Why this panel

Uric acid is the end product of purine breakdown, and about nine tenths of what your body makes is reabsorbed by the kidney rather than excreted. That makes the level mostly a question of handling rather than intake — which is why the diet advice most people are given moves the number far less than they expect, and why a rapid weight loss phase can trigger an attack in somebody eating almost nothing.

💡 What most people missCutting hard raises urate by two mechanisms at once and both are temporary. Ketones and lactate compete with urate for the same kidney transporters, so a very low-carbohydrate phase or a fast reduces urate excretion; and rapid tissue breakdown increases the purine load being processed. That is why the first gout attack of somebody's life so often lands in week three of a diet. It is also why measuring during the cut and again after it tells you something a single number never could. The number during an attack is the least useful number. Serum urate commonly falls into the normal range during an acute flare, so a normal result taken in a swollen joint does not exclude anything and a treatment target set from it is wrong. Measure between attacks. Kidney function is the other half of the answer, and creatinine cannot give it to a muscular person. Reduced filtration raises urate; raised urate is associated with faster loss of filtration. Cystatin C is what reads that relationship honestly when you carry a lot of muscle.

What this panel can settle, and by what logic

Nine markers around one number, because urate on its own tells you almost nothing about what to do next.

  1. Uric Acid read beside Fasting Insulin explains the mechanism most gout advice ignores. About nine tenths of filtered urate is reabsorbed rather than excreted, and insulin directly increases that reabsorption. Hyperuricemia therefore travels with the metabolic syndrome as a consequence of the same physiology rather than as a coincidence Yanai 2021. This is why the number often responds more to insulin sensitivity than to purine intake.
  2. The same transporters explain why a hard cut raises it. Urate handling at the proximal tubule is shared with organic anions, and drugs and metabolites that compete for those transporters change serum urate in both directions Tatrai 2021. Ketones and lactate are such competitors, which is why a first attack so often lands in week three of a diet or after a fast.
  3. Cystatin C with eGFR with the Urinalysis, Routine reads the kidney half honestly. Reduced filtration raises urate, and asymptomatic hyperuricemia is itself associated with faster loss of filtration Waheed 2021Barnini 2025. Creatinine cannot separate those two in somebody carrying muscle.
  4. hs-CRP (High-Sensitivity C-Reactive Protein) with the Complete Blood Count (CBC) with Differential separates an inflamed joint from a mechanically painful one, which is a question the urate value itself cannot settle either way.
  5. HbA1c (Hemoglobin A1c) and the Lipid Panel (Cholesterol, HDL, LDL, Triglycerides) complete the cluster. Guideline management of gout is explicitly about the cardiovascular and metabolic comorbidities as much as the joint Russell 2022.

One number is worth knowing before you buy anything: serum urate commonly falls into the normal range during an acute flare, so a value taken from a swollen joint understates your usual level and a treatment target set from it is wrong Roddy 2024.

What it cannot settle, and what would

It cannot tell you the joint that hurt was gout. Definitive diagnosis is the identification of monosodium urate crystals in fluid aspirated from the joint, and both the British and European guideline sets are built on that Roddy 2024Russell 2022. Pseudogout, a flare of inflammatory arthritis and septic arthritis look identical from the outside, and one of them destroys a joint in days. A hot, swollen, exquisitely painful joint with a fever needs to be seen the same day, not measured.

It cannot tell you whether to start urate-lowering treatment. That decision rests on attack frequency, tophi, joint damage and kidney disease as well as the number, and even within the guidelines there is active argument about how far to extend treatment Stamp 2024.

It cannot find crystal deposits. Dual-energy CT and ultrasound can visualize urate deposition in people whose blood urate is raised without symptoms; blood cannot Waheed 2021.

And a raised urate with no symptoms is not automatically a disease to treat. Asymptomatic hyperuricemia is a risk marker with a real association with kidney progression Barnini 2025, and it is also extremely common. Knowing it, re-measuring it once the cut has ended and acting on the metabolic drivers is a more defensible response than starting a drug.

Draw conditions that decide whether the money is wasted

Timing relative to the attack matters more than fasting does.

  1. Do not draw during a flare. Wait at least 2 weeks after the joint settles; urate falls during the acute episode and the result will understate your baseline Roddy 2024.
  2. Fast 9 to 12 hours, which the Lipid Panel (Cholesterol, HDL, LDL, Triglycerides) and Fasting Insulin need anyway.
  3. No alcohol for 72 hours. Beer is both a purine load and an excretion blocker, and the effect on a single reading is large.

  • Raised Uric Acid with raised HbA1c (Hemoglobin A1c), Fasting Insulin and triglycerides: the metabolic cluster, and urate is a passenger. The marker that should move first with weight and carbohydrate change is the insulin Yanai 2021.
  • Raised Uric Acid with a reduced Cystatin C with eGFR filtration rate or protein in the urine: the kidney takes priority Barnini 2025, and this belongs with a clinician rather than a diet change.
  • Recurrent attacks with a persistently raised urate between them: this is the situation where guidelines support treating to a urate target rather than treating flares Russell 2022. If you also have high blood pressure, losartan lowers urate through URAT1 while controlling it Hamada 2008.
