What Diosmin actually does
Diosmin is one hydrogen away from hesperidin, and it is mostly made from it. Hesperidin is hesperetin-7-O-rutinoside, a flavanone. Diosmin is diosmetin-7-O-rutinoside, a flavone: the same rutinose sugar, the same 7-position, but a C2–C3 double bond in the C ring that hesperetin does not have. Industrially, most diosmin is made by dehydrogenating hesperidin extracted from citrus peel. So the phrase citrus flavonoid on the bottle is chemically true and commercially misleading: this is a semi-synthetic molecule whose starting material is a different supplement sold in the next aisle.
What the extra double bond does. It flattens the C ring and conjugates it with the A ring, making diosmetin planar where hesperetin is puckered. Planar flavones stack more readily, are less water-soluble, and bind flat protein surfaces differently. That is the structural reason the two molecules are not interchangeable at the vein wall, and it is why the venous evidence attached itself to this one.
The vein-wall mechanism, named at the level it is argued. Two claims, and they are separable. The first is venous tone: prolongation of noradrenaline's constrictor action on the vein, attributed to reduced catabolism of the transmitter in the adventitia, which raises the pressure the vein can hold against a column of blood. The second is microvascular, and it is the more interesting one: reduction of leukocyte–endothelium adhesion through the selectins and ICAM-1 and VCAM-1, interrupting the leukocyte trapping that releases proteases into the capillary basement membrane and produces the leak Li 2021Kakkos 2018. There is a third claim, lymphatic: increased frequency and amplitude of lymphatic contraction, which matters because leg swelling is a balance between capillary filtration and lymphatic clearance Li 2021.
The concentration the effect was shown at, and it is extraordinary. The direct endothelial-cell work on this molecule was run on human umbilical vein cells and on endothelial cells taken from actual varicose veins, at diosmin concentrations of 1 and 10 mg/mL Gwozdzinski 2023. Diosmin's molecular weight is 608.5, so 10 mg/mL is roughly 16 millimolar. For scale, the plasma concentrations reached by dietary polyphenols after oral doses are measured in nanomoles to low micromoles per liter Manach 2005. That is a gap of four to five orders of magnitude between the dish and the person — the widest on this site, and it belongs to the venous product with the best clinical evidence, which is a genuinely uncomfortable pair of facts to hold together.
Cell, rodent, human — and where it stops
In cells. Human umbilical vein endothelial cells and endothelial cells harvested from varicose veins, exposed to diosmin at 1 and 10 mg/mL, at which concentrations viability was unaffected Gwozdzinski 2023.
In animals. This is the weak link in the chain and it is worth saying so rather than glossing it. The preclinical case for this class rests on microcirculatory models of venous hypertension and ischemia-reperfusion, summarized in reviews Li 2021Kakkos 2018, rather than on a landmark animal study anybody cites by name. Nobody moved from a defined rodent dose in mg/kg to a human dose here; the human dose came from clinical practice and worked backwards.
In people, and the numbers are good. Seven double-blind placebo-controlled trials of the micronized purified flavonoid fraction in 1,692 patients with chronic venous disease reduced leg pain, heaviness and swelling with ankle circumference down Kakkos 2018. Then the wider class review: Cochrane pooled 56 studies with usable data covering 7,690 participants of mean age 50, and concluded with moderate-certainty evidence that phlebotonics probably reduce edema slightly, relative risk 0.70 with a 95% confidence interval of 0.63 to 0.78 across 13 studies and 1,245 participants Martinez-Zapata 2020.
Read those two sentences together, because they are the honest shape of this product. The molecule-specific meta-analysis is enthusiastic Kakkos 2018; the class-wide Cochrane review, using the same literature under stricter certainty grading, lands on probably reduce edema slightly Martinez-Zapata 2020. Both are correct. The difference between them is what a certainty framework does to a body of trials that are mostly small, mostly industry-sponsored and mostly reporting subjective endpoints.
The specific obstacle is that milligrams do not predict exposure. A crossover in healthy volunteers compared two diosmin formulations head to head and found an area under the curve of 298.4 for the test product against 31.9 for the reference — close to an order of magnitude, in the same study, in the same subjects, from the same nominal molecule Russo 2018. Diosmin is not absorbed as diosmin at all: the rutinose has to be removed by colonic bacterial rhamnosidase before diosmetin can cross the enterocyte, so the absorbed dose is set by particle size, dissolution and somebody's gut flora rather than by the number on the box.
Diosmin — which form, and does it matter
The micronized purified flavonoid fraction is a manufacturing specification: 90% diosmin, 10% other flavonoids expressed as hesperidin, milled below about 2 micrometers. The micronization is not cosmetic. These flavonoids are barely soluble, dissolution is the rate-limiting step in their absorption, and surface area scales as the inverse of particle diameter. That is the mechanistic reason a micronized and a non-micronized product at identical milligrams are not the same drug, and the head-to-head bioavailability study is the measurement of it Russo 2018.
And now the fact that complicates the tidy version. The one review that asked directly whether plain diosmin performs differently from the micronized fraction found three eligible clinical studies, in which both forms reduced chronic venous disease symptom intensity by up to roughly 50% over one to six months, without statistical differences between them Cazaubon 2021. Three studies is not many and symptom intensity is a coarse instrument, so the most likely reading is that the endpoint cannot resolve a difference the pharmacokinetics can. But an honest page reports it: the pharmacokinetic separation between products is documented Russo 2018 and the clinical separation is not.
What the different products on a shelf actually are. Micronized purified flavonoid fraction 500 mg, taken twice daily, delivering 450 mg diosmin and 50 mg hesperidin per tablet. Plain diosmin 900 mg with hesperidin 100 mg, taken once daily — the ratio the site's card names. Plain diosmin 600 mg alone. Hesperidin 500 mg alone, which is a different purchase for a different reason. The first is the configuration behind the trial numbers Kakkos 2018; the second is the one most widely sold in the United States because the micronized fraction is a prescription drug in much of Europe and a supplement here.
Two practical label tests. First, does it say micronized or give a particle size? If not, assume it is not. Second, does it state diosmin in milligrams as a named ingredient, rather than as part of a citrus complex? A product that cannot tell you how much diosmin is in it cannot be compared with any trial on this page.
What would have to be true, and how you would know it was not
1. Leg volume by water displacement, in milliliters — down slightly, and slightly is the operative word. Fill a tall container to a marked line, submerge the leg to a marked height, measure the displaced water. Cochrane's verdict on this whole class is that it probably reduces edema slightly Martinez-Zapata 2020, so predict a fall of tens of milliliters over 8 weeks rather than a transformed leg. Same time each evening; the diurnal swing is larger than the treatment effect.
2. The dose-response prediction, which is the mechanistic test. If absorption is limited by colonic hydrolysis and dissolution rather than by dose Russo 2018, then doubling from 1,000 mg to 2,000 mg a day should not double the effect. Predict a flat or nearly flat response above the standard dose. Anybody who escalates and feels more benefit has most likely measured their own expectations.
3. The prediction that cuts against the product. The diameter of a visible varicosity on duplex ultrasound will not change, and neither will the reflux time at the saphenofemoral junction. Valve incompetence is a mechanical failure of anatomy; nothing in adhesion-molecule biology or venous tone repairs a valve leaflet Li 2021. So a reader buying this to make the veins go away is buying the wrong thing, and the honest answer for that goal is ablation or sclerotherapy.
4. hs-CRP, mg/L — unchanged. The proposed action is local: leukocyte adhesion in the microcirculation of one limb Kakkos 2018. That is far too small a compartment to shift a systemic acute-phase marker. Predict no change at 12 weeks, and treat a fall as evidence about something else you changed.
What will fool you: starting in October. Evening leg volume falls with ambient temperature, and a venotonic begun as the weather cools inherits the season's effect.
What nobody has tested yet
Nobody has run a trial stratified by plasma diosmetin. This is the obvious experiment and the tool already exists: two products with a documented tenfold difference in exposure Russo 2018. Give both to the same patients, measure plasma diosmetin conjugates, and then ask whether the high-exposure arm produces more leg-volume reduction. If it does, micronization is vindicated and labels need particle sizes. If it does not, the pharmacokinetic argument that sells the premium product is decorative, which would be equally worth knowing.
Nobody has compared diosmin with the micronized fraction using an objective endpoint. The three studies that made the comparison used symptom intensity and patient satisfaction Cazaubon 2021. Air plethysmography gives venous filling index and residual volume fraction; opto-electronic volumetry gives leg volume to a few milliliters. Either would answer in twelve weeks what symptom scores have failed to answer in twenty years.
Nobody has tested the antibiotic prediction. If diosmin requires colonic bacterial rhamnosidase before anything is absorbed, a week of a broad-spectrum antibiotic should temporarily inactivate the supplement. Measure urinary diosmetin conjugates before, during and after. It is a two-week study, it would confirm or destroy the microbial-activation model, and it has never been run.
And nobody has measured diosmetin in a vein. Varicose veins are removed surgically in large numbers every week. Dosing patients before a scheduled stripping and assaying the excised vessel would give the one number this entire mechanism rests on: the concentration at the target tissue, to be compared against the 1–10 mg/mL used in the dish Gwozdzinski 2023.
Diosmin — its own safety story, not its category's
The interaction that actually matters is antiplatelet, and it arrives with a paradox. Flavonoids of this class have weak antiplatelet activity, which is additive with aspirin, clopidogrel, and with the fibrinolytic enzymes elsewhere on this same shelf. The paradox is that one of the two indications this product is bought for is hemorrhoidal bleeding. Taking a mild antiplatelet agent for a bleeding complaint is not necessarily wrong — the venous mechanism may dominate — but it is worth knowing that the two actions pull in opposite directions, and it is worth stopping before any procedure.
Gastrointestinal adverse events are the commonest and they are the reason people stop. Across the phlebotonic class, adverse events are predominantly gastrointestinal, and Cochrane's assessment weighs them against a modest benefit Martinez-Zapata 2020. Splitting the daily dose and taking it with food is the practical answer and it also matches the absorption argument.
The masking problem, and it is not the one you are expecting. Bilateral leg swelling in an adult has several causes that a venotonic will partially relieve without touching: lymphedema, which needs compression and manual drainage rather than a flavonoid; heart failure, where the swelling is a symptom of a pump; and drug-induced edema, which is common and repeatedly missed. The three drug classes to check before buying anything are dihydropyridine calcium channel blockers such as amlodipine, gabapentinoids, and the thiazolidinediones. If the ankles started swelling within weeks of starting one of those, the fix is a prescription review, not a supplement.
Sourcing, because this molecule is semi-synthetic. Diosmin is manufactured by chemical dehydrogenation of hesperidin. That is a reagent-and-solvent process, and the impurity profile of the finished flavone is a manufacturing question that no supplement label answers. It is not a reason to avoid the product; it is a reason to prefer a manufacturer that will send you a certificate of analysis.
And the gap in the evidence for the people most likely to want it. The trial base is adults with chronic venous disease Kakkos 2018Martinez-Zapata 2020. Anticoagulated patients, people with active venous ulceration and pregnant women in the third trimester are the three groups with the most to gain and the least data, and no amount of mechanism closes that gap.

How you would know if it worked
There is no blood test for this one. That is not a criticism — it is a fact about the effect, and it changes how you should judge it.
- What to watch: Ankle circumference at a marked point on the same leg, and days per week with leg heaviness — plus, if hemorrhoids are why you are here, bleeding days per month, which is the endpoint with the strongest trial support for this molecule. Measure the ankle to the nearest 5 mm and count the bleeding days across 30 days. Check the label before you score anything: the studied unit is micronized diosmin 900 mg with hesperidin 100 mg, and a plain diosmin capsule at some other ratio is not the product those trials ran.
- How long before it means anything: 6-8 weeks for the venous read, and one full cycle of flares for the hemorrhoidal one. The proposed action is increased venous tone and reduced capillary leak, which accumulates over weeks — and which explicitly does not repair the valves, so damaged anatomy sets a ceiling on what any dose can deliver.
- What will fool you: Chronic venous symptoms swing with the season, with standing hours and with body weight, and all three tend to change in the month somebody finally buys a treatment. Hemorrhoid flares settle by themselves inside 7-14 days, so a product started at the peak of one collects the credit for the resolution. Rectal bleeding is not a symptom to score through — have it looked at rather than treated from a shelf.
Run it one variable at a time. Starting three things in one week means a result you cannot attribute, which is the same as no result.
Diosmin — safety & side effects
- GI upset and headache; generally well tolerated in the venous-insufficiency trials, which are reasonably large.
- Mild antiplatelet activity. Additive bleeding risk with anticoagulants and antiplatelets — warfarin, apixaban, rivaroxaban, clopidogrel, and aspirin at any dose. The interaction is pharmacodynamic rather than metabolic, so it does not show up as a changed drug level; it shows up as bruising, nosebleeds or a raised INR.
- Not established as safe in pregnancy or breastfeeding — not because harm is documented, but because these are the two populations systematically excluded from the trials. Absence of data is not reassurance. Usually taken with hesperidin as a micronized flavonoid fraction.
