A direct count of atherogenic particles. Every LDL, VLDL, IDL and Lp(a) particle carries exactly one ApoB molecule — so ApoB tells you how many plaque-causing particles are circulating, not just how much cholesterol they carry.

Arguably the most important cardiovascular marker you can run, and newly recommended in the US dyslipidemia guideline. It outperforms LDL-C at predicting events — especially in people with metabolic syndrome, diabetes, high triglycerides, or deceptively 'normal' LDL-C.

The plan of attack

In this order. Most people start at step four, which is why they change five things at once and learn nothing.

  1. Confirm the number is real
    Fasting state. Less affected by eating than a calculated LDL, which is one of its advantages — but it still shifts modestly. Fasted is preferable; consistency matters more.
  2. Read it with its partner
    Non-fasting is acceptable for ApoB. It's the marker to watch if you plan to be on hormones for decades. Draw it alongside: Lipoprotein(a) — Lp(a), Lipid Panel (Cholesterol, HDL, LDL, Triglycerides), hs-CRP (High-Sensitivity C-Reactive Protein).
  3. Work out which direction is yours
    If it's high — Increased atherosclerotic risk that compounds over years. Silent until it isn't.
  4. Fix it in this order
    Nutrition. Reduce saturated fat (the single biggest dietary lever on ApoB), increase soluble fiber (oats, psyllium, beans), add plant sterols, lose visceral fat, and replace saturated with mono/polyunsaturated fats.
    Lifestyle. Aerobic exercise, fat loss, smoking cessation, alcohol moderation.
    Supplements. Psyllium husk (~5–10% LDL reduction), plant sterols/stanols, berberine (meaningful LDL/ApoB reduction in trials), red yeast rice (contains natural monacolin K — same mechanism as a statin, so treat it like one), omega-3s primarily for triglycerides.
    Hormones. Statins, ezetimibe, bempedoic acid, PCSK9 inhibitors are the evidence-based tools and are extremely effective. If you're a lifelong optimizer using androgens, treating ApoB pharmacologically is often the rational trade — discuss with a physician. Correct hypothyroidism, which raises ApoB.
    Compounds. Anyone on TRT or AAS should be tracking ApoB, not just a standard lipid panel. Androgens — particularly orals — suppress HDL and raise ApoB. This marker tracks the actual long-term cost of your protocol.
    Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail.

The single most under-monitored consequence of androgen use. Androgens — especially oral 17-alpha-alkylated compounds — suppress HDL dramatically and raise ApoB. There are no symptoms; the cost is cumulative arterial damage measured in decades.

Many long-term users treat ApoB pharmacologically (statin/ezetimibe) with a physician — that's a rational trade if you're committed to being on hormones for years. Lp(a) is almost entirely genetic, largely unmoved by diet, and independently raises cardiovascular and aortic valve risk. A normal LDL alongside it is falsely reassuring.

Measure it once — it barely changes over a lifetime. You cannot lower it much yet, so the response is to drive everything else down hard: ApoB, blood pressure, insulin resistance, smoking. Tell your first-degree relatives to test theirs.

Cholesterol treated, inflammation ignored

Residual inflammatory risk. Plaque forms and destabilises through inflammation as well as lipids, so hitting an ApoB target while CRP stays high leaves a meaningful part of the risk untouched.

Rule out the obvious first — CRP rises with any infection, injury or hard training, so retest rested and well. If it stays up: sleep, visceral fat, periodontal disease, gut health, omega-3s. Persistent unexplained elevation deserves a real workup.
