Strontium

Best-in-class: Strontium Citrate

Minerals✅ Clinically validated📊 Correlative data🧪 Theoretical

A bone supplement with a genuinely misleading measurement problem that almost nobody explains.

How Strontium actually works

Strontium sits directly below calcium in the periodic table and substitutes for it in the hydroxyapatite crystal of bone. Beyond acting as a mineral stand-in it appears to have a genuine dual signalling effect — stimulating osteoblast activity while inhibiting osteoclast resorption, possibly through the calcium-sensing receptor. That combination is unusual; most bone agents do one or the other.

How to read these tiers

These tiers tell you how much human evidence exists — not how well something works. This is the research space, and most of what’s in here is new rather than disproven. Something sitting at “theoretical” usually means nobody has funded the trial, not that the trial was run and failed.

The trap runs the other way too: something can be clinically validated and still do very little for you specifically. A statistically significant result in a study population is not a promise about your body.

✗ is a safety flag, not a grade. Where you see it, the concern is harm — not a disappointing trial. A compound tested for one purpose and found not to help there can still be worth studying somewhere else, so a negative result never gets rendered as a cross. It sits alongside the tier, because something can be both well-studied and genuinely risky.

My job is to tell you which one you’re looking at, and let you make the call. Grading something low isn’t me dismissing it — it’s me refusing to oversell it. This is the research space, and being able to reason forward from a mechanism matters as much as waiting for the trial.

Coach Cam’s take

The measurement artifact is the thing to understand before you are impressed by a result. Strontium is denser than calcium, so a DEXA scan reads bone that has incorporated it as denser than it truly is — reported density gains substantially overstate the real structural change. Take away from calcium, since they compete for the same absorption pathway. The pharmaceutical form used in Europe carried a cardiovascular signal that the citrate sold as a supplement has not been tested for either way.

⚠️ Good to know: ⚠️ **The DEXA problem:** strontium has a higher atomic number than calcium, so it absorbs more X-ray energy. Incorporating it into bone makes your DEXA scan read HIGHER without a proportional increase in actual bone strength. You can appear to be improving on paper while nothing real has changed. It also competes with calcium for absorption. Take it and your scan stops being interpretable.
⏱ Timing that matters for safety: **It inflates DEXA readings artefactually** - your follow-up scan will over-read
