Why your TNF-Alpha might be wrong

Most abnormal results are interference, not disease. These are the things that measurably move this specific marker — check them before you change anything. Each one says whether the number is wrong and should be repeated, badly timed and should be redrawn, or real with a cause worth explaining.

TNF-alpha is unstable and cytokine results depend heavily on how fast the sample was processed and frozen. Between-lab agreement is poor.

Circulating cytokines follow a daily rhythm with an overnight peak and a mid-morning trough, so two draws hours apart are not comparable.

What TNF-Alpha means in combination

The combination that actually means something. The antibody alone is weak evidence; the antibody plus a clinical picture plus objective inflammation is a referral. Unexplained low white cells, platelets or haemoglobin carry more weight here than the titre does.

Rheumatology. If rheumatoid arthritis is the question, add anti-CCP — far more specific than rheumatoid factor and positive years before symptoms.
