The immune-modulating evidence is better than the sleep evidence, which is largely traditional plus animal data. Because it modulates rather than stimulates, it is a more sensible choice than the beta-glucan mushrooms for anyone with autoimmunity. It has antiplatelet activity, so it matters alongside anticoagulants or before surgery. Dual-extracted products capture both the water-soluble polysaccharides and the alcohol-soluble triterpenes; single extraction misses half.

How Reishi actually works

Triterpenes and polysaccharides with immunomodulatory rather than immunostimulatory activity — the distinction matters, since it appears to normalize rather than amplify immune signaling. The sleep effect is attributed to the triterpene fraction and shows up in rodent work as increased non-REM sleep, with the human mechanism not clearly established.

What Reishi actually does

Two chemically unrelated fractions carry the entire case for this mushroom, and they are not distributed evenly through it. The first is the triterpenoids — lanostane-derived ganoderic and ganoderenic acids, oxygenated tetracyclic molecules that are small enough to absorb and lipophilic enough to reach tissue. The second is the beta-glucans, cell-wall polysaccharides that are large, are not absorbed intact, and act where they are. When somebody says reishi is immunomodulating and calming in the same sentence, they are combining claims that belong to different molecules with different fates.

Where those fractions sit inside the fungus has now been mapped. Work on Ganoderma fruiting bodies measured the distribution of lanostene-derived triterpenoids and glucan content across the tissue De Oliveira Campos 2024. That is the study that makes the form section of this page possible: it means the phrase “reishi extract” describes a location as much as a species, and the two fractions do not travel together.

The beta-glucan mechanism is receptor-mediated and local. Fungal beta-1,3/1,6-glucans are recognized by dectin-1 on myeloid cells and by complement receptor 3, and the downstream response is cytokine release and phagocyte priming. This is a pattern-recognition event, not a drug binding a target, and its ceiling is set by how much glucan reaches gut-associated lymphoid tissue in a recognizable form. The systematic review of randomized controlled trials of fungal beta-glucans is the honest summary of what that produces in people Vlassopoulou 2021.

The triterpenoids are the fraction that could plausibly explain a calming effect, and they are also the fraction most often absent. Ganoderic acids are bitter — the bitterness of a reishi extract is a rough proxy for triterpene content — and they are what a hot-water extraction leaves behind. A dual extraction, water then ethanol, exists because no single solvent takes both fractions.

What the pooled clinical evidence supports is narrower than the shelf implies. A GRADE-assessed systematic review and meta-analysis of clinical trials of Ganoderma lucidum in human health is the current best summary, and GRADE assessment is the part that matters: it grades the certainty of each outcome rather than counting positive studies Jafari 2025. A reader who wants to know what this mushroom does should read the certainty ratings, not the abstract.

Cell, rodent, human — and where it stops

The chain breaks at a place that is unusual for a botanical: the organism itself.

In the dish and in the fungus. Triterpenoid and glucan distribution has been quantified in fruiting bodies De Oliveira Campos 2024, and comparative mitogenomics has produced new DNA barcodes for the genus because morphology could not reliably separate its members Chen 2024. Both of those are papers about identity, and both were necessary.

In people, on the beta-glucan side, there are randomized controlled trials and they have been reviewed as a group across fungal sources rather than for this species alone. That pooling is itself the translation problem: a beta-glucan from one fungus is not identical to a beta-glucan from another in branching, molecular weight or solubility, and those are the properties dectin-1 responds to.

In people, on the whole-product side, the pooling has been done with a certainty framework attached Jafari 2025. The obstacle it inherits is that the included trials used different material — different species within the genus, different tissue, different extraction — so heterogeneity is built into the input rather than being a property of the effect.

The spore literature is a separate translation chain and it is shorter than it looks. A systematic review of the pharmacological activities and safety of Ganoderma lucidum spores exists Thuy 2023. Spores are a different material from fruiting body with a different lipid profile and a hard sporoderm that has to be cracked before anything is accessible, so spore evidence does not transfer to a fruiting-body product or the other way around.

And the safety translation has been done properly at least once. A toxicological assessment of Ganoderma lucidum and Cordyceps militaris mushroom powders puts a formal safety evaluation behind material that is usually sold on tradition Chrysostomou 2024.

Reishi — which form, and does it matter

The species on the label is probably not the species in the bottle, and that is a taxonomy problem rather than a fraud problem. Ganoderma is a genus whose members are difficult to separate on appearance, which is why comparative mitogenomics was used to build new DNA barcodes for it Chen 2024. The cultivated East Asian material sold as reishi is widely treated in the modern literature as Ganoderma lingzhi rather than the European Ganoderma lucidum, and the case report of liver injury on this page names the species that way Guedikian 2023. Almost every supplement label still says lucidum.

Fruiting body, mycelium and spore are three products. The triterpenoids that give reishi its bitterness and its lipophilic fraction are a fruiting-body phenomenon, and their distribution has been measured there De Oliveira Campos 2024. Mycelium grown on grain is harvested with its substrate and is chemically a different material. Spores are a third thing with their own review Thuy 2023. A label that says only “reishi” has not answered the question.

Extraction chooses the fraction, and most products choose one. Beta-glucans are water-extractable; ganoderic acids largely are not. A hot-water extract is a glucan product with the triterpenes discarded, and an ethanol extract is the reverse. Dual-extracted products exist because that is the only way to have both, and the distinction is invisible on a supplement facts panel that lists a single milligram figure.

The polysaccharide percentage is the wrong number and it is usually the only number. A polysaccharide assay counts starch. Mycelium grown on rice will report a high polysaccharide content that is mostly cereal. The number that means something is beta-glucan specifically, ideally with the triterpene percentage beside it, and those two figures are what a reader should ask a vendor for.

Cracked-spore products are a real processing claim. The sporoderm is a resistant double wall; uncracked spores pass through. Whether cracking delivers anything clinically is a separate question from whether it is chemically necessary, and the spore review is the place to start rather than the label Thuy 2023.

What would have to be true, and how you would know it was not

1. Predict the sleep effect is subjective and does not show up on actigraphy. Reishi's traditional use is calming, and the plausible molecule is a triterpene rather than a sedative-hypnotic. Prediction: sleep-onset latency measured by actigraphy over two weeks does not shorten, while subjective sleep quality scores improve. If actigraphy did move, that would be the first objective evidence of a hypnotic action and would change how this product should be treated.

2. Predict ALT and AST are the markers that matter and the window is short. Acute liver injury attributed to this mushroom is documented Guedikian 2023. Baseline ALT, AST, ALP and bilirubin; repeat at four to six weeks. Prediction: unchanged in almost everyone, because the reaction is idiosyncratic rather than dose-dependent — which is exactly why a baseline is worth having before it is needed.

3. Predict an immune read-out moves as cytokines and not as counts. The mechanism is dectin-1 engagement, so the prediction is that total lymphocyte count and CD4/CD8 ratio are unchanged at twelve weeks while cytokine measures, if anybody bothers to run them, are where any signal sits Vlassopoulou 2021.

4. Predict a bleeding-time effect before a coagulation-panel one. Antiplatelet activity is pharmacodynamic and does not change drug levels. Prediction: INR is unchanged in somebody on warfarin while bruising and gum bleeding increase — which is the failure mode that makes this interaction dangerous, because the number people watch stays reassuring.

5. The prediction that would falsify most of the shelf. Buy ten reishi products, sequence them against the published barcodes Chen 2024 and assay them for triterpene and beta-glucan content De Oliveira Campos 2024. Prediction: species assignment and triterpene content vary widely, and price does not predict either. If price did predict them, the form argument here is wrong.

What nobody has tested yet

Nobody has published the market survey that would settle the species question. The barcodes exist Chen 2024 and the assays exist De Oliveira Campos 2024. A study that applies both to on-market products has not been done, and until it is, the phrase “Ganoderma lucidum” on a label carries no verified content.

Nobody has run fruiting body against mycelium in a person. The two materials differ measurably in chemistry, and no randomized trial has given matched doses of each to the same population with a single endpoint. Every clinical conclusion about reishi is therefore a conclusion about whichever material that trial happened to use.

Nobody has characterized the liver injury. There is a case report with a literature review attached Guedikian 2023 and a formal toxicological assessment of mushroom powders Chrysostomou 2024. What there is not is a case series with product analysis: which species, which extract, which contaminant, and whether the injury tracks with a fraction. That is answerable and unanswered.

And nobody has tested the sleep claim objectively. The traditional use is the calming one and the pooled clinical evidence has been graded Jafari 2025. A polysomnography trial — the measurement that would make the claim checkable — has not been published for this mushroom.

Reishi — its own safety story, not its category's

The defining safety fact about reishi is that its serious reaction is idiosyncratic, which means dose discipline does not protect against it. Acute liver injury has been reported in the setting of alcohol use, with a review of the prior literature attached Guedikian 2023. An idiosyncratic hepatic reaction is not predicted by the amount taken, so the only defense is knowing what normal looked like beforehand and stopping quickly.

The alcohol context in that report is worth reading rather than skipping. A hepatotoxic signal that appears alongside alcohol is the pattern that should make anybody cautious about stacking a botanical of unknown hepatic behavior onto an already-loaded liver. The symptoms that matter — unexplained fatigue, nausea, dark urine, yellowing sclera — are the reason to get enzymes checked rather than to wait and see.

Antiplatelet activity is the interaction most likely to cause harm and least likely to be detected. It is pharmacodynamic. Warfarin, the direct oral anticoagulants, clopidogrel and aspirin all have their effect added to rather than their concentration changed, so the standard monitoring test reads normal while the risk is higher.

The immune argument runs both ways and the direction depends on the person. Dectin-1 engagement by beta-glucans is a stimulus Vlassopoulou 2021. In autoimmune disease or on immunosuppression after transplant, a stimulus is the wrong direction, and the fact that the stimulus is natural has no bearing on it.

Contamination is a real category here and has been formally assessed. A toxicological assessment of mushroom powders exists Chrysostomou 2024, which is more than most of this shelf can say. Heavy metals accumulate in fungi; a certificate of analysis covering lead, cadmium, arsenic and mercury is a reasonable thing to ask for, and its absence is itself informative.
