How L-Arginine actually works

The direct substrate for nitric oxide synthase. The problem is first-pass metabolism — intestinal and hepatic arginase degrade most of an oral dose before it reaches circulation, which is why plasma arginine rises far less than the dose suggests. Sustained-release formulations partially address this by spreading absorption rather than saturating the enzyme.

What L-Arginine actually does

Arginine is the nitrogen donor for nitric oxide, and the enzyme that uses it is not the enzyme that most of a dose meets. Endothelial nitric oxide synthase oxidizes the guanidino nitrogen of L-arginine to give nitric oxide and citrulline, using tetrahydrobiopterin, FAD, FMN and heme. That reaction is the entire basis for the product.

The competing enzyme is arginase and it wins. Arginase hydrolyzes arginine to ornithine and urea, has a much higher maximum velocity than nitric oxide synthase, and is abundant in the small intestine and the liver. An oral arginine dose is therefore attacked twice before it reaches the circulation, which is why plasma arginine rises far less than the dose implies.

The paradox that made this a real research question is that the enzyme is already saturated. The Michaelis constant of endothelial nitric oxide synthase for arginine is in the low micromolar range and plasma arginine sits at roughly 60 to 100 micromolar. On paper the enzyme cannot be substrate-limited, and yet arginine sometimes improves endothelial function. Proposed resolutions include compartmentalized intracellular arginine pools and competition from an endogenous inhibitor.

That inhibitor is measurable and it is the more interesting target. Asymmetric dimethylarginine, produced when methylated arginine residues are released from proteins, competes at the same active site and is cleared by dimethylarginine dimethylaminohydrolase. Raised asymmetric dimethylarginine is associated with endothelial dysfunction, and the arginine-to-asymmetric-dimethylarginine ratio is a better predictor than either alone da Silva 2023.

Citrulline solves the delivery problem by taking a different road. Citrulline is not an arginase substrate, escapes intestinal and hepatic degradation, and is converted to arginine in the kidney by argininosuccinate synthase and lyase. Gram for gram it raises plasma arginine more than arginine does, which is one of the cleanest form-versus-molecule results in this catalog Smeets 2021.

Cell, rodent, human — and where it stops

The marker on this page is plasma arginine, the product raises it poorly, and the alternative that raises it better has the better-controlled outcome data.

In cells and in acute human physiology the pathway transfers. Intravenous or intra-arterial arginine produces vasodilation and improves flow-mediated dilation acutely, which established that the substrate can be limiting under some conditions. The peptide chemistry of arginine and its analogs has been reviewed in detail Kluczyk 2021.

Chronic oral supplementation has produced a mixed and mostly modest literature. A critical review of vasoactive nutrients for endothelial dysfunction and arterial stiffness places arginine among interventions whose mechanistic case is stronger than their trial record da Silva 2023.

The comparison that matters has been done and arginine loses. A meta-analysis of randomized controlled trials of citrulline supplementation and watermelon consumption on longer-term and postprandial vascular function found effects on vascular and cardiometabolic markers Smeets 2021, and a randomized trial in hypertensive postmenopausal women examined citrulline on endothelial function and blood pressure directly Maharaj 2022. The precursor of the precursor works better than the precursor.

Where oral arginine does have an outcome literature it is obstetric. A systematic review and meta-analysis found L-arginine efficacy for preventing preeclampsia and improving maternal and neonatal outcomes in high-risk pregnancies Naderipour 2024. That is a clinical endpoint in a defined population under care, and it is not a fitness claim.

The obstacle to transfer for the sexual-function claim is that the trials are combinations. A systematic review and meta-analysis of L-arginine AND Pycnogenol in male erectile dysfunction reports on the combination Tian 2023, which cannot attribute the effect to either component. That is the shape of most of the arginine literature outside pregnancy.

L-Arginine — which form, and does it matter

Free base, hydrochloride and alpha-ketoglutarate are three salts and none of them fixes the enzyme problem. L-arginine hydrochloride is roughly 83 percent arginine by weight and is acidic enough to be unpleasant; the free base is better tolerated; alpha-ketoglutarate adds a citric acid cycle intermediate whose contribution has never been isolated. All three meet the same intestinal arginase.

The exposure numbers are the whole argument for buying something else. Oral bioavailability of L-arginine is roughly 20 to 40 percent, because 40 percent or more of an oral dose is consumed by intestinal arginase and much of the rest by hepatic first-pass metabolism. Peak plasma concentration arrives at about 1 to 2 hours and the plasma half-life is roughly 1 to 2 hours, with renal clearance and continued arginase hydrolysis both contributing. There is no cytochrome step.

Citrulline's numbers are better at every stage. It is not an arginase substrate, has oral bioavailability above 70 percent, and raises plasma arginine higher and for longer than an equal dose of arginine. If the goal is plasma arginine, the correct product is citrulline, and that has been shown repeatedly Smeets 2021 Maharaj 2022.

The dose split is a tolerability constraint rather than a pharmacological choice. Single doses above roughly 9 g reliably produce osmotic diarrhea, which caps what can be given at once; splitting 3 to 6 g twice or three times daily is how the trials handled it.

What the label does not tell you is the ratio that matters. Endothelial performance tracks with the arginine to asymmetric dimethylarginine ratio rather than with arginine alone da Silva 2023, and no product declares anything about the inhibitor. That measurement is available as ADMA & SDMA on specialty panels and is almost never ordered.

What would have to be true, and how you would know it was not

1. Predict plasma arginine rises modestly and citrulline raises it more. Predict that 6 g of oral arginine produces a smaller and shorter plasma arginine rise than 6 g of citrulline, because intestinal and hepatic arginase consume the first and not the second Smeets 2021. That is directly testable and it is the central form claim.

2. Predict the endothelial read-out is flow-mediated dilation. Predict a measurable improvement in flow-mediated dilation over 4 to 8 weeks in someone with impaired baseline endothelial function, and none in a healthy young adult da Silva 2023 Maharaj 2022. Retest at 8 weeks.

3. The prediction that cuts against the product. Predict no meaningful change in resting blood pressure or in exercise performance in a healthy trained adult on 6 g a day, because the enzyme is not substrate-limited at normal plasma arginine. A well-powered trial in healthy adults showing a performance benefit would falsify this page.

4. Predict ADMA is the marker that identifies a responder. Predict that people with a high asymmetric dimethylarginine and a low arginine-to-ADMA ratio respond and people with a normal ratio do not da Silva 2023. No trial has stratified on it, which makes this a prediction rather than a finding.

5. Predict the combination trials cannot be attributed. Predict that a trial of arginine with Pycnogenol cannot tell you what arginine contributed Tian 2023, and that a factorial design would show most of the effect belongs to one component or to neither.

What nobody has tested yet

Nobody has run the head-to-head that would settle the category. Arginine against citrulline at matched molar dose, with flow-mediated dilation as the endpoint, in a population with impaired endothelial function. Everything needed for that trial exists Smeets 2021 Maharaj 2022.

The arginine paradox has never been resolved in a human. Whether intracellular compartmentalization, ADMA competition or arginase induction explains why a saturated enzyme responds to more substrate is still argued from indirect evidence da Silva 2023.

The post-myocardial-infarction signal has never been explained. A trial of arginine after myocardial infarction was stopped for excess mortality in the treatment arm, and no mechanism has been established. That single result is why this page does not recommend arginine to anybody with established coronary disease.

And the preeclampsia result deserves a confirmatory trial. A meta-analysis reporting benefit on maternal and neonatal outcomes Naderipour 2024 is the strongest outcome evidence this molecule has, and it pools small trials in a high-stakes setting. A large confirmatory trial would either establish a genuine clinical use or close it.

L-Arginine — its own safety story, not its category's

The dose-limiting effect is osmotic and it arrives predictably. Nausea, bloating, cramping and diarrhea at single doses above roughly 9 g, which is how most people find their ceiling.

The serious signal is in established coronary disease and it is the reason for the strongest caution on this page. A randomized trial of L-arginine after myocardial infarction was terminated early because of higher mortality in the arginine arm. The mechanism is unresolved and the finding has not been overturned; anybody with established coronary disease should not be self-prescribing this.

Herpes reactivation is a plausible and frequently cited concern. Herpes simplex replication requires arginine and is inhibited by lysine competing for the same transporter, which is the basis for the lysine-to-arginine ratio advice. The clinical evidence is weak in both directions; the mechanism is real.

The pharmacodynamic interactions are all vasodilatory. Nitrates, phosphodiesterase-5 inhibitors and antihypertensives all add to the blood pressure effect. Arginine also stimulates growth hormone and insulin secretion at high intravenous doses, which is a diagnostic use rather than an oral effect but is worth knowing in anyone on insulin.

Who should be careful, and one honest recommendation. Anyone with coronary artery disease, anyone with a recent myocardial infarction, anyone with active herpes disease, and anyone on a nitrate. And anyone whose actual goal is plasma arginine, who should be looking at citrulline instead Smeets 2021. How you would know if it worked

The ADMA page on this site puts the problem with this product in writing: it prescribes L-citrulline at 6-8 g and says so specifically because citrulline survives first-pass metabolism, which arginine largely does not. So the honest prediction for oral arginine is a smaller move in the same direction, and ADMA — the methylated arginine that competitively inhibits nitric oxide synthase — is the number that would show it. Homocysteine belongs beside it because raising it raises ADMA, which means a reader with high homocysteine and low folate is looking at the cheaper fix for the same pathway. If ADMA has not moved at 6 months, switch to citrulline rather than raising the arginine dose.
