Men need estradiol. It's essential for bone density, libido, erectile function, joint comfort, lipids and cognition. The 'crush estrogen' reflex in the TRT world causes real, documented harm.

In this order. Most people start at step four, which is why they change five things at once and learn nothing.

  1. Confirm the number is real
    Standard immunoassay in men. The routine oestradiol assay was validated at female concentrations and reads unreliably in the male range. A great deal of unnecessary aromatase-inhibitor use traces back to a bad number from the wrong assay. In men, insist on the sensitive LC/MS-MS assay. Do not act on a standard immunoassay result.
  2. Read it with its partner
    In men, always order the sensitive LC/MS-MS assay. The standard ECLIA assay is unreliable at male concentrations and has driven countless unnecessary AI prescriptions. Draw it alongside: Total Testosterone, Free Testosterone, SHBG (Sex Hormone-Binding Globulin).
  3. Work out which direction is yours
    If it's high — Water retention, moodiness, nipple sensitivity in men. But many symptoms blamed on 'high E2' are actually low E2 or a poor T:E2 ratio — don't reflexively reach for an AI.
    If it's low — Joint pain and 'dry' joints, low libido, poor erections, flat affect, and over time reduced bone mineral density.
  4. Fix it in this order
    Nutrition. Reduce body fat to lower aromatase activity. Cut alcohol — it both raises estradiol and impairs hepatic clearance. Cruciferous vegetables (DIM/I3C precursors) support estrogen metabolism modestly.
    Lifestyle. Fat loss, alcohol reduction, and — for men on TRT — more frequent, smaller injections to smooth peaks that drive aromatization.
    Supplements. DIM or calcium-D-glucarate support estrogen metabolism/clearance gently — far preferable to an AI as a first move. Zinc has weak aromatase-inhibiting activity.
    Hormones. Aromatase inhibitors (anastrozole) should be a last resort, at the lowest effective dose, and only with sensitive E2 confirming true elevation plus symptoms. Over-suppression is more common and more harmful than high E2 in practice. For women, HRT decisions belong with a qualified provider.
    Compounds. Any protocol that raises testosterone raises estradiol downstream — including hCG, which stimulates intratesticular aromatization notably.
    Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail.
  5. Retest
    6–8 weeks after any TRT or AI change. Change one thing at a time, or the retest can't tell you which thing worked.

🔬 Biotin (B7)The number is wrong — repeat it

Biotin interferes with the streptavidin-biotin immunoassays most hormone tests run on. Depending on the assay design it pushes the result falsely HIGH or falsely LOW — there is no single direction to correct for. Hair, skin and nail products are the usual source, and doses in those are far above the RDA.

Stop biotin 48–72 hours before the draw, and check every supplement label rather than just the one labelled biotin.

Post-cycle: the axis has not come back

Suppressed HPTA that has not restarted. The longer it sits shut down, the harder recovery gets — and "waiting it out" past a few months is usually not a plan.

The ovary is becoming less responsive, so FSH climbs to push it. Oestradiol does not fall smoothly — it spikes and crashes, which is why symptoms are worse than a single "normal" oestradiol suggests.
