Why this panel

AAS use has four well-characterized organ risks: blood viscosity, cardiovascular (specifically a collapse in HDL and a rise in ApoB), liver — especially with orals — and kidney. All four show up in blood before they show up as symptoms.

💡 What most people missHematocrit is the one most likely to hurt you soonest, and it's on a $9 CBC. Rising hematocrit thickens blood and raises clot risk, and it climbs quietly. Alongside it: AAS can crush HDL by 50–70%, and ApoB tells you the real damage better than the standard lipid panel does. On kidney markers — creatinine-based eGFR is unreliable in anyone carrying significant muscle, which is why cystatin C is on this panel; it isn't affected by muscle mass and will save you an unnecessary kidney scare. Liver enzymes have the mirror-image problem: AST and ALT rise from hard training alone, so a raised ALT without a raised GGT is often muscle, not liver.

What this panel can settle, and by what logic

Eleven markers covering four organ risks. The value is in three pairings where one marker corrects the other, because on this protocol half the standard panel reads wrong for reasons that are not disease.

  1. Hematocrit on the Complete Blood Count (CBC) with Differential, read as a trajectory rather than a level. Androgen-driven erythrocytosis is dose- and exposure-related, and in long-term testosterone use the proportion crossing each successive hematocrit threshold falls off sharply Madsen 2021. What that means practically is that the number to watch is the slope between two draws, not whether today's value cleared a cut-off. Drug-induced erythrocytosis is a recognized entity with a defined work-up when it does not settle Liu 2025.
  2. Cystatin C with eGFR against the creatinine on the Comprehensive Metabolic Panel (CMP) separates muscle from kidney. In this population the creatinine-based estimate is systematically pessimistic, and reviews of bodybuilding practice find little evidence that high protein intake or moderate creatine harms normal kidneys Tidmas 2022. Where the two eGFR estimates disagree, the discordance identifies which one to trust Wang 2023.
  3. ALT and AST on the Comprehensive Metabolic Panel (CMP) read against GGT. Oral 17-alkylated compounds are genuinely hepatotoxic, but heavy training raises AST and ALT from muscle. GGT does not come from muscle, so a raised ALT with a flat GGT is usually the gym and a raised ALT with a raised GGT is usually the tablets Basheer 2025.
  4. Total Testosterone by mass spectrometry with a sensitive Estradiol, Sensitive (LC/MS-MS). At supraphysiologic concentrations, and for estradiol at male concentrations, the recommended methods are chromatographic rather than immunoassay Casals 2023, which is why this panel specifies them and why cheaper versions of the same tests are not the same test.

What it cannot settle, and what would

It cannot tell you what was actually in the vial, and that is the largest unknown in the entire protocol. Pooling 19 studies and 5,413 black-market anabolic samples, the mean estimate for counterfeit product was 36% (95% CI 29 to 43), with a further 37% (95% CI 17 to 63) substandard Magnolini 2022. Products contained no active ingredient, a different active ingredient, or a different amount than the label claimed. A Total Testosterone of 2,400 ng/dL tells you that something androgenic is present at a high concentration. It does not tell you which molecule, at what dose, or what else came with it. The only test that answers that is an analysis of the product itself.

It cannot see the cardiac remodeling that is the main long-term risk. Left ventricular hypertrophy and reduced systolic function are echocardiogram findings. ApoB (Apolipoprotein B) and the Lipid Panel (Cholesterol, HDL, LDL, Triglycerides) describe the atherogenic side of the risk and say nothing about the muscle of the heart Basheer 2025.

An immunoassay result at these concentrations can be wrong in either direction. Apparent hyperandrogenemia caused purely by immunoassay interference, and resolved only when the sample was rerun by mass spectrometry, is documented Huang 2025. If a number does not fit the protocol, suspect the method before rewriting the protocol.

And it cannot make anything safe. Every marker here is a detector, not a control. Monitoring finds hematocrit rising and lipids collapsing earlier than symptoms would; it does not prevent either, and a clean panel at week 6 is not evidence that a compound is being tolerated at week 30 Liu 2025.

Draw conditions that decide whether the money is wasted

On this protocol the draw conditions decide whether half the panel is readable, and two of them are unique to this page.

  1. 72 hours clear of any hard session. AST, ALT and creatinine all rise from muscle work Tidmas 2022, and this is the audience most likely to train the day before a draw. A leg session on Tuesday produces a liver flag and a kidney flag on Wednesday in somebody with neither problem.
  2. Fix the draw to the same point in the injection cycle every time. Serum concentrations of injected esters peak and trough across the week, so a trough draw and a peak draw differ by more than most dose changes do Basheer 2025. Trough, immediately before the next injection, is the reproducible choice.
  3. Insist on mass spectrometry for testosterone and a sensitive assay for estradiol. Direct immunoassays are not the recommended method at these concentrations Casals 2023, and a wrong estradiol is the number most likely to trigger an unnecessary aromatase inhibitor Huang 2025.
  4. No biotin for 3 days, and hydrate normally. TSH (Thyroid-Stimulating Hormone), Prolactin and PSA (Total + Free + % Free) are commonly streptavidin-based, and arriving dehydrated raises hematocrit by concentration alone, which is the single number on this panel most likely to prompt a blood donation.

How you would know it answered your question, and what each pattern means next

Baseline, mid-cycle, and 4 to 6 weeks after finishing. Five patterns, and only two of them wait.

  • Hematocrit climbing more than about 3 points between draws: act on the slope rather than the threshold, because the trajectory is what predicts where you finish Madsen 2021. Dose reduction first; if it does not settle, the work-up for drug-induced erythrocytosis includes looking for the causes that are not the drug Liu 2025.
  • HDL collapsed with ApoB (Apolipoprotein B) rising: the cardiovascular signal, and it is the one that persists. Retest 6 to 8 weeks after orals are stopped, because hepatic lipid handling recovers over weeks rather than days Basheer 2025.
  • ALT raised with a normal GGT and a normal bilirubin: repeat at 72 hours of rest before assuming the liver. If it normalizes, it was muscle Tidmas 2022.
  • Cystatin C with eGFR rising while creatinine is stable: this is the real kidney signal and the one that outranks the creatinine Wang 2023. It goes to a doctor.
  • Everything acceptable: the panel has done its job and it still cannot tell you what you injected Magnolini 2022. The next meaningful test is not a blood test at all.
