Reverse T3

Also known as: rT3

An inactive mirror-image of T3. Under stress the body shunts T4 into reverse T3 instead of active T3 — effectively applying the metabolic brakes.

The plan of attack

In this order. Most people start at step four, which is why they change five things at once and learn nothing.

  1. Confirm the number is real
    Biotin (B7). Biotin interferes with the streptavidin-biotin immunoassays most hormone tests run on. Depending on the assay design it pushes the result falsely HIGH or falsely LOW — there is no single direction to correct for. Hair, skin and nail products are the usual source, and doses in those are far above the RDA. Stop biotin 48–72 hours before the draw, and check every supplement label rather than just the one labelled biotin.
  2. Read it with its partner
    Always order alongside Free T3 — the ratio carries the information, not the single value. Draw it alongside: Free T3 (Triiodothyronine), TSH (Thyroid-Stimulating Hormone), Cortisol (AM).
  3. Work out which direction is yours
    If it's high — Metabolic down-regulation — fatigue, cold, stalled progress despite adherence.
    If it's low — Rarely a concern in isolation.
  4. Fix it in this order
    Nutrition. Restore calories and carbohydrate; end the extended deficit. This is the primary lever.
    Lifestyle. Deload, sleep, reduce total stress load, treat underlying illness or inflammation.
    Supplements. Selenium, zinc, iron (if ferritin low) support proper deiodination toward active T3.

Most abnormal results are interference, not disease. These are the things that measurably move this specific marker — check them before you change anything. Each one says whether the number is wrong and should be repeated, badly timed and should be redrawn, or real with a cause worth explaining.

Metabolic adaptation to sustained energy deficit. The body shunts T4 into inactive reverse T3 and down-regulates the whole endocrine axis. TSH stays normal, so a basic thyroid panel misses it entirely.

Restore calories and carbohydrate, take a genuine diet break, deload, sleep. Check ferritin and selenium — both are required for conversion. This is physiology, not a broken thyroid.

Converting T4 into active T3 needs selenium, iron, zinc and adequate energy. When those are short — or under stress and illness — you make inactive reverse T3 instead.

Adding more T4 to a conversion problem does not work. Correct ferritin and selenium first, address energy availability and stress, and retest in 8–12 weeks before changing any thyroid medication.
