The route matters more here than for most mushrooms — the clinical evidence is for intravenous lentinan, and extrapolating that to an oral extract is a real leap. Oral evidence is weaker. Shiitake also occasionally causes a distinctive flagellate dermatitis from lentinan when eaten raw or undercooked, which is harmless and alarming.

How Shiitake (Lentinan) actually works

Lentinan is a beta-1,3-glucan that activates macrophages and NK cells. It is used intravenously as a cancer adjuvant in Japan, where the systemic route bypasses the absorption problem that limits oral polysaccharides.

⚠️ Good to know: The approved cancer indication is an INJECTED purified fraction used with chemotherapy. Swallowing a mushroom capsule is not that, and any product implying otherwise is misleading you. Raw or undercooked shiitake can cause shiitake dermatitis — a striking whip-like rash.

What Shiitake (Lentinan) actually does

The interesting molecule in shiitake is not a glucan, and almost nobody selling it knows the molecule exists. Shiitake contains eritadenine, an acyclic adenosine analog. It inhibits S-adenosylhomocysteine hydrolase — the enzyme AHCY — with an IC50 of 7 nanomolar, and there is a published crystal structure of the enzyme with D-eritadenine bound in its active site Huang 2002. A nanomolar enzyme inhibitor with a solved co-crystal structure, present in a food, is not something this shelf usually has to offer.

What that enzyme does, and why it makes this a methylation story. S-adenosylmethionine is the universal methyl donor; every methyltransferase leaves behind S-adenosylhomocysteine, and AHCY is the only enzyme that clears it, hydrolyzing it to homocysteine and adenosine. SAH is a product inhibitor of essentially every SAM-dependent methyltransferase, so the SAM-to-SAH ratio sets cellular methylation capacity. Inhibit AHCY and SAH accumulates. That is a real and consequential pharmacology, and it belongs to a mushroom people buy for their immune system.

And the cholesterol claim runs through the same enzyme, not through HMG-CoA reductase. Eritadenine's hypocholesterolemic action in rats is mediated by a change in hepatic phospholipid metabolism — the phosphatidylethanolamine to phosphatidylcholine step, which is itself a SAM-dependent methylation — rather than by inhibiting cholesterol synthesis Sugiyama 1995. The lipid effect and the methylation effect are the same effect seen from two sides.

The glucan half, and what is different about this one. Lentinan is a beta-1,3-glucan with beta-1,6 branches whose activity is tied to a triple-helical tertiary structure in water. For maitake the argument is which linkage; for yeast glucan it is particulate against soluble; here the axis is conformation. Denature the helix and you have the same chemical formula and a different molecule. No retail shiitake product reports the conformation of its polysaccharide, and no assay a buyer can request measures it.

Cell, rodent, human — and where it stops

Enzyme. D-eritadenine bound to human S-adenosylhomocysteine hydrolase, IC50 7 nM, structure solved Huang 2002.

Rodent. Dietary eritadenine lowers cholesterol in rats through hepatic phospholipid remodeling Sugiyama 1995. Rat, oral, weeks — and dosed as a purified compound in chow rather than as mushroom.

Human, the mushroom. A four-week parallel-group study in 52 healthy participants eating either 5 g or 10 g of dried shiitake daily. Ex vivo proliferation of gamma-delta T cells rose 60% (p < 0.0001) and of NK-T cells about two-fold (p < 0.0001) Dai 2015. Read the intervention carefully: that is 5–10 grams of dried mushroom, which is food, not a capsule of extract.

Human, the drug. An individual-patient meta-analysis of lentinan pooled 650 patients from five trials in unresectable or recurrent gastric cancer and reported a hazard ratio of 0.80 (95% CI 0.68–0.95, stratified log-rank p = 0.011) Oba 2009. The randomized phase III that followed compared S-1 alone against S-1 plus lentinan in 154 and 155 patients: the two arms' median overall survival was 13.8 and 9.9 months and the difference did not reach significance (P = 0.208), while median time to treatment failure was 4.3 against 2.6 months (P < 0.001) Yoshino 2016.

Three obstacles, and they point in three different directions. The immune data come from a food dose of whole mushroom Dai 2015. The oncology data come from a purified polysaccharide given by injection alongside chemotherapy Oba 2009 Yoshino 2016. The capsule of extract on the shelf is neither, and has neither dataset. And the survival benefit that the meta-analysis found was not confirmed when a properly powered randomized trial went looking for it Yoshino 2016 — which is the usual fate of a pooled retrospective signal.

And Dai's outcome is a plate, not a person. A 60% change in ex vivo proliferation Dai 2015 is a laboratory measurement in healthy young adults. The trial did not count illnesses, and nobody has.

Shiitake (Lentinan) — which form, and does it matter

The word “lentinan” on a capsule is doing work it has not earned. It carries the credibility of an injectable oncology adjunct Oba 2009 across to an oral product that has never been compared with it. If a label uses the word, the two questions that matter are the beta-glucan assay and whether the polymer's helical conformation survived processing, and no retail product answers either.

Fruiting body against mycelium, with a shiitake-specific twist. Eritadenine is described from the mushroom itself Sugiyama 1995; whether mycelium grown on a grain substrate produces it at all is not a question any label answers, and it is the one molecule here with a nanomolar target Huang 2002. On this mushroom the fruiting-body preference has a second, sharper justification than the usual beta-glucan argument.

The cheapest effective form is a vegetable. Dai's intervention was 5–10 g of dried shiitake a day Dai 2015 — roughly 50–100 g fresh, a portion of food, at a fraction of the price of any capsule, and it is the only oral shiitake regimen with a randomized human immune read-out behind it.

Cooked, always. Raw and undercooked shiitake is the form that causes the one reaction you are genuinely likely to meet Nguyen 2017. Heat is the fix, and it is not a trade-off against the eritadenine, which survives cooking.

What would have to be true, and how you would know it was not

1. Homocysteine, and this is the prediction nobody on this shelf makes. If eritadenine inhibits AHCY at intakes a person can eat, the enzyme's own reaction predicts the direction: blocking the hydrolysis of SAH to homocysteine should push plasma total homocysteine down while intracellular SAH rises Huang 2002. Measure tHcy at baseline and 8 weeks on 5–10 g of dried shiitake daily. A fall is the mechanism showing itself in a human for the first time. A rise falsifies the simple model, which would be just as interesting.

2. Lipids, with a size attached. Predict a small fall in total and LDL cholesterol on a food dose over 8–12 weeks — and predict under 0.2 mmol/L, which is inside the noise of a single draw. Predict nothing at all from a 500 mg capsule, because the lipid mechanism belongs to a defined molecule at a dietary intake Sugiyama 1995, and a capsule does not deliver a meal.

3. The prediction that cuts against the product. Predict no change in how often you get ill. The one oral immune trial measured cells in a dish Dai 2015, and the oncology evidence is about an injected polymer in people receiving chemotherapy, where the phase III did not confirm a survival benefit Yoshino 2016. Neither supports a claim about your winter.

4. What will fool you. A private laboratory's immune panel. Lymphocyte subsets and NK percentages move for a dozen reasons and a 60% change in ex vivo proliferation Dai 2015 is not a statement about whether you will be ill in February.

What nobody has tested yet

Nobody has measured homocysteine, SAM or SAH in a human eating shiitake. The enzyme, the inhibitor, the potency and the crystal structure are all published Huang 2002 and the human measurement is a blood draw. That gap is the single most obvious unrun experiment in this cohort.

Nobody has quantified eritadenine in retail extracts. Without it there is no way to know whether a capsule delivers a microgram or a milligram of the one compound here with a nanomolar target.

Nobody has run an oral lentinan trial with the conformation characterized. The injectable evidence exists Oba 2009 Yoshino 2016; an oral trial that reported whether the helix survived would tell you whether an oral product can be the same drug at all.

And nobody has explained the phase III result. Time to treatment failure separated strongly at P < 0.001 while overall survival did not separate Yoshino 2016. That pattern usually means something real is happening that the survival endpoint is not capturing, and it has not been chased.

Shiitake (Lentinan) — its own safety story, not its category's

Shiitake dermatitis is the effect you are actually likely to meet, and it is unmistakable. A flagellate, whip-like linear erythema across the trunk and limbs, appearing a day or two after eating raw or undercooked shiitake, sometimes with photosensitivity; the clinical features have been collected in a systematic review Nguyen 2017. It is self-limiting, it is dramatic, and cooking prevents it.

The methylation caution, which is specific to this mushroom and appears on no label. Eritadenine is a nanomolar AHCY inhibitor Huang 2002. Anybody already running a heavy methylation-directed stack — high-dose methylfolate, methylcobalamin, betaine, SAMe — is combining that with a dietary compound acting on the same cycle from the other end. Nobody has studied the combination in a person, and the honest statement is that the interaction is plausible and unmeasured rather than known.

You cannot borrow the injectable's safety record either way. Lentinan's adverse-event profile was generated in oncology patients receiving chemotherapy under supervision Yoshino 2016. That record is not transferable to a capsule, in the same way its efficacy is not.

The rest. Mushroom allergy is an absolute exclusion. Mild antiplatelet activity is reported for this family and matters mainly around surgery. And if you are in cancer treatment, this belongs to your oncologist: the one thing the trials agree on is that lentinan was studied alongside chemotherapy and never instead of it Oba 2009.
