What SPMs (Pro-Resolving Mediators) actually does
Resolution is an active biochemical program, not the absence of inflammation, and that is the one genuinely new idea in this category. At the peak of an acute inflammatory response the arachidonic acid pathway switches product classes - a lipid mediator class switch - and starts producing molecules that stop neutrophil recruitment, drive macrophages to swallow the apoptotic neutrophils already there, and clear the debris. The switch is enzymatic and it is the reason a wound stops being red.
The mediators are 3 enzymatic steps downstream of the fatty acid in the softgel, and this is the conversion the category is sold past. Eicosapentaenoic acid is oxygenated to 18-hydroxy-EPA, usually by acetylated cyclooxygenase-2 or by a cytochrome P450, then 5-lipoxygenase converts that intermediate to the E-series resolvins. Docosahexaenoic acid goes to 17-hydroxy-DHA by 15-lipoxygenase and then to the D-series resolvins, the protectins and, by a separate 12-lipoxygenase route in macrophages, to the maresins. Each step needs a specific enzyme in a specific cell type at a specific moment.
Which means the enzyme, not the substrate, is usually the limiting factor. 15-lipoxygenase in a macrophage or eosinophil cannot be bought. Aspirin acetylates cyclooxygenase-2 and switches its product to the 18-hydroxy intermediate, which is why aspirin-triggered resolvins are a real class - and why somebody not taking aspirin runs the pathway differently. Adding more substrate to a pathway whose bottleneck is enzymatic raises output only if substrate was in fact the bottleneck, and in a well-fed omnivore taking fish oil it usually is not.
Receptor identity is where this stops being lipid chemistry and becomes pharmacology. Resolvin D1 acts at ALX/FPR2 and GPR32, resolvin E1 at ChemR23 and as a partial antagonist at the leukotriene B4 receptor, and maresin 1 at LGR6. These are G-protein-coupled receptors with nanomolar affinities, which is why the concentrations that matter are picomolar to nanomolar and why milligrams of anything is the wrong unit to think in.
The other end of the conversion chain is disposal, and it is fast. Resolvins D5 and D1 undergo phase II metabolism by uridine diphosphate glucuronosyltransferases Nogueira 2024, on top of the eicosanoid oxidoreductase route that inactivates them. A signaling molecule that is glucuronidated is designed to have a short life and a local action, which is the opposite of what a daily capsule is shaped to deliver.
And the field is candid about how much of this is settled. A review titled for exactly that question walks formation, signaling and occurrence and asks what the evidence so far actually supports Schebb 2022, including the analytical difficulty of measuring picomolar lipids in plasma without generating them during sample handling. That measurement problem is why the strongest claims in this space are the least reproducible.
Cell, rodent, human — and where it stops
Cell and tissue biology here is excellent. The human supplementation chain is 1 step long, and that step has been measured.
In healthy people. A marine oil enriched in specialized pro-resolving lipid mediators was given to healthy subjects, with pharmacokinetics and a lipid mediator profile as the endpoints Irún 2023. That is the study this category needed, and it is worth reading for what it measures: mediator profiles in plasma, not clinical outcomes.
In disease populations, the literature is observational and mechanistic rather than interventional. Specialized pro-resolving mediators in respiratory disease have their own review Cagnina 2022, a systematic review covers them alongside dietary omega-3 and omega-6 fatty acids in inflammatory skin disease Bielach-Bazyluk 2025, and a mechanistic meta-analysis examines the precursors in depression Iqbal 2026. Note the word precursors in that last one: much of what is called an SPM literature is a literature about the fatty acids upstream of them.
The obstacle is the one this whole page is about. A softgel does not contain resolvins in a pharmacological quantity. It contains an oil enriched in the hydroxylated intermediates - 18-hydroxy-EPA, 17-hydroxy-DHA, 14-hydroxy-DHA - which still require 5-lipoxygenase or 12-lipoxygenase inside a leukocyte to become the mediators the receptor biology describes. A person with the wrong cell state converts little.
The second obstacle is measurement, and it is unusually severe. These lipids sit at picomolar to low nanomolar concentrations, oxidize during blood handling, and require liquid chromatography with tandem mass spectrometry and careful antioxidant protocols. Discrepancies between laboratories are a live methodological argument rather than a settled matter Schebb 2022, so a positive plasma finding in 1 laboratory is weaker evidence than it looks.
And no randomized trial has shown a clinical outcome for this as a supplement. The concept is strong, the receptor biology is real, and the sentence that would justify a purchase - this product reduced this symptom against placebo - has not been written yet.
SPMs (Pro-Resolving Mediators) — which form, and does it matter
The form question is whether what is in the capsule is a mediator or a precursor, and for every retail product it is a precursor. Fractionated marine oil concentrates the monohydroxylated fatty acids. It does not concentrate resolvins, protectins and maresins, because those are made enzymatically in tissue and are not stable articles of commerce.
So the number that should be on the panel is micrograms of 18-hydroxy-EPA, 17-hydroxy-DHA and 14-hydroxy-DHA per serving. Very few labels carry it. A panel that says a proprietary marine lipid concentrate with no hydroxy-fatty-acid figure is not describing the dose of anything the science is about, and the pharmacokinetic study that does exist measured exactly these intermediates Irún 2023.
The Thorne product on this card is Pro-Resolving Mediators, and no filed Supplement Facts panel for it has been read for this site. supplements_data.BLENDS has no entry for it, so this page does not state its contents, its serving size or its hydroxy fatty acid figures. That is the honest position and it is also the actionable one: the question to ask a vendor is how many micrograms of the monohydroxy intermediates a serving delivers.
Oxidation is a form issue with a real consequence. Hydroxylated polyunsaturated fatty acids are more prone to further oxidation than the parent fatty acids, so a fractionated oil is a less stable product than a plain fish oil at the same storage temperature. That argues for cold storage, a peroxide value on the certificate of analysis, and small bottles.
And the cheapest form question is whether the plain oil does the same job. Eicosapentaenoic and docosahexaenoic acid are the substrates for the entire pathway, cost a fraction as much per gram, and have randomized outcome data behind them that this category does not. A product priced several times higher owes a reader a head-to-head comparison, and none exists Cagnina 2022.
What would have to be true, and how you would know it was not
1. The pharmacokinetic prediction, and it has been half-run. After a single serving, predict measurable increases in plasma 18-hydroxy-EPA and 17-hydroxy-DHA within 2 to 6 hours, since that is what a specialized-mediator-enriched marine oil produced in healthy subjects Irún 2023. Predict much smaller and less consistent increases in the downstream resolvins themselves, because that step needs a leukocyte.
2. The prediction that cuts against the product, and it is the one to run. In a healthy adult with no active inflammation, predict no change in hs-CRP, no change in erythrocyte sedimentation rate and no change in a validated pain score over 8 weeks, because there is no unresolved inflammatory event for the pathway to terminate. A well-controlled trial showing benefit in that population would falsify this page.
3. The prediction that would distinguish this from fish oil. Run the enriched fraction against a plain fish oil matched on total eicosapentaenoic and docosahexaenoic acid, with hs-CRP and the omega-3 index at 12 weeks. Prediction: the omega-3 index moves with total eicosapentaenoic and docosahexaenoic acid and not with the enrichment, because red cell incorporation is a function of the parent fatty acids Schebb 2022.
4. The aspirin prediction, which is the mechanistic tell. Low-dose aspirin acetylates cyclooxygenase-2 and redirects it toward the 18-hydroxy intermediate. Prediction: in people already on low-dose aspirin for a cardiovascular indication, the measured mediator response to the same serving is larger. That has never been tested and it would be a clean confirmation of the mechanism.
5. The glucuronidation prediction, at 6 hours. If resolvins are cleared by uridine diphosphate glucuronosyltransferases Nogueira 2024, predict that plasma concentrations of the free mediators fall sharply between 4 and 8 hours while their conjugates rise. Nobody has published the conjugate measurements, and they would settle whether once-daily dosing makes any sense.
What nobody has tested yet
Nobody has run a randomized clinical outcome trial. The reviews assemble mechanism and association Cagnina 2022 Bielach-Bazyluk 2025, and the meta-analysis in depression is explicitly about precursors Iqbal 2026. A trial with a symptom endpoint, a placebo and a plain fish oil comparator is the missing study and the category is being sold ahead of it.
Nobody knows what fraction of an oral intermediate becomes a mediator. The pharmacokinetic study measures what appears in plasma Irún 2023; it does not measure conversion efficiency at the site of inflammation, which is where the biology happens and where nobody can sample.
Nobody has agreed on how to measure these compounds. The analytical dispute is live enough to be the subject of its own review Schebb 2022. Until interlaboratory agreement exists, a reported plasma resolvin concentration is a laboratory-specific number.
And nobody has tested the obvious clinical hypothesis. If resolution failure is the defect in chronic inflammation, the group that should respond is people with a documented failure to resolve - persistent post-surgical inflammation, non-healing wounds, chronic rhinosinusitis. No supplementation trial has selected that population, which is the single most obvious study in the field.
SPMs (Pro-Resolving Mediators) — its own safety story, not its category's
The safety story here is mostly the fish oil safety story, with one addition that belongs only to this product. A concentrated marine lipid fraction carries the usual bleeding-time and platelet-aggregation considerations of high-dose omega-3, which matter alongside anticoagulants, antiplatelets and planned surgery.
The addition is oxidation. Hydroxylated polyunsaturated fatty acids are the intermediates of lipid peroxidation, and a fraction enriched in them is chemically less stable than the oil it came from. Rancid marine oil is an oxidative load rather than an anti-inflammatory one. A fishy-smelling capsule, a bottle stored above room temperature or a product past date should be discarded, and a certificate of analysis with a peroxide value is a fair thing to ask for.
The interaction that is specific and rarely mentioned is with aspirin. The aspirin-triggered branch of this pathway means the 2 are mechanistically linked rather than independent Schebb 2022. That is not a warning against the combination; it is a reason not to assume the effects simply add, and a reason to tell a prescriber both are being taken.
Shellfish and fish allergy is a hard stop for a marine-derived product, and iodine and heavy metal content is a purity question that belongs on the certificate of analysis rather than in a reassurance. Neither is unique to this product and both are worth checking because the material is a concentrate.
And the honest closing risk is financial and diagnostic rather than toxicological. This is among the most expensive items in the catalog per month, it has no randomized outcome evidence, and unresolving inflammation is a reason to be examined rather than supplemented. How you would know if it worked
There is no blood test for this one. That is not a criticism — it is a fact about the effect, and it changes how you should judge it.
- What to watch: One injury or one hard block, followed closely: days until you could train the movement again, and soreness scored 0 to 10 at 24 and 48 hours after the same session repeated. These are resolution molecules, so the claim is about how fast inflammation switches off — not about how it feels while it is running.
- How long before it means anything: A short course around a defined event, which is how it is sold: days to a few weeks. If you cannot name the event you are recovering from, there is nothing here to measure.
- What will fool you: Time, which resolves nearly everything you would take this for. Soft-tissue injuries and heavy-training soreness improve on almost exactly the timescale of a short course, so the course and the recovery finish together whatever you do. Human outcome data is early enough that a strong personal impression is running well ahead of the evidence — and pain that is not improving on schedule needs a diagnosis rather than a longer course.
Run it one variable at a time. Starting three things in one week means a result you cannot attribute, which is the same as no result.
