How Pantethine actually works

The disulphide dimer of pantetheine and a direct precursor to coenzyme A, which sits at the centre of fatty acid oxidation and cholesterol synthesis. The proposed lipid mechanism involves modulation of HMG-CoA reductase activity and increased fatty acid oxidation, though it is not fully settled — which is why the effect is reported honestly as observed rather than fully explained. Plain pantothenic acid does not reproduce it.

How to read these tiers

These tiers tell you how much human evidence exists — not how well something works. This is the research space, and most of what’s in here is new rather than disproven. Something sitting at “theoretical” usually means nobody has funded the trial, not that the trial was run and failed.

The trap runs the other way too: something can be clinically validated and still do very little for you specifically. A statistically significant result in a study population is not a promise about your body.

A compound tested for one purpose and found not to help there can still be worth studying somewhere else, so a negative result never gets rendered as a cross. It sits alongside the tier, because something can be both well-studied and genuinely risky.

600 mg daily split across two or three doses, and give it 16 weeks — the trials that show a roughly 10% LDL reduction ran that long. Read the result against ApoB rather than LDL-C alone. The important framing: a 10% reduction is not a statin and should not be used to defer a statin conversation if your actual risk warrants one. Very well tolerated; mild gastrointestinal upset is the main complaint. No significant interactions, but tell your prescriber if you are on lipid-lowering therapy so the numbers are attributed correctly.
