What Phenibut actually does
Phenibut is gamma-aminobutyric acid with a phenyl ring bolted onto the beta carbon, and that ring is the entire design. GABA itself does not cross the blood-brain barrier in useful amounts — it is small, zwitterionic and hydrophilic, and the barrier is built to exclude exactly that. Adding an aromatic ring raises lipophilicity enough to get a GABA-shaped molecule into the brain. Everything else about the compound follows from having solved a delivery problem rather than a receptor problem.
The received description is that it is a GABA-B agonist. The binding data say its higher-affinity target is something else. Radioligand work using gabapentin as a subunit-selective probe measured equilibrium dissociation constants at the alpha-2-delta subunit of the voltage-dependent calcium channel: R-phenibut 23 microM, S-phenibut 39 microM, baclofen 156 microM, gabapentin 0.05 microM. The authors state plainly that R-phenibut's affinity for alpha-2-delta is four times higher than its affinity for the GABA-B receptor Zvejniece 2015.
Two things fall out of those four numbers, and both are load-bearing. First, this is a gabapentinoid as much as it is a GABA-B agonist, and the alpha-2-delta subunit is the target of pregabalin and gabapentin — a completely different clinical class with a completely different risk profile. Second, look at the scale: 23 microM against gabapentin's 0.05 microM is roughly 460-fold weaker. That is why the dose is measured in hundreds of milligrams and not in single milligrams. A low-affinity ligand needs a high brain concentration, and a compound that needs a high brain concentration has a narrow gap between the dose that works and the dose that does something else.
The pharmacology was separated experimentally rather than assumed. R-phenibut's anti-nociceptive effect in the formalin-induced paw-licking test was not blocked by the GABA-B-selective antagonist CGP35348, and both enantiomers relieved mechanical and thermal allodynia after chronic constriction injury of the sciatic nerve — while at doses up to 100 mg/kg R-phenibut did not affect pentylenetetrazole-induced seizures Zvejniece 2015. An effect that survives GABA-B blockade is not a GABA-B effect.
Only one enantiomer matters, and the product is racemic. R-phenibut carries the GABA-B activity; S-phenibut does not bind GABA-B at all Zvejniece 2015. A racemic gram is therefore roughly half a gram of the active enantiomer plus half a gram of a compound that still binds alpha-2-delta and still has to be cleared. Nobody sells the resolved enantiomer, and nobody labels which is in the bottle.
Cell, rodent, human — and where it stops
Step one, binding, in rat brain membranes. Subunit-selective radioligand displacement, four compounds, one assay Zvejniece 2015. Clean, quantitative, and a long way from a person.
Step two, rodent behavior and rodent injury models. Formalin-induced paw licking and chronic constriction injury for pain Zvejniece 2015; a separate experimental traumatic brain injury model reporting mitochondrial-protective effects of R-phenibut Kupats 2020. Species: rat. Route: injected. Endpoints: nociceptive behavior and mitochondrial function. None of these is anxiety, sociability or sleep — the three things the compound is actually taken for.
Step three, humans — and this is where the chain breaks. There is no randomized controlled trial of phenibut for any indication in the indexed literature. What exists in humans is a case literature, and it is a case literature about harm: dependence and a withdrawal syndrome requiring management Ahuja 2018, psychomotor agitation non-responsive to treatment as a presentation of withdrawal Esposito 2021, acute psychosis after ingestion Acosta 2021, and inclusion in a review of adverse-event reports for novel psychoactive substances between 2017 and 2020 Mohr 2022.
State that plainly, because the asymmetry is the finding. The efficacy evidence in humans is anecdote and the harm evidence in humans is published. That is the exact reverse of the usual shape on this site, where a compound has promising human trials and unknown long-term safety. Here the long-term safety signal is the documented part.
The obstacle, and it is a chemistry problem as much as a trial problem. The comparator that shows what a proper evaluation looks like is picamilon — another GABA analog marketed as a nootropic, which was examined against the criteria for what may legally be sold as a supplement and analyzed as a pharmacological agent rather than accepted as one Santillo 2023. Phenibut has had no equivalent scrutiny in most markets, which is why the material's identity, dose per scoop and enantiomeric composition are all unverified at the point of purchase.
Phenibut pharmacokinetics — how much of it actually gets in
The card says ~5 hours. No human pharmacokinetic study of phenibut appears in the indexed literature, so that number cannot be sourced — and the clinical picture is inconsistent with it in a way that matters.
What can be reasoned from structure, and it is a lot. Phenibut is a small zwitterionic amino acid analog of roughly 179 daltons with a free carboxylate and a free amine. Molecules of that description are poor substrates for cytochrome P450 oxidation and are generally cleared unchanged by the kidney. Oral absorption is likely carrier-mediated rather than passive — this is the same structural class whose transport across the gut and the blood-brain barrier uses amino acid carriers — and carrier-mediated absorption saturates, which predicts that doubling an oral dose does not double exposure at the top of the range.
Renal clearance is the prediction with real consequences. If elimination is renal and largely unchanged, then reduced kidney function raises exposure proportionally, and there is no hepatic route to compensate. That is testable with a single creatinine and cystatin C, and it is the mechanism by which an ordinary dose becomes a large one in a person who has no idea their filtration is reduced.
The number that does not fit. A drug with a five-hour half-life is functionally gone in a little over a day. The published human record describes a withdrawal syndrome lasting days to weeks and requiring active management Ahuja 2018Esposito 2021. Those two facts are only compatible if the syndrome is driven by receptor and channel adaptation rather than by residual drug — which is exactly what a low-affinity ligand occupying alpha-2-delta and GABA-B at high concentrations for weeks would be expected to produce. Clearly labeled as extrapolation: the duration of withdrawal is a measure of how far the system moved, not of how long the molecule lasts, and tapering schedules built from the half-life will therefore be too fast.
Comparison with the injectable route, which does not exist here. Every human report involves oral dosing; there is no injected preparation and no reason to make one, because the compound was designed for oral use and its whole point is crossing a barrier that an injection does not bypass. The absence of an injectable comparator means there is no way to separate absorption from elimination in the human data, which is one more reason the quoted half-life is unverifiable.
What would have to be true, and how you would know it was not
Three predictions. The first is a target-engagement test nobody has run, the second is a safety marker, and the third argues against the way the compound is usually used.
1. Growth hormone should rise after a dose if GABA-B is really being engaged — and that makes GH the cheapest possible target-engagement assay. GABA-B agonists are used as neuroendocrine probes precisely because activating that receptor provokes GH release. Since the binding data put phenibut's GABA-B affinity below its alpha-2-delta affinity Zvejniece 2015, the prediction is specific and could easily be wrong: draw growth hormone and prolactin at baseline and at 60, 120 and 180 minutes after a single oral dose. If GH rises, GABA-B is engaged at ordinary oral doses. If it does not, the anxiolysis people report is running through the calcium channel subunit, and phenibut should be reasoned about as a weak gabapentinoid rather than as an oral baclofen. This experiment has never been published and it would take one morning.
2. Renal function is the exposure variable, so measure it first. For a small polar amino acid analog with no obvious hepatic route, clearance is renal. Prediction: cystatin C and a CMP at baseline will identify the people in whom a standard dose produces a much larger exposure. This is not a monitoring suggestion for later — it is a before-you-start measurement, because the group at risk cannot feel their reduced filtration.
3. Against the compound: tolerance should appear on a timescale of days, not months, and the dose people land on is evidence of it. A ligand with a 23 microM dissociation constant Zvejniece 2015 is occupying its target only at high concentration, so adaptation shifts the required dose quickly. Prediction: on any schedule more frequent than about twice weekly, the dose required for the same subjective effect rises measurably within two to three weeks, and the published human record is what that trajectory ends in Ahuja 2018Esposito 2021Acosta 2021. The falsifier is simple and the person taking it is the only one who can run it: a written dose log with dates. If the numbers on the log are flat after a month of regular use, this prediction is wrong.
What nobody has tested yet
Four things that are genuinely unknown, and one of them is embarrassing for the field.
Nobody has published a human pharmacokinetic curve. Not absorption, not half-life, not the effect of food, not renal dependence. A compound sold by the kilogram and taken by the gram has no published human exposure data at all, which is why the number on every card and every forum post traces back to nothing.
Nobody has compared the enantiomers in a person. R-phenibut carries the GABA-B activity and S-phenibut does not bind GABA-B Zvejniece 2015, and both bind alpha-2-delta with the same order of affinity. Whether the racemate's subjective effect is R, S or both has never been tested in humans, and it is a straightforward crossover with resolved material.
Whether the mitochondrial finding means anything outside injury. R-phenibut showed mitochondrial-protective effects after experimental traumatic brain injury in rats Kupats 2020. Whether that is a property of the compound or a property of the injury model — and whether anything analogous happens in an uninjured brain — is untested in any species.
What the actual content of retail material is. Adverse-event surveillance for novel psychoactive substances treats phenibut as one of a class of products whose composition is not guaranteed Mohr 2022, and the regulatory analysis applied to a sibling GABA analog found that the question of what may legally be sold is separate from the question of what is in the container Santillo 2023. Independent assay of retail phenibut for identity, purity and enantiomeric ratio has not been published.
Phenibut — its own safety story, not its class's
This compound's own risk is dependence, and unlike almost everything else in the Vault the human evidence for the harm is stronger than the human evidence for the benefit.
The withdrawal syndrome is documented and it is not mild. The case literature describes dependence requiring managed withdrawal Ahuja 2018 and a presentation of psychomotor agitation non-responsive to initial treatment that turned out to be phenibut withdrawal Esposito 2021. The clinical shape resembles sedative-hypnotic withdrawal, which is the category of withdrawal that can be dangerous rather than merely unpleasant.
Acute psychosis has been reported after ingestion. Acosta 2021 A single case report is a single case report; it is also the kind of event that does not get reported when it is mild, and it belongs on this page because nothing in the way the compound is marketed prepares anyone for the possibility.
The mechanism explains the dependence rather than excusing it. A low-affinity ligand at two inhibitory targets, taken at gram-scale doses to reach occupancy Zvejniece 2015, produces exactly the conditions for rapid adaptation: large receptor occupancy, frequent dosing, and a wide gap between the concentration that works and the concentration that is toxic. Compounds that need grams to work are compounds where tolerance is a pharmacological prediction, not a personal failing.
Combination is where the serious harm sits. The alpha-2-delta and GABA-B actions are both inhibitory, and they add to alcohol, benzodiazepines, opioids and gabapentinoids. The specific overlap worth naming is with gabapentin and pregabalin, because they share the same target with roughly 460-fold higher affinity Zvejniece 2015 — running them together is two ligands on one subunit, not a second mechanism.
Where this page stops. Withdrawal from a sedative-hypnotic-like agent is a medical problem with a medical answer, and it is the one situation in this cohort where the correct next step is a clinician rather than a protocol. What the mechanism predicts
Derived from the molecule, not a trial.
- This group acts directly on established CNS receptors — GABA-B (phenibut), serotonergic and histaminergic (trazodone, doxepin), beta-adrenergic (propranolol), dopaminergic (cabergoline, apomorphine). These are pharmacological drugs, not research peptides, and the predicted problems are the known ones for each receptor.
- Phenibut is the one that needs saying plainly: it is physically addictive. GABA-B agonism produces tolerance within days of regular use, and withdrawal is genuinely severe — anxiety, insomnia, tremor, and in heavy users, psychosis and seizures. It is closer to a benzodiazepine than to a nootropic in this respect, and it is sold as though it were the latter.
- Propranolol blunts the physical symptoms of adrenaline. That predicts the useful effect and also the problem — it blunts the training response and masks hypoglycemia.
- Dopamine agonists predict nausea, orthostatic hypotension and, at the doses used in Parkinson's, impulse-control problems. Cabergoline's half-life is very long, so effects persist well past a dose.
What has actually been reported
- Phenibut dependence and withdrawal are well documented in case reports and poison-center data.
- Trazodone: sedation, orthostatic hypotension, and rarely priapism — which is a medical emergency.
- Abrupt propranolol cessation causes rebound tachycardia and hypertension. Do not stop a beta-blocker suddenly.
- Cabergoline at high cumulative doses is associated with cardiac valve changes; at the low doses used for prolactin this has not been shown.
How to reduce the risk
Same mechanism as the prediction.
- For phenibut, the only reliable mitigation is frequency: occasional use does not produce dependence, regular use does. There is no dose that makes daily use safe.
- Taper anything in this group rather than stopping abruptly.
- Take the first dose of anything with orthostatic effects at home, sitting down.
