How Serrapeptase actually works
Serrapeptase is a proteolytic enzyme originally isolated from bacteria in the silkworm gut. The proposed mechanism is systemic breakdown of non-living proteins — fibrin, inflammatory debris and the protein matrix of mucus — after absorption from the intestine. That last step is the weak link in the whole story: it's an enzyme, meaning a large protein, and the gut exists specifically to digest those. Enteric coating is used to get it past stomach acid, but how much intact enzyme reaches circulation in humans remains poorly demonstrated.

A compound tested for one purpose and found not to help there can still be worth studying somewhere else, so a negative result never gets rendered as a cross. It sits alongside the tier, because something can be both well-studied and genuinely risky.

Popular well beyond what its evidence supports, and worth being straight about. Most of the human work is old, small and from a narrow set of sources, and the absorption question underneath it has never been resolved convincingly. That doesn't make it useless — unproven is not the same as ineffective, and the sinus and swelling reports are consistent enough to be interesting — but it does mean it shouldn't sit alongside creatine in your confidence ranking. - GI upset, and occasionally cough or rash.
- Mild fibrinolytic and antiplatelet activity — Additive bleeding risk with anticoagulants and antiplatelets — warfarin, apixaban, rivaroxaban, clopidogrel, and aspirin at any dose. The interaction is pharmacodynamic rather than metabolic, so it does not show up as a changed drug level; it shows up as bruising, nosebleeds or a raised INR.
- Pneumonitis and severe lung reactions have been reported, rarely, and are the reason to stop immediately for unexplained breathlessness or persistent cough.
- Enteric coating is essential or stomach acid destroys it. Stop at least two weeks before any surgery or dental extraction. Surgeons ask about prescription blood thinners and rarely about supplements, so this one is on you to volunteer.
