The primary adrenal stress hormone, following a strong circadian rhythm — peaking 30–45 min after waking and falling through the day.
Chronically elevated cortisol degrades body composition, suppresses testosterone and T4→T3 conversion, impairs sleep and recovery, and drives insulin resistance and visceral fat.
The plan of attack
In this order. Most people start at step four, which is why they change five things at once and learn nothing.
- Confirm the number is real
Steep diurnal rhythm. Cortisol peaks within an hour of waking and falls all day. A midday value is close to uninterpretable against a morning reference range. Draw 7–9am, and use the same window on every repeat. - Read it with its partner
Draw between 7–9am — an afternoon cortisol can't be compared to a morning reference range, which is the most common misinterpretation of this marker. Draw it alongside: DHEA-S, Total Testosterone, Free T3 (Triiodothyronine). - Work out which direction is yours
If it's high — Central fat gain, muscle loss, poor sleep, suppressed testosterone and thyroid conversion, elevated glucose.
If it's low — Fatigue, low blood pressure, salt craving, hypoglycemia, dizziness on standing. Suspected adrenal insufficiency is a medical evaluation, not a supplement problem. - Fix it in this order
Nutrition. Stop under-eating — energy deficit is a physiological stressor that raises cortisol. Adequate carbohydrate around training blunts the cortisol response. Limit caffeine after midday and alcohol entirely.
Lifestyle. This is where the real fix lives: sleep 7–9h on a consistent schedule, deload from training every 4–8 weeks, morning daylight to anchor the rhythm, breathwork/meditation, and reduce chronic stress load.
Supplements. Ashwagandha (KSM-66) has the best RCT evidence — roughly 25–30% cortisol reduction. Phosphatidylserine blunts exercise-induced cortisol. Rhodiola for stress-related fatigue. Magnesium glycinate for sleep.
Hormones. Suspected Cushing's or adrenal insufficiency requires endocrine workup (dexamethasone suppression, ACTH stimulation). Not a self-managed situation.
Compounds. No peptide is an appropriate cortisol treatment. If IGF-1, testosterone and thyroid all look mediocre simultaneously, elevated cortisol is often the upstream driver — fix that first and the others frequently follow.
Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail. - Retest
As clinically indicated; 4-point salivary or DUTG testing gives rhythm rather than a single point. Change one thing at a time, or the retest can't tell you which thing worked.

Why your Cortisol (AM) might be wrong
Most abnormal results are interference, not disease. These are the things that measurably move this specific marker — check them before you change anything. Each one says whether the number is wrong and should be repeated, badly timed and should be redrawn, or real with a cause worth explaining.

Suppress your own cortisol. Prednisolone also cross-reacts with some cortisol assays, reading falsely high.
Declare every steroid — nasal sprays and skin creams included.
Raises cortisol-binding globulin, so TOTAL cortisol reads high while free cortisol is normal.
A high cortisol on the pill is usually binding protein, not Cushing's.
Some people mount a genuine cortisol response to venepuncture.
Sit quietly for 15 minutes first. Repeat an isolated high value.
Muscle releases AST/ALT and CK; inflammation markers spike. Routinely misread as liver damage.
No hard or novel training for 48–72 hours before the draw.
Raise glucose and white cells; suppress your own cortisol and ACTH production.
Note recent steroid use — including injections — when interpreting. Never stop long-term steroids abruptly.
Testosterone and cortisol peak in the morning; an afternoon testosterone can read 20–25% lower.
Always draw between 7–10am and keep the same window for every retest.
What Cortisol (AM) means in combination
A single marker tells you a little. Combinations tell you the story — these are the named patterns this marker takes part in, and what each one points at.
Sustained overreaching — training load, sleep debt, energy deficit or life stress. The body down-regulates output across the board, which looks like several separate hormone problems and is really one.
No peptide or supplement outruns this. Deload, sleep, eat more, and cut total stressors for 4–8 weeks, then retest. If morning cortisol is genuinely low rather than low-normal, adrenal insufficiency needs excluding properly.
Chronically elevated cortisol drives insulin resistance, central adiposity and blood pressure directly. Most cases are lifestyle and sleep; a minority are Cushing's and get missed for years.
Address sleep, alcohol, caffeine timing and training load first, then retest. Persistently high cortisol with the physical features — central weight, easy bruising, purple striae, proximal weakness — needs a proper workup, not a cortisol-lowering supplement.
The single most important pairing in adrenal medicine and the reason cortisol alone is close to useless. High ACTH with low cortisol means the gland is failing and the pituitary is compensating — primary adrenal insufficiency. Low or inappropriately normal ACTH means the signal never arrived, which is a pituitary problem with a different workup entirely.
Either way this is a doctor, promptly — untreated adrenal insufficiency can kill in a crisis. Check sodium and potassium on the same CMP: low sodium with high potassium supports the primary pattern.
