How Plant Sterols & Stanols actually works

Plant sterols are close enough in shape to cholesterol to compete with it for space in the mixed micelles that carry fat across the intestinal wall, and for the NPC1L1 transporter that does the absorbing. Displaced cholesterol — both dietary and the much larger amount your liver secretes in bile — leaves in stool instead of being reabsorbed, and the liver upregulates LDL receptors to compensate. That NPC1L1 step is the identical target as the prescription drug ezetimibe, at a fraction of the potency.

An unusually well-evidenced supplement with an approved health claim, and it does exactly one thing: it lowers LDL by roughly eight to twelve percent. It does not replace a statin and the outcome data — fewer heart attacks, not just better numbers — has never been demonstrated for sterols the way it has for the drugs. It has to be taken with a meal containing fat to work at all, since it competes inside micelles that only form when you've eaten. People with the rare condition sitosterolemia must avoid it entirely.

⚠️ Good to know: Must be taken with food containing fat — taken fasted it does close to nothing, and that single detail is why people report no effect. It also modestly lowers carotenoid absorption, so keep coloured vegetables in the diet.
