Why this panel

Breast tissue in a man is driven by the RATIO of estrogen to androgen acting on the breast, not by either hormone alone — which is why it appears at a normal estradiol in a man whose testosterone has fallen, and at a normal testosterone in a man whose estradiol has risen. The list of causes is long and almost all of it is benign, but two entries on it are not, and both are found by cheap blood tests that nobody runs because the assumption is always that it is the cycle.

💡 What most people misshCG is the test that changes the stakes, and it is the one that gets left off. A small number of germ cell tumors secrete hCG, which stimulates the testis to aromatize testosterone into estradiol and produces gynecomastia as the first symptom. It is uncommon; it is also the reason guidelines put a quantitative hCG in the initial workup rather than after everything else has failed. If you are using hCG as part of a protocol, the result reports the drug — say so on the requisition, because an unexplained positive and a self-administered one look identical on paper. Prolactin and thyroid are the two treatable causes that get missed. A prolactin-secreting pituitary adenoma suppresses gonadotropins and lowers testosterone; thyrotoxicosis raises SHBG and shifts the free hormone balance toward estrogen. Both are found on this panel and neither needs surgery. The estradiol assay has to be the sensitive one. Standard immunoassays are unreliable at male concentrations, and an inaccurate estradiol is the number this whole question turns on.

Prolactin is the one that is easy to ruin: no nipple stimulation, no chest training and no sexual activity for 24 hours, and sit quietly for 20-30 minutes before the draw, because venepuncture stress alone raises it. If you are on TRT, draw at trough and record where you are in the injection cycle. If you have used hCG in the past three weeks, note it — the test cannot tell your dose from a tumor's output.

What this panel can settle, and by what logic

Eight markers arranged around one clinical guideline's initial workup, because the situation has a defined first set of tests and almost nobody gets them Pozza 2026.

  1. Estradiol, Sensitive (LC/MS-MS) with Total Testosterone and Free Testosterone is the ratio the tissue responds to, not either number alone. Breast tissue in a man is driven by the balance of estrogen to androgen acting locally, which is why it appears at a normal estradiol when testosterone has fallen and at a normal testosterone when estradiol has risen. The ratio has been reviewed as a measurement in its own right, including how badly it is affected by assay choice Swislocki 2024.
  2. The estradiol has to be mass spectrometry. Direct immunoassay estradiol is unreliable at male concentrations, and an Endocrine Society position statement was written specifically about the measurement problem Rosner 2013. This panel carries only the LC-MS/MS assay.
  3. Quantitative beta-hCG is the test that changes the stakes and the one that gets left off. A minority of germ cell tumors secrete hCG, which stimulates testicular aromatase and raises estradiol, and gynecomastia can be the presenting symptom. It sits in the guideline's initial biochemical workup rather than at the end of it Pozza 2026.
  4. LH & FSH localizes the problem in one pair of numbers. Low testosterone with raised LH and FSH points at the testes; low testosterone with low or normal LH and FSH points above them, and Prolactin is the first thing to read in that pattern Petersenn 2023.
  5. TSH (Thyroid-Stimulating Hormone) and Comprehensive Metabolic Panel (CMP) cover the two systemic causes that are treatable and cheap to exclude: thyrotoxicosis, which raises SHBG (Sex Hormone-Binding Globulin) and shifts the free hormone balance toward estrogen, and liver disease, which does the same thing by a different route.

What it cannot settle, and what would

It cannot tell you whether what you are feeling is glandular tissue or fat. That is the first question and it is answered by examination, with ultrasound where the examination is unclear Pozza 2026. Fat does not respond to any hormonal treatment, and buying a hormone panel for a chest that is simply fatter is the commonest way to waste this money.

It cannot date the tissue, and the date decides the treatment. Gynecomastia present beyond roughly a year has usually become fibrotic, at which point it stops responding to drugs regardless of what the hormones say Pozza 2026. A perfect panel does not make old tissue reversible.

A raised hCG here is not automatically a tumor, and that matters. Exogenous hCG used in a protocol reads as your own, and false positives from heterophile antibodies are a described and well-characterized problem with a defined resolution pathway Oyatogun 2021. Say what you have taken on the requisition, and ask for a repeat on a different platform before anything drastic happens.

And it cannot image anything. A detectable hCG in a man who has taken none leads to a testicular ultrasound; a markedly raised Prolactin leads to pituitary imaging, after macroprolactin has been excluded on the same sample Smith 2022. Both are the next step, and neither is a blood test.

Draw conditions that decide whether the money is wasted

Four conditions, and the Prolactin is the one that gets ruined most often.

  1. Morning, ideally 8 to 10am, fasted 9 to 12 hours. Testosterone follows a daily rhythm and an afternoon value is not comparable to the reference intervals it will be judged against.
  2. Nothing that stimulates prolactin for 24 hours: no nipple or chest stimulation, no heavy pressing, no sexual activity. Then sit quietly for 20 to 30 minutes before the needle, because the stress of the draw itself is a documented cause of a mildly raised result Petersenn 2023.
  3. If you are on testosterone, draw at trough and write the injection interval on the requisition. The same protocol produces completely different estradiol and testosterone numbers at peak and at trough Swislocki 2024.
  4. Declare any hCG used in the past three weeks. The assay cannot distinguish your vial from a tumor's output, and an undeclared positive starts an urgent pathway that did not need to start Oyatogun 2021.

How you would know it answered your question, and what each pattern means next

Five patterns, ordered by urgency rather than by likelihood.

  • Detectable hCG, Quantitative in a man who has taken none: repeat promptly on a different platform to exclude interference Oyatogun 2021, and see a clinician for a testicular examination and ultrasound without waiting for the rest of the panel to be interpreted.
  • Raised Estradiol, Sensitive (LC/MS-MS) with raised Total Testosterone on cycle: aromatization. The conversation is about the protocol, and the marker that should fall if a change works is the estradiol at the same trough timing.
  • Low testosterone with raised LH & FSH: a testicular problem. Recheck at 6 to 8 weeks; a persistent pattern is a referral Pozza 2026.
  • Low testosterone with low LH & FSH and a raised Prolactin: ask the laboratory to screen the same sample for macroprolactin before anything else Smith 2022, then a clinician.
  • Everything normal: common, and informative. It moves the question to the examination and the timeline, which is where gynecomastia is actually settled Pozza 2026.
