How DHEA actually works

An adrenal prohormone that converts peripherally to both testosterone and estradiol, with the direction depending on tissue enzyme expression — aromatase-rich tissue makes estrogen, 5-alpha-reductase-rich tissue makes androgen. It also has direct activity as a neurosteroid at GABA-A and NMDA receptors, independent of its conversion products. Levels peak in the twenties and fall steeply with age.

How to read these tiers

These tiers tell you how much human evidence exists — not how well something works. This is the research space, and most of what’s in here is new rather than disproven. Something sitting at “theoretical” usually means nobody has funded the trial, not that the trial was run and failed.

The trap runs the other way too: something can be clinically validated and still do very little for you specifically. A statistically significant result in a study population is not a promise about your body.

A compound tested for one purpose and found not to help there can still be worth studying somewhere else, so a negative result never gets rendered as a cross. The best evidence is in women rather than men: vaginal DHEA is an approved treatment for genitourinary syndrome of menopause, and oral DHEA improves wellbeing and libido in adrenal insufficiency. In men with functioning testes it mostly aromatises. Because it converts to both hormones it is genuinely contraindicated in hormone-sensitive cancers, and it is banned in most sport. Test DHEA-S before and after rather than dosing blind.
