Why this panel

Ferritin is the most misread number on a standard panel. It is an iron-storage protein and an acute-phase protein at the same time, so it rises with genuine iron overload, with any inflammation, with alcohol, with a fatty liver and with metabolic syndrome — four completely different problems with four completely different answers. A raised ferritin on its own does not distinguish them. Transferrin saturation does, and it costs under ten dollars.

💡 What most people missTransferrin saturation is the fork in the road, not the ferritin. Genuine iron loading pushes saturation up first and ferritin later; the metabolic and inflammatory causes raise ferritin with a normal or low saturation. That single comparison decides whether the next step is a genetic test and a hematology conversation, or a liver and metabolic conversation — and they are not the same appointment. The commonest cause of a high ferritin is not hemochromatosis. It is the cluster of raised triglycerides, raised glucose, raised liver enzymes and central weight that now has its own name in the literature. Donating blood for that picture treats a number rather than a disease, and can leave somebody iron deficient with a still-raised ferritin.

What this panel can settle, and by what logic

Eight markers for one decision: whether a raised Ferritin is iron that has accumulated, or a ferritin that has risen for a reason unrelated to iron. Those two answers lead to different specialists, and the marker that separates them costs under ten dollars.

  1. Transferrin saturation on the Iron Panel (Iron, TIBC, Transferrin Saturation) is the fork in the road, not the ferritin. Genuine iron loading raises saturation first and ferritin second, which is why hemochromatosis guidelines start the diagnostic pathway at a raised saturation rather than at a raised ferritin Crawford 2023. In a series of patients referred with suspected hereditary hemochromatosis, ferritin and saturation were read together against genotype precisely because neither settles it alone Sandnes 2021.
  2. Transferrin disambiguates a borderline saturation. Saturation is a ratio, and a ratio can be pushed up by a low denominator as easily as by a high numerator. Transferrin falls in inflammation and in liver disease and rises in iron deficiency, so it tells you which half of the ratio moved.
  3. hs-CRP (High-Sensitivity C-Reactive Protein) decides whether the ferritin is even a fact about iron. Ferritin is a positive acute-phase protein, and inflammation correction methods exist for micronutrient biomarkers for exactly this reason Luo 2023. A raised ferritin with a raised CRP is an inflammation result until the inflammation is gone and it has been repeated.
  4. The Comprehensive Metabolic Panel (CMP), HbA1c (Hemoglobin A1c), Lipid Panel (Cholesterol, HDL, LDL, Triglycerides) and Uric Acid together test the commonest explanation of all. Raised ferritin with a normal saturation, in the company of raised triglycerides, raised glucose, raised liver enzymes and central adiposity, now has a formal definition and a name of its own: metabolic hyperferritinemia Valenti 2023. It is not iron overload and venesection is not its treatment.

Read as a set, the panel returns one of three verdicts. Saturation persistently high with a low transferrin points at true loading and earns a genetic test Crawford 2023. Saturation normal with the metabolic cluster points at the liver and the metabolic clinic Valenti 2023. Saturation low with a high CRP and a low transferrin points at inflammation, and the ferritin is a passenger Anders 2026.

What it cannot settle, and what would

It cannot measure how much iron is in your liver, and that is the number that decides whether venesection is needed. Liver iron quantification by MRI is the non-invasive standard and biopsy the older one Hsu 2022. What this panel does is tell you whether that scan is the right next spend, which for most people with a raised ferritin it is not.

It carries no genetic test. HFE genotyping is a separate $200 order and it is deliberately not bundled here, because ordering it before the saturation is known is the wrong sequence: most people with a raised ferritin do not have a raised saturation, and most people with the common genotype never develop iron overload Sandnes 2021. If the saturation is high on two draws, the HFE genetic test is the next order.

It cannot diagnose the rarer inherited causes. A high ferritin with a persistently normal saturation and no metabolic or inflammatory explanation can be a hereditary hyperferritinemia, including the variant that comes with early cataracts and carries no iron overload at all Piperno 2023. Venesection for that picture removes iron somebody did not have to spare.

And it cannot stage liver damage. Raised liver enzymes on the Comprehensive Metabolic Panel (CMP) say something is happening; they do not grade fibrosis. The ELF fibrosis test and elastography do that, and either is a better second step than repeating the ferritin.

Draw conditions that decide whether the money is wasted

Three conditions, and the first is the one that produces most of the false alarms.

  1. Leave 2 to 3 weeks after any infection, injury or hard training block. Ferritin is an acute-phase protein and can move enough on inflammation alone to generate the entire referral Luo 2023Cancado 2025. A ferritin drawn a week after influenza is not a fact about iron.
  2. Fast 9 to 12 hours and draw before 10am. The triglycerides and glucose need the fast, and serum iron has a morning peak, so an afternoon transferrin saturation is not comparable to the morning value the guidelines are written around Crawford 2023.
  3. Two weeks clear of alcohol, not two days. Alcohol raises ferritin and liver enzymes and enlarges red cells at the same time, which reproduces most of the metabolic pattern this panel is trying to identify Anders 2026.

One thing not to do: donating blood before the draw. It lowers the Ferritin you are trying to measure and can convert a diagnostic panel into a normal one without changing anything about the underlying problem.

The result that ends the question: a normal transferrin saturation with a normal CRP and normal metabolic markers means the ferritin was never the finding it was treated as, and an annual Ferritin with an Iron Panel (Iron, TIBC, Transferrin Saturation) is enough follow-up.
