Why this panel

A high ferritin has many causes and most of them are not iron. A high TRANSFERRIN SATURATION has few, and iron overload is the main one — which is why saturation, not ferritin, is the screening number. Hereditary hemochromatosis is among the commonest inherited conditions in people of northern European ancestry, it is treated by taking blood off, and untreated it damages liver, pancreas, joints and heart over decades. It is also, genuinely, over-diagnosed from ferritin alone.

💡 What most people missGenotype and phenotype are different questions and this panel answers both. Carrying the C282Y homozygous genotype does not mean you have iron overload: a large population study found that only a minority of people with the genotype ever develop clinical disease, and the proportion differs sharply between men and women. The genotype tells you the risk and settles it permanently, once, for you and for your siblings and children. The iron studies tell you whether it is happening now. Neither answers the other. Saturation is the number to act on and ferritin is the number to follow. Saturation rises first, because transferrin fills before storage overflows. Ferritin then tracks the size of the load, which is what treatment reduces. hs-CRP is here because a ferritin during inflammation proves nothing. Ferritin is an acute-phase protein, and a raised ferritin with a NORMAL saturation is far more often inflammation, alcohol or fatty liver than iron overload.

Transferrin saturation follows a daily rhythm and rises after a meal; an afternoon non-fasted saturation is the single commonest reason a normal person is told they might have hemochromatosis. Stop all iron, including a multivitamin containing it, for at least 7 days. No alcohol for 72 hours; it raises ferritin independently. Do not draw within 2-3 weeks of an infection. A raised saturation should always be repeated fasted before anything else happens.

What this panel can settle, and by what logic

Eight markers that answer two separate questions with one draw: is iron accumulating now, and is the genetic reason for it present.

  1. Transferrin saturation on the Iron Panel (Iron, TIBC, Transferrin Saturation) is the screening number, and Ferritin is not. Saturation rises first because transferrin fills before storage overflows, and it has few causes; a raised ferritin has many, most of which are not iron. The relationship between HFE genotype, ferritin and saturation has been measured explicitly in people referred with suspected overload Sandnes 2021.
  2. The HFE genotype settles the inherited question once, for you and for your siblings and children. It is also not the same as having the disease: in a large population cohort, common conditions associated with the genetic variants were quantified at population scale Pilling 2019, and follow-up to age 80 measured how many carriers were ever diagnosed and what happened to them Lucas 2024. Genotype and phenotype are separate facts, and this panel buys both Anderson 2021.
  3. Transferrin is on the list because saturation is a ratio. Transferrin falls with inflammation and with liver disease, which raises the calculated saturation without any change in iron.
  4. hs-CRP (High-Sensitivity C-Reactive Protein) is the decoder. Ferritin is a positive acute-phase protein, and micronutrient markers cannot be read against a reference range without an inflammation marker beside them Luo 2023. A raised ferritin with a NORMAL saturation is far more often alcohol, fatty liver or inflammation than iron overload, which is also why a single global ferritin threshold has never been agreed Cancado 2025.
  5. Comprehensive Metabolic Panel (CMP) and HbA1c (Hemoglobin A1c) cover the two organs iron reaches first. Liver enzymes and glucose handling are the earliest evidence of deposition, and iron overload disorders are characterized by that organ pattern Hsu 2022.
  6. The Complete Blood Count (CBC) with Differential gives the hemoglobin that venesection is followed by, and the platelet count that changes if the liver is involved.

What it cannot settle, and what would

It cannot measure how much iron is in your liver. Hepatic iron concentration by MRI is the non-invasive standard and it is what decides treatment in the cases where the blood work is ambiguous Crawford 2023. No serum marker substitutes.

It cannot stage liver damage. Normal liver enzymes do not exclude fibrosis, and a raised ALT with iron overload needs imaging and a fibrosis assessment rather than a repeat panel Crawford 2023Hsu 2022.

It cannot predict who becomes ill. Carrying the C282Y homozygous genotype is not the same as developing clinical disease, and the proportion who do differs sharply by sex and by age Pilling 2019Lucas 2024. A genotype result is a reason for periodic iron studies, not a diagnosis.

And the HFE panel covers the common European variants only. Iron overload without C282Y or H63D exists, is rarer, and belongs with a hepatologist rather than with a repeat of this panel Anderson 2021Hsu 2022.

Draw conditions that decide whether the money is wasted

Fasting is the condition that decides whether the saturation means anything.

  1. Fasted, in the morning. Transferrin saturation follows a daily rhythm and rises after eating; a non-fasted afternoon saturation is the single commonest reason a well person is told they might have hemochromatosis Sandnes 2021.
  2. Stop all iron, including any multivitamin containing it, for at least 7 days. A supplement taken that morning raises serum iron directly and the saturation with it.
  3. No alcohol for 72 hours. It raises Ferritin independently of iron stores and is one of the commonest confounders in this exact situation Cancado 2025.
  4. Not within 2 to 3 weeks of an infection, for the same reason Luo 2023.
  5. Repeat a raised saturation fasted before anything else happens. Confirmation on a second draw is the step that prevents most of the unnecessary genetic testing Sandnes 2021.
