18% at therapeutic concentrations Galantamine label 2021, against 96% for donepezil. A high free fraction means the displacement interactions that bedevil highly-bound drugs largely do not apply here, and it also means more of any given total concentration is pharmacologically live at once.

Two enzymes and a kidney, all of them variable. CYP2D6 and CYP3A4 are the major routes and about 20% leaves unchanged in urine within 24 hours Galantamine label 2021. CYP2D6 is genetically polymorphic and inhibited by several widely prescribed antidepressants, so two people taking the same tablet can sit at different exposures, and the person with reduced kidney function clears less of the remainder.

The dosing consequence of a 7-hour half-life. Steady state arrives in roughly a day and a half, not the two weeks that memantine and donepezil need. That makes this the one drug of the three where a short trial actually reflects the eventual exposure — and where a rapid dose escalation produces cholinergic side effects rapidly, which is why the label titrates in steps rather than starting at target.

What would have to be true, and how you would know it was not

Three predictions, and the first is about who should not be running this experiment at all.

1. A vascular baseline, before anything else. Blood pressure, ApoB, Lp(a) and hs-CRP in one sitting. The reason is specific rather than generic: the deaths in the mild cognitive impairment trials were roughly half vascular — myocardial infarction, stroke, sudden death Galantamine label 2021. Nobody has shown that baseline vascular risk explains that signal, and nobody has shown it does not. Anyone who cannot say what their own numbers are is volunteering for a trial arm without knowing which one.

2. Resting heart rate for a week before and a week after. The label carries bradycardia, syncope and heart-block warnings Galantamine label 2021, and cholinergic slowing of the sinus node is the mechanism. Prediction: resting heart rate drops by a few beats per minute at target dose. A wearable already measures this, and a drop large enough to see is a reason to stop and speak to a prescriber rather than evidence of effect.

3. Against the drug: MMSE or MoCA at baseline and 12 weeks. In dementia the pooled effect is at best around 1.4 MMSE points Veroniki 2022. In a cognitively normal adult, prediction: no change beyond test-retest variation. This has never been tested in healthy volunteers, and the closest population that was tested produced the mortality paragraph rather than a cognitive gain.

What nobody has tested yet

Nobody has explained the mild cognitive impairment deaths. Thirteen against one, across 2,048 randomized participants over two years Galantamine label 2021. Two decades later there is no published mechanism, no replication attempt in that population, and no analysis identifying who was at risk. It sits in the label as an unexplained observation, which is the worst possible state for a finding that speaks directly to the people most likely to self-prescribe this drug.

Nobody has studied the way this molecule is actually used outside medicine. Middle-of-the-night dosing for lucid dreaming is the dominant non-medical use, it is supported by the drug's own 1-hour peak and 7-hour half-life Galantamine label 2021, and there is no published study of its cardiovascular effect during sleep, its effect on sleep architecture, or its tolerability under repetition. One polysomnography study with a few dozen participants would produce more information than currently exists.

Nobody has reconciled the two syntheses. Veroniki 2022 places this drug below its clinical-importance threshold; Li 2026 ranks it first among drugs. Both are large, both are recent, and the disagreement is methodological rather than factual. A single analysis reporting both absolute MMSE change and standardized effect from the same trial set would settle it in an afternoon.

Extrapolation, labeled as such. If the nicotinic allosteric action is real and clinically relevant — a claim the label does not make Galantamine label 2021 — three consequences follow that nobody has tested: the effect should not be reproduced by a pure acetylcholinesterase inhibitor at matched enzyme inhibition, it should be blunted by a nicotinic antagonist, and it should show a dose-response distinct from the enzyme-inhibition curve. Three falsifiable predictions from the community's favorite fact about this molecule, and no published attempt at any of them.

Galantamine — its own safety story, not its class's

Three things this molecule owns, and the first is the reason the page exists in this form.

The excess-death paragraph applies to the people most likely to read this. Mild cognitive impairment is the clinical name for what a lot of people describe as memory being a step slower, and it is the reason a healthy adult reaches for a cholinergic drug in the first place. Tell an anesthetist before any surgery. The label states that galantamine exaggerates neuromuscular blocking effects during anesthesia Galantamine label 2021. Anyone taking this should say so before any procedure requiring a muscle relaxant.

The cardiac and pulmonary cautions are the class's, and they are not soft.
