The plant aspirin came from, and not a plant version of aspirin. Salicin is a prodrug: gut bacteria cleave the glucose, the liver oxidizes the resulting saligenin to salicylic acid. Do not use this as an aspirin substitute for cardiovascular protection — the pharmacology is explicit that it cannot do that job. As an analgesic it is real: the arthritis meta-analysis is positive at 120 to 240 mg of salicin daily. Every aspirin contraindication still applies, because the molecule that arrives is still salicylic acid: salicylate allergy, aspirin-exacerbated respiratory disease, ulcer, bleeding disorders, third-trimester pregnancy, and no use in children or teenagers during a viral illness. Tinnitus is the classic signal that the salicylate load is too high and the right response is to stop.

How White Willow Bark actually works

Salicin is a prodrug and the sequence is worth stating step by step, because the last step is what people get wrong. Gut bacteria hydrolyze salicin to saligenin, which is absorbed and oxidized in the liver to salicylic acid. Salicylic acid inhibits cyclooxygenase reversibly, by competitive occupancy. Acetylsalicylic acid does something categorically different: its acetyl group transfers to Ser530 of COX-1, permanently inactivating the enzyme, and because platelets have no nucleus and cannot resynthesize it, the effect lasts the platelet's life. Willow bark has no acetyl group to transfer. It therefore has analgesic and anti-inflammatory activity and cannot produce the irreversible platelet inhibition that low-dose aspirin is prescribed for. Adding to the picture, the measured anti-inflammatory activity of standardized Salix extracts exceeds what their delivered salicylate accounts for, and analytical work attributes part of it to the polyphenol fraction.

⚠️ Good to know: It is not a cardioprotective aspirin substitute and it must not be used as one. The antiplatelet effect of low-dose aspirin comes from irreversible acetylation of platelet COX-1, and salicin cannot acetylate anything.

What White Willow Bark actually does

Salicin is a prodrug, and the sequence has four steps of which the last one is where every misunderstanding lives.

Step one: bacteria. Salicin is salicyl alcohol carrying a glucose. Human enzymes do not remove it efficiently; colonic bacterial beta-glucosidases do, releasing saligenin. Step two: the liver. Saligenin is oxidized to salicylic acid. Step three: the enzyme. Salicylic acid inhibits cyclooxygenase reversibly, by competitive occupancy of the active-site channel, and reduces prostaglandin E2 production Phillips 2022. Step four, and this is the one people skip: aspirin does something categorically different. Acetylsalicylic acid transfers its acetyl group to Ser530 of COX-1, covalently and permanently inactivating the enzyme. Platelets have no nucleus and cannot make more, so the effect lasts the platelet's 7 to 10 day lifespan. That is the entire basis of low-dose aspirin for cardiovascular protection.

Willow bark has no acetyl group to transfer. It therefore has analgesic and anti-inflammatory activity and cannot produce irreversible platelet inhibition. It is not a natural aspirin; it is the molecule aspirin was made from, before the modification that made aspirin different.

The second mechanistic surprise is that the salicylate does not account for the activity. Analytical work identifying the salicylates in Salix cortex found the extract's anti-inflammatory effect is not explained by its salicin content alone Antoniadou 2021, and a direct comparison of Salix extracts against acetylsalicylic acid in human immune-cell systems found the two do not produce the same inflammatory profile Le 2021. The polyphenol and flavonoid fraction is the leading candidate for the difference.

So the honest mechanistic summary is a reversible COX inhibitor delivered late, at a low salicylate dose, alongside a polyphenol fraction that appears to be doing part of the work.

Cell, rodent, human — and where it stops

In vitro, against the drug itself. Salix cortex extracts and acetylsalicylic acid were compared in SARS-CoV-2 peptide and LPS-activated human systems and produced different anti-inflammatory profiles Le 2021. That is the cleanest available demonstration that the extract is not a dilute aspirin.

In an animal, on the actual enzyme product. Oral white willow bark was compared with intravenous flunixin meglumine, an NSAID, on prostaglandin E2 in healthy dairy calves Phillips 2022. This is a real pharmacodynamic endpoint in a live animal rather than a cell line, and it is the step most botanical analgesics have nothing for.

In people. A meta-analysis of randomized controlled trials of willow bark for pain relief in arthritis reports a measurable analgesic effect at standardized salicin doses Lin 2023. The trials cluster at 120 to 240 mg of salicin daily.

The specific obstacle to transfer is a dose comparison people make in their heads and get backwards. 240 mg of salicin yields considerably less salicylic acid than a 325 mg aspirin tablet, and it arrives hours later because the first step is bacterial. Readers who substitute willow bark for aspirin are making two errors at once: the salicylate dose is smaller, and the antiplatelet action they are substituting for does not exist in this molecule at any dose.

White Willow Bark — which form, and does it matter

Standardization here is to salicin, and unlike feverfew that marker really is a precursor of the active. Trials used extracts delivering 120 to 240 mg of salicin daily Lin 2023. Raw bark powder carries variable and usually much lower salicin, so a '400 mg willow bark' capsule with no percentage stated is an unknown dose.

Species is a real variable. Salix alba, S. purpurea, S. daphnoides and S. fragilis differ several-fold in salicin content, and pharmacopoeial extracts are usually blends adjusted to a target percentage rather than a single species Antoniadou 2021.

The exposure arithmetic that decides the timescale. Because step one is bacterial, time to peak salicylic acid after willow bark is on the order of 2 hours rather than the 30 to 40 minutes of an aspirin tablet, and the peak is lower and flatter. That predicts a slower, gentler onset and a longer useful interval, and it is the reason twice daily dosing is standard in the trial literature.

The polyphenol fraction is a form question nobody labels. Since part of the anti-inflammatory activity does not track salicin Antoniadou 2021 Le 2021, an extract selected purely to hit a salicin percentage may have been optimized for the wrong constituent. No commercial product states a polyphenol specification.

What would have to be true, and how you would know it was not

The efficacy prediction is a pain score with a defined onset window. At 120 to 240 mg of salicin daily, a measurable fall in an arthritis pain score should appear within one to two weeks Lin 2023. Because absorption depends on a bacterial step, relief within thirty minutes of the first dose is not what this pharmacology predicts, and somebody reporting it is describing expectation rather than salicylate.

The prediction that distinguishes it from aspirin, and it is testable. Low-dose aspirin measurably suppresses platelet aggregation and prolongs bleeding time for the life of the platelet. Willow bark at a therapeutic salicin dose should not produce equivalent irreversible platelet inhibition on a platelet function assay, because there is no acetyl group to transfer. If it did, the central claim of this page would be wrong — and until somebody runs it, nobody should use willow bark where aspirin was prescribed.

The safety read-out is a symptom, not a number. Tinnitus is the classic early sign of salicylate excess and it appears before anything else does. It means stop, not persist.

And hs-CRP is the marker worth watching for the anti-inflammatory claim, since the extract lowers prostaglandin E2 production in a live animal Phillips 2022 — a systemic inflammatory marker that does not move over eight weeks argues the effect is analgesic rather than anti-inflammatory in that person.

What nobody has tested yet

The polyphenol contribution has been identified and never quantified. Two independent lines of work say the activity exceeds what salicin explains Antoniadou 2021 Le 2021, and no study has isolated the non-salicylate fraction and tested it alone in people. Until somebody does, the industry is standardizing to a marker that carries part of the effect.

Nobody has measured the antiplatelet question directly in humans. The absence of irreversible COX-1 acetylation is inferred from chemistry, and it is a sound inference — but a platelet function study comparing standardized willow bark against 81 mg aspirin has not been published, and it is the study that would stop people substituting one for the other.

And the microbiome dependence is unexplored. If step one is bacterial deglycosylation, then interindividual variation in that capacity should produce variation in salicylic acid exposure, and a recent antibiotic course should reduce it. Neither has been measured.

White Willow Bark — its own safety story, not its category's

Every aspirin contraindication applies, because the molecule that arrives is salicylic acid. Salicylate allergy, aspirin-exacerbated respiratory disease, active peptic ulcer, and bleeding disorders are all exclusions, and none of them is softened by the product being botanical.

Do not give it to children or teenagers during a viral illness. Reye's syndrome is a salicylate association, and salicin becomes salicylic acid. This is the single most important sentence on the page, because willow bark is sold as a gentle herbal option and parents buy it as one.

Additive bleeding risk with anticoagulants, antiplatelets and NSAIDs — reversible COX inhibition still adds to bleeding risk even without the acetylation step, and herb-drug interaction reviews list willow among the phytomedicines that matter in inflammatory disease Lippert 2022.

And it has a cytochrome interaction profile of its own that aspirin does not. Work in human hepatocyte-like HepaRG cells found drug-drug interaction potential, cytotoxicity at higher concentrations and reactive oxygen species production from Salix cortex extracts Gomes 2021. That is a property of the extract, not of the salicylate, and it is the reason 'it is just plant aspirin' is wrong in the direction of underestimating risk as well as overestimating benefit.

Gastrointestinal irritation and tinnitus are the dose-related effects, and tinnitus is the one to act on.

Avoid in pregnancy, particularly the third trimester, on the same grounds as other salicylates: premature closure of the ductus arteriosus and bleeding risk at delivery.
