Why this panel

PSA on its own is a blunt instrument that causes a great deal of unnecessary anxiety and a number of unnecessary biopsies. Measuring free PSA alongside total makes the result far more informative.

💡 What most people missPercent-free PSA is what separates benign enlargement from something that needs investigating, and most panels only order total PSA. Cancerous tissue produces proportionally more bound PSA — so a low percentage free (roughly under 10–15%) carries substantially higher risk than a high percentage, even at the same total. The other thing that catches men out: 5-alpha reductase inhibitors (finasteride, dutasteride) roughly halve PSA. If you take one for hair loss or BPH, your reading must be doubled to be interpreted — and forgetting this has delayed real diagnoses.
