What MediClear (Detox Shake) actually does
The molecule this category is built on is not in the product. Sulforaphane is an isothiocyanate. What broccoli and broccoli seed contain is glucoraphanin, a stable glucosinolate that is sulforaphane's inert precursor. Converting one to the other requires a thioglucosidase — myrosinase — which the plant keeps physically separated from its glucosinolates until the tissue is damaged. That separation is a plant defense system, and it is also the reason a capsule of dried extract can contain plenty of precursor and release very little active compound.
Once formed, sulforaphane has a genuinely elegant mechanism. It is an electrophile that reacts with specific cysteine thiols on KEAP1, the adaptor protein that normally hands NRF2 to a ubiquitin ligase for destruction. Modify those cysteines and NRF2 escapes degradation, accumulates, enters the nucleus and drives transcription of the antioxidant response element battery — glutathione S-transferases, NAD(P)H quinone dehydrogenase 1, glutamate-cysteine ligase, heme oxygenase-1. Those are literally the phase II conjugation enzymes, so “supporting detoxification” is for once a mechanistically precise phrase rather than a marketing one.
Phase II conjugation consumes amino acids, which is why a protein base is not decoration. Glutathione is a tripeptide of glutamate, cysteine and glycine, and its synthesis is limited by cysteine availability. Sulfation depends on inorganic sulfate derived largely from methionine and cysteine. Glucuronidation consumes UDP-glucuronic acid derived from glucose. Inducing conjugation enzymes without supplying substrate raises capacity without raising throughput, which is a coherent argument for pairing the two.

What follows is the chemistry of the two things a formula of this type is built from, and where the claims for each of them actually come from.
The rate-limiting step, then, is conversion, and it has been modeled. A physiologically based model has been built to simulate sulforaphane bioavailability and kinetics from broccoli products Shekarri 2021, which is the right response to a system where the input is a precursor and the output depends on an enzyme that may or may not be present.
Cell, rodent, human — and where it stops
The cell biology is excellent, the human conversion step is the bottleneck, and one recent trial addresses the bottleneck directly.
In the dish and in a digestion model. Sulforaphane bioavailability from a glucoraphanin-rich broccoli seed extract was optimized in a dynamic gastric digestion model with absorption across Caco-2 monolayers Zhu 2024. That is a system built specifically to ask how much active compound survives the journey, which is the question the shelf skips.
In the gut microbiome, where the fallback conversion lives. When plant myrosinase has been destroyed by heat or processing, glucosinolates can still be hydrolyzed by bacterial thioglucosidase activity in the colon. The metabolism of glucosinolates by gut microbiota is its own literature Sikorska-Zimny 2021, and the important feature of it is that this capacity varies enormously between people. Two people taking the same capsule can generate very different amounts of sulforaphane.
In people, and this is the result that changes the category. A randomized clinical study added exogenous myrosinase from mustard seed to a glucoraphanin-rich broccoli seed extract and found it increased sulforaphane bioavailability Mastaloudis 2026. In one sentence: the missing enzyme was the problem, and supplying it fixed a measurable amount of the problem.
The obstacle to translation has been written up by the people who ran the trials. The challenges of designing and implementing clinical trials with broccoli sprouts, and of turning that evidence into public health action, are the subject of their own paper Fahey 2021. When the investigators publish a paper about why their field is hard to test, that is worth more to a reader than another positive biomarker study.
MediClear (Detox Shake) — which form, and does it matter
SGS on a label means sulforaphane glucosinolate, which means glucoraphanin, which means precursor. This is the single most important sentence on the page. A product declaring milligrams of sulforaphane glucosinolate has declared how much of the inert starting material it contains, not how much of the active compound the reader will make. Those two numbers are related by a conversion efficiency that is neither fixed nor printed.
Whether the product carries active myrosinase is the question that decides everything else. Myrosinase is a plant enzyme and it is heat-labile. Extracts processed with heat, and many capsules, carry precursor without functional enzyme. The randomized trial that added mustard-seed myrosinase to a glucoraphanin-rich extract and measured a bioavailability increase is the direct evidence that this matters in a person Mastaloudis 2026.
Without the enzyme, the conversion is outsourced to bacteria and the reader becomes the variable. Colonic microbiota hydrolyze glucosinolates with wide inter-individual variation Sikorska-Zimny 2021. That means a fixed dose produces a variable exposure, and it means antibiotic use, diet and transit time all move the delivered dose of a product whose label has not changed.
Stabilized sulforaphane is a third form and it has its own problem. Free sulforaphane is chemically reactive — that reactivity is the mechanism — and it degrades in storage. Products that supply preformed sulforaphane or a stabilized complex have solved the conversion problem and inherited a shelf-life problem, and there is no way to tell from outside the bottle which of those has been solved better. The physiologically based model exists because this whole space needed a way to reason about exposure without assuming it Shekarri 2021.
And a protein base is a form question of its own that this page will not answer for a specific product. Rice protein, pea protein and dairy protein differ in cysteine and methionine content, and cysteine is the rate-limiting substrate for glutathione synthesis. Which protein a given formula uses, and how much of it, is a matter for its filed label, and no filed label for this product has been read into the site's blend table.
What would have to be true, and how you would know it was not
1. Predict urinary sulforaphane metabolites, because that is the measurement that closes the loop. Sulforaphane is excreted as mercapturic acid pathway metabolites, and total dithiocarbamate output over 24 hours is the standard bioavailability read-out — it is the measure the myrosinase trial used the logic of Mastaloudis 2026. Prediction: output varies several-fold between individuals on the identical dose Sikorska-Zimny 2021.
2. Predict a recent course of antibiotics lowers the delivered dose. If bacterial thioglucosidase is doing the conversion, disrupting the microbiome should reduce sulforaphane output from a precursor-only product and should not affect a product supplying active enzyme. That is a clean, falsifiable and unrun experiment.
3. Predict the enzyme induction shows up before any clinical endpoint does. NRF2 target induction is measurable in peripheral blood mononuclear cells within days. Prediction: NQO1 and glutathione S-transferase expression rise inside a week while hs-CRP, ALT and GGT take months or never move at all in a healthy person. The gap between those two timescales is where most detox marketing lives.
4. Predict liver enzymes do not fall in somebody whose liver enzymes are normal. Baseline ALT, AST and GGT; repeat at twelve weeks. Prediction: no change, because there is no elevated value to correct. This one cuts directly against the product category and is trivial to test.
5. Predict the thyroid question is answerable and worth answering. Glucosinolate breakdown also yields thiocyanate, which competes with iodide at the sodium-iodide symporter. Baseline TSH and free T4, repeat at twelve weeks on a high-dose cruciferous extract. Prediction: no change in an iodine-replete person, and this is exactly the kind of thing the trial-design paper is warning is under-measured Fahey 2021.
What nobody has tested yet
Nobody has surveyed myrosinase activity across on-market products. The enzyme's presence is decisive Mastaloudis 2026 and it is not declared. A survey that measured active myrosinase and glucoraphanin content in thirty commercial products, and then computed the sulforaphane yield each would give, would reorganize this entire category and has not been published.
Nobody has tested the pairing that the formula concept assumes. The argument for combining an NRF2 inducer with a protein base is that induced conjugation enzymes need amino acid substrate. Whether adding protein raises glutathione conjugate output beyond what the inducer achieves alone has not been measured in a person. It is a two-arm study with a urinary endpoint.
Nobody knows who the poor converters are. The microbial conversion route varies widely between individuals Sikorska-Zimny 2021, and no one has identified the taxa or the marker that predicts it. Until they do, dosing this category by milligrams of precursor is dosing by hope.
And the clinical endpoints remain the field's own stated problem. The people running these trials have written about why translating the biomarker work into public health action is difficult Fahey 2021. Most published sulforaphane work reports enzyme induction rather than an outcome anybody would notice, and the distance between those has not been closed.
MediClear (Detox Shake) — its own safety story, not its category's
The first honest statement is about what is not known: this page does not assert the contents of the product. No filed Supplement Facts panel for it has been read into the site's blend table, so any safety statement here is about the ingredient classes such a formula is built from and not about a verified list. Anyone with an allergy or an interaction to check should read the label on the tub, and treat a product that does not publish a full quantitative panel as a product that has not answered the question.
The thyroid interaction is the class-level one worth planning around. Glucosinolate hydrolysis yields thiocyanate as well as isothiocyanate, and thiocyanate competes with iodide for transport into the thyroid. This is a competitive effect, so it matters most where iodine intake is marginal, and it argues for checking iodine status rather than for avoiding cruciferous compounds.
NRF2 induction is not automatically benign, and the honest version says so. The same transcriptional program that protects normal cells from electrophilic stress can protect established tumor cells from chemotherapy, and it also upregulates efflux transporters and conjugating enzymes that clear drugs faster. Anybody on active cancer treatment, or on a narrow-therapeutic-index medication, is adding a variable to a system that was carefully titrated.
Gastrointestinal upset is the common complaint and it has two possible sources. Isothiocyanates are irritant electrophiles, and a large protein load is its own gastrointestinal event. Which of the two is responsible is distinguishable by taking them at different times, and almost nobody bothers.
The structural risk of a detox formula is the word rather than the chemistry. The phase II induction is real Mastaloudis 2026, and the leap from induced enzymes to removing something a person is worried about is not supported by anything measured. The people who study this most closely have published on that gap themselves Fahey 2021.

How you would know if it worked
There is no blood test for this one. That is not a criticism — it is a fact about the effect, and it changes how you should judge it.
- What to watch: What you removed, not what you added. This is a hypoallergenic shake built to carry an elimination diet, so the read-out is the elimination: the one symptom you are chasing, scored 0 to 10 daily, through the removal phase and then through a deliberate one-food-at-a-time reintroduction. The reintroduction is the experiment. Removal on its own proves nothing.
- How long before it means anything: Two to four weeks of removal, then a week per food added back. A food that provokes something usually does it within 72 hours, so an open-ended shake habit with no reintroduction phase is a program with no endpoint and therefore no answer.
- What will fool you: The word detox. Nothing in this powder has been shown to remove a toxin from you and no test would show it if it had — what changed is that you stopped eating your usual diet, dropped some weight and slept differently, all in the same week. That is worth having and it is also four variables at once. If you are replacing meals to chase a symptom nobody has diagnosed, get the diagnosis first.
Run it one variable at a time. Starting three things in one week means a result you cannot attribute, which is the same as no result.
