What this panel can settle, and by what logic
Seven tests for $74 buys breadth, and breadth answers one specific kind of question well: whether anything common and cheap to find is currently wrong. Two of the seven are here because of who is buying.
- Ferritin beside the Complete Blood Count (CBC) with Differential is the highest-yield pairing on the list. Iron stores empty long before hemoglobin falls, so the combination catches a stage that either test alone reports as normal, and it is the stage that explains most unexplained fatigue in menstruating women Cancado 2025.
- TSH (Thyroid-Stimulating Hormone) costs $9 and covers a condition several times commoner in women than men. It is a screening test rather than a thyroid workup, which is a real limitation and a reasonable trade at this price.
- HbA1c (Hemoglobin A1c) with the Lipid Panel (Cholesterol, HDL, LDL, Triglycerides) and the Comprehensive Metabolic Panel (CMP) is a trajectory rather than a diagnosis. HbA1c averages roughly three months of glucose, and it is as much a red cell measurement as a glucose one — anything that shortens red cell survival, including iron deficiency itself, shifts it independently of glycemia Chen 2022. On a panel that also measures ferritin you can at least see that interaction.
- Vitamin D (25-Hydroxy) is here because deficiency is the rule, with the caveat that 25-hydroxyvitamin D results carry measurable laboratory-to-laboratory variability Wise 2021.
What it cannot settle, and what would
It measures nothing that is specifically female. There is no estradiol, no progesterone, no LH or FSH, no testosterone, no SHBG and no thyroid antibody on this list. What makes it a female starter is that two of the seven — Ferritin and TSH (Thyroid-Stimulating Hormone) — were chosen on prevalence in women Cancado 2025. That is a sound reason to include them and not a reason to expect a cycle, a perimenopause or a PCOS question to be answered here.
A normal Comprehensive Metabolic Panel (CMP) does not mean a normal liver. Steatotic liver disease is common in people with entirely normal liver enzymes, which is the finding a middle-aged overweight cohort study was built to quantify van Son 2024. The ALT on this panel rules out very little on its own.
It has no fasting insulin, no ApoB and no hs-CRP, which are the three cheap additions that most often change what somebody does. Without a CRP the Ferritin is also uncorrected for inflammation, which is the adjustment the BRINDA methods exist to make Luo 2023.
And a single TSH (Thyroid-Stimulating Hormone) is not a thyroid diagnosis. In pooled trial data from older adults, a raised TSH normalized on its own in 60.8% within about a year, and in 39.9% even after a confirmatory repeat van der Spoel 2023. One value here is a reason to measure again, not a reason to start a tablet.
Draw conditions that decide whether the money is wasted
Cycle day does not apply to this panel, because nothing on it moves with the cycle — which is worth knowing precisely so you do not delay a $74 test waiting for day 3. Four things do matter.
- Draw in the week after a period rather than before one. Ferritin sits at its lowest just after the monthly loss, and that is the honest figure for what you are running on across a month Cancado 2025.
- Fasting is worth doing here but it is not sacred. The European consensus is that a lipid profile does not routinely require fasting Nordestgaard 2016; what fasting buys on this particular list is a cleaner triglyceride and a comparable glucose on the Comprehensive Metabolic Panel (CMP). If a fasted appointment means postponing by a month, take the non-fasted one.
- No biotin for 72 hours. TSH (Thyroid-Stimulating Hormone) is the only streptavidin-based assay on the panel, which makes this the cheapest instruction here: one supplement, one wrong result, and it is the result most likely to be acted on.
- Not within 2 weeks of an illness. Ferritin rises with inflammation and this panel has no CRP to reveal it Luo 2023, so an unwell week turns the most valuable marker on the list into a reassuring-looking number you cannot use.
How you would know it answered your question, and what each pattern means next
Five results and what each one costs to follow up, which is the decision this tier is really about.
- Ferritin under 30 ng/mL: the commonest actionable finding. Treat and retest at 12 weeks; if it falls again within 6 months, the question is where the iron is going rather than how much to take Cancado 2025.
- TSH (Thyroid-Stimulating Hormone) above 4 mIU/L: repeat at 8 to 12 weeks before anything else, and only then add Free T4 (Thyroxine) and Thyroid Antibodies (TPO + TgAb) van der Spoel 2023.
- HbA1c (Hemoglobin A1c) between 5.7% and 6.4% with a low ferritin: read them together rather than separately, because iron status shifts HbA1c on its own Chen 2022. Correct the iron, then repeat the HbA1c at 3 months, which is one red cell lifespan.
- Low mean cell volume on the Complete Blood Count (CBC) with Differential with a normal ferritin: not iron deficiency. Ask about thalassemia trait before starting anything Xiao 2021.
- All seven optimal and you still feel unwell: that is a real answer and it narrows the field. The next $50 is better spent on fasting insulin, ApoB and hs-CRP than on repeating these seven, and the panel above this one is where those live.
