SHBG is the hidden variable behind most confusing hormone panels — and an independent metabolic marker in its own right. Low SHBG is a well-validated early signal of insulin resistance, often abnormal years before glucose or HbA1c move.
The plan of attack
In this order. Most people start at step four, which is why they change five things at once and learn nothing.
- Confirm the number is real
Route and timing of any oestrogen. Oral oestrogen raises SHBG two- to fourfold through first-pass liver metabolism; transdermal barely touches it. So the same hormone at the same dose produces completely different SHBG depending only on how it was taken. Record the route, not just the hormone — an SHBG drawn on oral oestrogen cannot be compared to one drawn off it. - Read it with its partner
Never interpret SHBG alone — pair with fasting insulin. Low SHBG + high fasting insulin is one of the clearest early metabolic-dysfunction signatures in bloodwork. Draw it alongside: Fasting Insulin, Free Testosterone, Total Testosterone. - Work out which direction is yours
If it's high — Lowers free testosterone despite a normal total T. Investigate thyroid, liver, and whether the person is under-eating.
If it's low — Treat as a metabolic warning sign. Strongly associated with type 2 diabetes risk in Mendelian randomization data — i.e. likely causal, not just correlated. - Fix it in this order
Nutrition. Low SHBG: reduce refined carbohydrate and total energy excess, lose visceral fat, add fiber — all improve insulin sensitivity and raise SHBG. High SHBG: stop under-eating, restore dietary fat and calories.
Lifestyle. Resistance training + zone-2 cardio, sleep, and fat loss are the durable levers for raising low SHBG.
Supplements. Low SHBG is best treated by fixing insulin resistance: berberine, inositol, magnesium, omega-3s. For high SHBG, boron lowers it modestly.
Hormones. Thyroid hormone raises SHBG; androgens lower it. If SHBG is very low, screen for fatty liver and insulin resistance before assuming it's 'just' androgens.
Compounds. GH secretagogues (incl. MK-677) can lower SHBG — free T may drift up without total T changing. GLP-1s that improve insulin sensitivity will typically raise a suppressed SHBG.
Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail. - Retest
Every 3–6 months alongside testosterone. Change one thing at a time, or the retest can't tell you which thing worked.

Investigate why SHBG is high: aggressive dieting, very-low-fat intake, hyperthyroidism, liver disease, aging, anticonvulsants. Often the fix is eating more and adding dietary fat rather than adding testosterone.
The classic polycystic ovary syndrome pattern, with insulin resistance as the usual engine driving androgen excess.
