A pituitary hormone best known for lactation, but clinically important in both sexes — elevation suppresses the entire reproductive axis.

High prolactin is a correctable cause of low testosterone, low libido and ED that gets missed when only testosterone is measured. In women it causes cycle irregularity and infertility.

What Prolactin actually measures — the analyte, and the assay

The analyte is prolactin, a single-chain polypeptide of about 23 kDa secreted by pituitary lactotrophs. It is measured by a two-site sandwich immunoassay, and the whole difficulty of this page comes from one fact: prolactin circulates in more than one size, and the antibodies cannot all tell them apart.

Three species share the blood. Monomeric prolactin, the bioactive one. A covalent dimer, sometimes called big prolactin. And macroprolactin — prolactin bound to an immunoglobulin into a complex of roughly 150 kDa that is too large to leave the circulation efficiently, accumulates, and is biologically inert. An assay that counts it reports hyperprolactinemia in a person who does not have any.

How common is that? One in seven. In 1,229 consecutive people found to have a prolactin above 18 ng/mL, 168 (13.7%) had macroprolactinemia on polyethylene glycol precipitation, defined as recovery of under 40% of the original result — 15.03% of the women and 6.28% of the men Sharma 2021. That is not a rare artifact. It is the second commonest explanation for a mildly raised prolactin.

And how much of it your report contains is a property of the antibody, not of you. Screening 670 hyperprolactinemic sera, one platform left 24.6% of them with a normal monomeric prolactin after PEG treatment while another put the prevalence at 6.2%; at a common screening threshold of 700 mU/L, the sensitivity for detecting macroprolactinemia was 90% on the Roche assay and 59% on the Tosoh Smith 2022. Two CE-marked prolactin assays, the same sera, and a different answer to the question ‘does this person have too much prolactin’.

The opposite failure exists too, and it is worse. In a very large prolactinoma, antigen can saturate both the capture and detection antibodies so that neither sandwich forms — the hook effect — and the reported prolactin comes back near normal. A 2026 systematic review pooled 23 documented cases from 14 studies in 9 countries: median prolactin before dilution was 100.0 ng/mL and after dilution 6,460.0 ng/mL, a median 64.7-fold underestimate Yadav 2026. So the practical instruction is exact: if there is a large pituitary mass and the prolactin is only modestly raised, ask the laboratory for a serial dilution before anyone operates.

Prolactin: what changes the blood, and what only changes the reading

What changes the prolactin in your blood — and this list starts with things that happened in the hour before the needle:

  1. Stress, including the venipuncture itself. Prolactin is a stress-responsive pituitary hormone; a difficult draw can raise it. Within-subject variation measured on weekly samples in 38 men was 13%, higher than FSH, testosterone or DHEA-S in the same study Itkonen 2024.
  2. Sleep and time of day. Prolactin peaks during sleep and falls through the morning, so an early draw after a broken night reads high.
  3. Nipple or chest-wall stimulation, and recent orgasm. Genuine physiological release, not artifact — which is why the retest conditions on this page are as specific as they are.
  4. Dopamine antagonism. Prolactin is under tonic dopaminergic inhibition, so antipsychotics, metoclopramide, some antidepressants and opioids raise it by removing the brake. Drug-induced hyperprolactinemia is a leading cause in unselected series Sharma 2021.
  5. Primary hypothyroidism, where elevated TRH drives lactotroph output — a correctable cause found by a TSH, not by a pituitary MRI.
  6. A lactotroph adenoma, which is the diagnosis everything else on this list has to be excluded before you reach Petersenn 2023.
  7. 19-nor compounds such as nandrolone, through progesterone-receptor agonism.

What changes only the reading:

  1. Macroprolactin. The dominant analytical term on this marker: 13.7% of hyperprolactinemic people in a 1,229-patient series Sharma 2021, and a prevalence that itself changes with the platform Smith 2022.
  2. The hook effect, which drives the number DOWN rather than up and did so by a median factor of 64.7 in reviewed cases Yadav 2026.
  3. Biotin on streptavidin platforms and heterophile antibodies, both of which are recognized causes of a prolactin that does not match the patient Alvarez-Payares 2021.
  4. Which assay, since cross-reactivity with the macro form differs between manufacturers by design Smith 2022.

Reference interval or decision threshold — which kind of number Prolactin is

A sex-specific reference interval — roughly 4–15.2 ng/mL in men and 4.8–23.3 ng/mL in non-pregnant women — and it is the central 95% of a measured population on one analyzer, with no outcome attached to any value inside it.

What makes this marker unusual is that the interval sits on top of a population that includes people whose prolactin is inert. A screening study of 670 hyperprolactinemic sera found that on one platform 24.6% had a normal monomeric prolactin once the macro form was precipitated Smith 2022. The printed upper limit is therefore partly a statement about how much macroprolactin that manufacturer's antibody sees.

There are two genuine decision thresholds here and neither is on your report. The first is the PEG recovery cut-off: recovery below 40% after polyethylene glycol precipitation defines macroprolactinemia and is the point at which the rest of the workup stops Sharma 2021. The second is the screening threshold a laboratory chooses for reflex PEG testing — at 700 mU/L, sensitivity was 90% on one assay and 59% on another, and raising the threshold cut workload by 45% and 37% respectively Smith 2022. Both are decisions about laboratory policy that silently determine whether your mildly raised prolactin is ever investigated.

Beyond that, interpretation is consensus-driven rather than threshold-driven: the Pituitary Society's international consensus statement is the document that governs how a raised prolactin is worked up and treated Petersenn 2023.

How you would know your Prolactin was wrong — and when to redraw

Repeat any mildly elevated prolactin before anything else happens. That is not caution, it is the arithmetic: a stress-responsive hormone with 13% within-subject variation Itkonen 2024 needs roughly a 36% difference between two draws before chance stops being a good explanation. The redraw is cheap and the alternative — an MRI ordered on a single number — is not.

Conditions for the second draw: rested, at least an hour after waking and not immediately after arriving in a hurry; no nipple or chest-wall stimulation and no orgasm in the preceding 24 hours; and the same laboratory, because cross-reactivity with the macro form is assay-specific Smith 2022.

Two questions to put to the laboratory rather than to a scanner: ask for PEG precipitation if the elevation is mild, and ask for a serial dilution if there is a known pituitary mass and the prolactin seems too low for its size Yadav 2026.

What would have to change alongside it:

  • TSH and free T4, because primary hypothyroidism is a reversible driver and treating it can normalize the prolactin.
  • Total testosterone and LH/FSH in a man, because a prolactin high enough to matter suppresses the axis — a raised prolactin with entirely normal gonadotropins and testosterone is biologically inconsistent and points at macroprolactin.
  • Estradiol and cycle history in a woman, for the same reason.

That last line is the falsifiable prediction and it is the most useful sentence on the page: real hyperprolactinemia has downstream consequences, so if LH/FSH and total testosterone are untouched, suspect the assay before the pituitary.

What Prolactin cannot tell you

A single raised prolactin cannot diagnose anything. Between stress, sleep, drugs, hypothyroidism and macroprolactin, the prior probability that one mildly high value represents a lactotroph adenoma is low — 13.7% of one large hyperprolactinemic series was macroprolactin alone Sharma 2021.

It cannot tell you the size of a tumor, and a normal-ish value does not exclude a large one. The hook effect underestimated prolactin by a median factor of 64.7 in the reviewed cases, 82.6% of which presented with a pre-dilution result of only 26 to 200 ng/mL — and failure to recognize it led to unnecessary surgery in 55.6% of the documented cases Yadav 2026.

It cannot separate bioactive from inert prolactin without PEG. The routine number is a total, and the fraction that matters is the monomer Smith 2022.

A low prolactin is not a finding. There is no deficiency syndrome to chase outside the postpartum setting.

The wrong inference runs in both directions here, which is unusual. Readers treat a mildly high prolactin as a pituitary tumor, when it is more often a drug, a stressful draw, an underactive thyroid or an antibody complex; and they treat a mid-range prolactin as reassurance in someone with a large sellar mass, when it may be the hook effect Yadav 2026. Both errors are fixed by asking the laboratory a question rather than the number.

The plan of attack

In this order. Most people start at step four, which is why they change five things at once and learn nothing.

  1. Confirm the number is real
    Macroprolactin. A biologically inactive prolactin-antibody complex that the assay counts as prolactin. It causes a substantial minority of raised results, in people with no symptoms at all — and has led to unnecessary pituitary imaging. Ask the lab to check for macroprolactin before any scan.
  2. Read it with its partner
    Any unexplained low libido or ED despite adequate testosterone should trigger a prolactin check — it's cheap and frequently the answer. Draw it alongside: Total Testosterone, LH & FSH, TSH (Thyroid-Stimulating Hormone).
  3. Work out which direction is yours
    If it's high — Low libido, ED, infertility, galactorrhea, cycle irregularity. Markedly elevated values warrant pituitary MRI. Note prolactin rises with stress and even the venipuncture itself — repeat a mildly high value before acting.
    If it's low — Rarely clinically significant.
  4. Fix it in this order
    Nutrition. No direct dietary lever. Correct hypothyroidism, which is a common secondary driver.
    Lifestyle. Reduce stress, address sleep. Re-draw fasted, rested, and having avoided orgasm/nipple stimulation for 24h.
    Supplements. Vitamin B6 (P5P) has modest dopaminergic prolactin-lowering effects. Zinc supports dopamine. Vitex is used in women but should be discussed with a provider.
    Hormones. Dopamine agonists (cabergoline, bromocriptine) are the medical treatment — prescription only, with real side effects, and not something to self-administer. Treat underlying hypothyroidism first.
    Compounds. Relevant for anyone who has used 19-nor compounds. No peptide lowers prolactin.
    Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail.
  5. Retest
    Repeat any elevated value before acting; then per provider. Change one thing at a time, or the retest can't tell you which thing worked.
