How Acetyl-L-Carnitine actually works

Carnitine with an acetyl group attached, which lets it cross the blood-brain barrier where plain carnitine cannot. Once there it does two jobs: it shuttles fatty acids into neuronal mitochondria for oxidation, and it donates its acetyl group to form acetyl-CoA, which is the direct precursor for acetylcholine synthesis. So it is simultaneously a mitochondrial and a cholinergic agent.
