How Pantethine actually works
The disulphide dimer of pantetheine and a direct precursor to coenzyme A, which sits at the centre of fatty acid oxidation and cholesterol synthesis. The proposed lipid mechanism involves modulation of HMG-CoA reductase activity and increased fatty acid oxidation, though it is not fully settled — which is why the effect is reported honestly as observed rather than fully explained. Plain pantothenic acid does not reproduce it.

✅ Clinically validated
- Multiple randomised trials, including a US double-blind placebo-controlled study, show reductions in LDL cholesterol and non-HDL cholesterol of roughly 10% over 16 weeks at 600 mg daily.
- Trials are small and the total evidence base is modest, but the direction is consistent and the safety record is good.
- It is nowhere near statin-magnitude and should not be presented as an alternative to one where a statin is indicated.

- Plain pantothenic acid at equivalent doses does not reproduce the lipid effect, which supports the idea that the disulphide form matters rather than the B5 content.
🧪 Theoretical / extrapolated benefits
- A precursor to coenzyme A; proposed to modulate HMG-CoA reductase and fatty acid oxidation, though the exact mechanism for the lipid effect is not settled.

The trap runs the other way too: something can be clinically validated and still do very little for you specifically. A statistically significant result in a study population is not a promise about your body.

- 📊 Correlative — observational or epidemiological data. Suggestive, and genuinely useful for direction, but it cannot establish cause.
- 🧪 Theoretical / mechanistic — the mechanism is understood and often demonstrated in cells or animals, and the human trial doesn’t exist yet. Unproven is not the same as ineffective. Plenty of what’s standard practice today sat here five years ago.
✗ is a safety flag, not a grade. Where you see it, the concern is harm — not a disappointing trial. A compound tested for one purpose and found not to help there can still be worth studying somewhere else, so a negative result never gets rendered as a cross. It sits alongside the tier, because something can be both well-studied and genuinely risky.
My job is to tell you which one you’re looking at, and let you make the call. Grading something low isn’t me dismissing it — it’s me refusing to oversell it. This is the research space, and being able to reason forward from a mechanism matters as much as waiting for the trial.
600 mg daily split across two or three doses, and give it 16 weeks — the trials that show a roughly 10% LDL reduction ran that long. Read the result against ApoB rather than LDL-C alone. The important framing: a 10% reduction is not a statin and should not be used to defer a statin conversation if your actual risk warrants one. Very well tolerated; mild gastrointestinal upset is the main complaint. No significant interactions, but tell your prescriber if you are on lipid-lowering therapy so the numbers are attributed correctly.
