TGF Beta-1
A signaling protein central to tissue repair, fibrosis and immune regulation.
A more specialized inflammatory marker, relevant to fibrotic processes — including liver and cardiac fibrosis. Of interest to anyone concerned about long-term tissue remodeling from years of hormone use.
Serum and plasma TGF-β1 are not the same measurement, and the difference is enormous. Platelets carry large stores of TGF-β1 and release them during clotting, so a serum value largely reports platelet count and how the tube was handled — how long before it was spun, how hard, how cold. Plasma values are far lower and far more sensitive to pre-analytical technique. Most of TGF-β1 also circulates latent, and total-versus-active measurement depends on an acid-activation step that varies between kits. The consequence: two labs can return numbers that differ several-fold on the same person, which is why a target figure would be meaningless without naming the lab and the method behind it.

What to do next. Handle this result with more caution than most, because sample handling affects it substantially and reference ranges differ between laboratories. If the underlying question is chronic inflammation, hs-CRP is more reproducible and far better validated. VIP and low-dose naltrexone are the Vault entries most often discussed in this context, both on mechanism rather than trial evidence.
What TGF Beta-1 actually measures — the analyte, and the assay
Almost none of the TGF-β1 in you is switched on, and the assay decides whether that counts. The protein is secreted as a 25 kDa dimer still wrapped in its own propeptide, the latency-associated peptide, which is in turn tethered to a latent TGF-β binding protein. In that state the growth factor physically cannot reach its receptor. Releasing it is a local, mechanical event — integrin traction at a cell surface, thrombospondin, proteases, acid pH — and it happens in tissue, not in a tube.
So a laboratory has to choose what to report, and the choice changes the number by an order of magnitude. Most commercial immunoassays add an acid step that strips the latency peptide off, then neutralize and measure everything — that is a total result. Measuring what was already free gives an active result. Measuring the latent pool on purpose needs an assay raised against the latency-associated peptide itself Arestrom 2012, and measuring biological activity rather than protein needs a cell bioassay Khan 2012. Four legitimate answers, one analyte name.
The original human survey shows how wide that gets. Across human sera, mean acid-activatable plus active TGF-β was 330 pmol/L with a range from under 4 pmol/L to 1,400 pmol/L; mean active TGF-β was 230 pmol/L, range under 20 to 1,400 pmol/L Grainger 1995. For a 25 kDa dimer, 330 pmol/L is roughly 8 ng/mL — and the healthy spread inside a single method was more than three hundredfold.
Different immunoassays for the same molecule were compared formally as far back as 1997, because they were already known not to agree Kropf 1997. Nothing since has produced an international reference material that would let two laboratories' TGF-β1 values be placed on the same scale.
TGF Beta-1: what changes the blood, and what only changes the reading
What changes the growth factor in your body:
- Your platelet mass. Platelets are a major circulating reservoir of TGF-β, carried in alpha granules Karolczak 2021. More platelets genuinely means more of the protein available in blood — and it also means a bigger artifact, which is why this marker sits on the boundary between the two lists.
- Drug exposure that acts on the same cells. In 52 allograft recipients, mean TGF-β1 was 28,090 ± 26,860 pg/mL on cyclosporine against 7,173 ± 10,610 pg/mL on tacrolimus (P < 0.002) — a near-fourfold difference attributable to which immunosuppressant somebody was taking Coupes 2001.
- Active tissue repair and fibrogenesis, which is the process readers are usually trying to see. It is real, and it is the smallest of the three effects on this list.
What changes only the reading — and on this marker the second list dominates the first so completely that a value collected without a stated protocol is close to uninterpretable:
- How the blood was drawn. This is the finding that should govern the whole page. Ten healthy volunteers were bled twice by two different venipuncture techniques. With careful collection, 8 of 10 plasma samples had no detectable TGF-β1 at all — below 100 pg/mL. In the paired samples drawn by ordinary vacutainer, all 10 were measurable, median 7.70 ng/mL, interquartile range 5.87 to 13.64 Coupes 2001. Same volunteers, same morning, same circulation: a roughly seventy-fold difference produced by the needle and the technique.
- Serum or plasma. Clotting is deliberate platelet degranulation. A serum tube instructs the platelets to empty their granules into the sample you are about to measure Karolczak 2021.
- Time to the centrifuge and the spin itself. Platelet-poor plasma and platelet-rich plasma are different samples; an unspun tube left on a bench is a third one.
- Whether the assay acid-activated your sample, which decides whether you are reading the latent pool as well as the free one Grainger 1995 Arestrom 2012.
- Which kit, and whether it was an immunoassay at all. Protein-based and bioassay-based measurements of active and total TGF-β answer different questions Khan 2012, and immunoassays have disagreed with each other since they were first compared Kropf 1997.
- A freeze-thaw cycle on anything that still contains platelets, which lyses them and releases the contents into the measurement.
Reference interval or decision threshold — which kind of number TGF Beta-1 is
There is no reference interval for this marker in any meaningful sense, and a report that prints one is printing the kit's insert. A reference interval is the central 95% of a defined healthy population measured by a stated method. Both halves of that definition fail here: the method is not standardized between manufacturers Kropf 1997, and the healthy distribution inside one method already spanned under 4 to 1,400 pmol/L Grainger 1995.
It is certainly not a decision threshold. No value of circulating TGF-β1 has been tied to an outcome in a way that triggers an action, and no treatment is titrated against it.
What the number can legitimately be used for is comparison with itself, in one laboratory, on one platform, with the collection protocol held fixed. Outside that, the between-run variance introduced by the draw is larger than any biological signal being looked for Coupes 2001.
The site's own position, stated in this marker's data row, is that no optimum exists. That is not caution for its own sake. It follows from the numbers above.
How you would know your TGF Beta-1 was wrong — and when to redraw
The biology's clock is the platelet's, and it is 8 to 10 days long. A platelet cohort turns over inside about a week and a half, so a genuine change in the platelet-borne pool cannot be faster than that. Nothing about a repeat draw at 48 hours can be a biological finding.
But the retest that matters here is not a second week's blood. It is a second tube from the same needle. Because the pre-analytical effect is roughly seventyfold and the biology is not, the first question a strange TGF-β1 raises is whether the sample made it.
How you would know the value was an artifact:
- Draw plasma and serum at the same sitting. If the serum runs an order of magnitude above the platelet-poor plasma, you have reproduced Coupes 2001 rather than discovered anything about yourself.
- Put a CBC beside it. A high TGF-β1 in someone with a platelet count at the top of the interval is the expected consequence of the count.
- Ask the laboratory two questions in writing — total or active, and acid-activated or not. If the answer is not on file, the result cannot be compared with any other result.
- Test the fibrosis story somewhere it can be tested. If the reason for measuring was tissue remodeling, an ELF Test and an hs-CRP are answerable numbers where this one is not.
Same laboratory, same assay, same tube, same technique, or do not compare the two values at all. That is a stricter instruction than this site gives for any other marker, and the seventyfold figure is why.
What TGF Beta-1 cannot tell you
It cannot tell you what your tissues are doing, because activation is local and this measurement is not. TGF-β1 is switched on at a cell surface by mechanical and enzymatic events in a specific place. A blood concentration is the pool that never got there.
It cannot distinguish fibrogenesis from platelet release. Those are the two dominant contributors to a circulating value and the assay cannot separate them Karolczak 2021.
It cannot be compared against a published figure. With no international standard and no harmonization program, a value of 12 ng/mL in one paper and 12 ng/mL in another are not the same measurement Kropf 1997.
It cannot be moved on purpose in a way anybody has shown to matter. No supplement, drug or training change has been demonstrated to alter a clinical outcome by way of a circulating TGF-β1 target.
The wrong inference readers actually draw is that a high TGF-β1 indicates fibrosis somewhere. The likelier explanation, by a wide margin, is the blood draw — and the honest version of the sentence is that the number reported the tube Coupes 2001.

What people use TGF Beta-1 to decide
Nobody orders a test for its own sake. TGF Beta-1 is on the test list for these pathways — each one links to what the pathway claims, and what its test list is read for before you spend anything on it.
Collagen synthesis has obligate cofactors — without vitamin C, cross-linking fails outright; without copper, lysyl oxidase can't finish the job. Supplying substrate while a cofactor is missing is the most common wasted effort in this pathway.
Distinguishes normal healing inflammation from a chronic smolder that never resolves. Persistently high CRP months after an injury is a resolution failure, and it responds to completely different things than acute inflammation does.
Why your TGF Beta-1 might be wrong
Most abnormal results are interference, not disease. Check these before you change anything. Each says whether the number is wrong (repeat it), badly timed (redraw it), or real with a cause.
Platelets are packed with TGF-beta1 and release it if the draw is slow or the sample is handled roughly — serum values run far higher than plasma for this reason alone.
Serum and plasma results are not comparable. A difficult draw inflates it substantially.
This is an acute-phase reactant — it moves substantially during any infection, injury or inflammatory episode, and the change has nothing to do with your baseline.
Wait two weeks after any illness, vaccination or injury before drawing or interpreting.
What TGF Beta-1 means in combination
One marker tells you a little; combinations tell you the story. These are the named patterns this one takes part in.
The combination that actually means something. The antibody alone is weak evidence; the antibody plus a clinical picture plus objective inflammation is a referral. Unexplained low white cells, platelets or hemoglobin carry more weight here than the titre does.
Rheumatology. If rheumatoid arthritis is the question, add anti-CCP — far more specific than rheumatoid factor and positive years before symptoms.

Frequently asked questions
Assay dependent — wide reference ranges. Ranges vary by laboratory and assay — always compare to the range printed on your own report.
No established optimum. Serum values largely report platelet release during clotting, so they are not comparable between labs. No guideline body publishes an optimal TGF-β1 concentration.
Persistent elevation may reflect ongoing fibrotic activity — interpret with organ-specific markers (ELF for liver, imaging for heart).
