A tumor marker associated with pancreatic and biliary cancers, used for monitoring known disease.

Same honest caveat as all tumor markers: not a screening test. It's elevated in many benign conditions and normal in some cancers — and roughly 5–10% of people genetically can't produce it at all.

Two facts make CA19-9 a poor number to own. Roughly 5–10% of people are Lewis antigen-negative and cannot produce CA19-9 at all — their result is zero whether or not disease is present. And it rises in benign biliary obstruction, cholangitis, pancreatitis, cirrhosis and diabetes, sometimes to values that look alarming. Its defensible use is following a known cancer, where the trend carries the signal.

What was your CA 19-9 result?

Within reference range — Not a rule-out. Around 5–10% of people lack the Lewis antigen entirely and cannot produce CA 19-9 at all, so their result is always normal regardless of disease.
Mildly raised — Raised in pancreatitis, cholangitis, biliary obstruction, cirrhosis and several benign conditions. Obstruction alone can raise it substantially.
Substantially raised — Needs prompt assessment. Used mainly to monitor known pancreatic and biliary cancer rather than to detect it.

What to do next. This belongs with a clinician. CA 19-9 has poor specificity, and the most common reason for a high result in someone otherwise well is a benign biliary problem rather than malignancy — but that is a determination made with imaging, not with reassurance from a website.

What CA 19-9 actually measures — the analyte, and the assay

This page assumes a CA 19-9 result already exists. It is not a case for ordering one, and this site does not send anybody for it. In a person without symptoms the number raises alarms far more often than it finds disease, and the follow-up is imaging and sometimes a biopsy. What follows is how to read a figure that is already in front of somebody.

CA 19-9 is a carbohydrate, and that single fact drives everything else. The antigen is sialyl-Lewis a, a tetrasaccharide carried on mucins and glycolipids, and the assay detects the sugar rather than the protein carrying it Scara 2015. It is a blood-group-related structure, which is why an individual's ability to make it at all is genetically determined.

Between 5 and 10% of people cannot produce it, ever. Synthesis requires a functional fucosyltransferase 3 enzyme; people with an inactive Lewis genotype do not make sialyl-Lewis a and have essentially undetectable CA 19-9 regardless of disease Indellicato 2020. Lewis-antigen-negative pancreatic cancer has been described as an aggressive subgroup precisely because the marker cannot flag it Liu 2020. A CA 19-9 of zero in such a person is not good news; it is no news.

The converse also happens. Unexpected serum CA 19-9 elevation in Lewis-negative patients has been investigated and reported, which means the simple genotype-to-phenotype rule has exceptions and the assay can detect related structures Hamada 2012.

CA 19-9: what changes the blood, and what only changes the reading

What changes the CA 19-9 in you:

  1. Biliary obstruction, and it is the largest benign cause by far. Serum CA 19-9 in obstructive jaundice falls after biliary drainage in benign and malignant disease alike — enough that correction formulas have been proposed to interpret a value measured while the bile duct is obstructed Zhao 2021. A very high figure in a jaundiced person is often the obstruction speaking.
  2. Cholangitis, cholecystitis and choledocholithiasis, which raise it acutely.
  3. Pancreatitis, acute or chronic, and pancreatic duct obstruction.
  4. Cirrhosis, hepatitis and any cholestatic liver disease, including primary sclerosing cholangitis.
  5. Diabetes with poor glycemic control, and thyroid disease, both associated with modest elevations.
  6. Pancreatic, biliary, gastric and colorectal adenocarcinoma, which is what the assay was developed against Scara 2015.

What changes the number without changing you:

  1. Your Lewis genotype, which sets the ceiling on what the assay can ever report Indellicato 2020 Liu 2020.
  2. Which platform. Tumor marker harmonization assessed through global external quality assessment programs remains incomplete, so a value from one laboratory is not directly comparable to another's van Rossum 2024 Sturgeon 2016.
  3. Biological variation. The biological variation and reference change values of CA 19-9 in healthy individuals have been measured Erden 2008, and the reference change value is what decides whether a difference between two results is a change at all.
  4. Heterophilic and anti-animal antibodies, the standard sandwich-assay false positive, exactly as documented for the other tumor markers on this site.
  5. Drawing the sample while a duct is obstructed, which is the commonest reason a startling number turns out to mean something benign Zhao 2021.

Reference interval or decision threshold — which kind of number CA 19-9 is

The 37 U/mL cut-off is a specificity threshold from the assay's development, not a reference interval and not a biological boundary. It was set against a healthy reference group, and the distribution above it in unselected people is dominated by benign hepatobiliary disease Scara 2015.

Its positive predictive value in an asymptomatic person is poor. Pancreatic cancer is uncommon enough that most people above the cut-off do not have it, and the resulting investigations are invasive.

Its negative predictive value is undermined by genetics. In the 5–10% of people who cannot synthesize the antigen, a normal result carries no information at all Indellicato 2020 Liu 2020.

Where the number does work is as a series in someone with a known diagnosis. Trend after resection or during chemotherapy is an established use, and interpreting a trend requires knowing the reference change value rather than the cut-off Erden 2008 Scara 2015.

How you would know your CA 19-9 was wrong — and when to redraw

If a value is raised, the first question is whether the biliary tree is obstructed, and the second is whether to wait. CA 19-9 has a half-life measured in days, so a value driven by an obstruction falls substantially within 1 to 2 weeks of drainage Zhao 2021.

For a mild elevation with no jaundice and no symptoms, 4 to 8 weeks is a reasonable interval before repeating, because a transient inflammatory cause will have resolved and a progressive one will have moved.

Conditions that must match: the same laboratory and platform van Rossum 2024; the same biliary and glycemic state; and a difference large enough to exceed the reference change value before it counts as a change Erden 2008.

What would have to change for a second value to mean something:

  • Raised with jaundice or itch? The tests that matter are bilirubin, ALP and GGT inside the CMP, plus imaging. The tumor marker is the least informative number on that list Zhao 2021.
  • Raised with normal liver chemistry? Repeat before acting. A modest isolated elevation that returns toward normal is the commonest outcome Scara 2015.
  • Undetectable, and someone is treating that as reassurance? Ask whether the person is Lewis-negative, because a value of zero in that setting excludes nothing Indellicato 2020 Liu 2020.
  • Rising across two or three samples on the same platform? A trend exceeding the reference change value is the only pattern here that carries weight, and it belongs with a clinician Erden 2008.
  • Diabetes poorly controlled? HbA1c and the lipid panel address a benign contributor that is worth fixing for its own sake.

What CA 19-9 cannot tell you

It cannot screen. In people without symptoms the false alarms outnumber the findings, and each one is answered with imaging and sometimes a biopsy Scara 2015.

It cannot be interpreted in a Lewis-negative person at all, and nothing on a standard report says which people those are Indellicato 2020 Liu 2020.

It cannot separate malignant from benign obstruction. Both raise it, sometimes to the same degree, and drainage lowers it in both Zhao 2021.

It cannot be compared across laboratories, because harmonization is incomplete van Rossum 2024 Sturgeon 2016.

It cannot localize. Pancreatic, biliary, gastric, colorectal and ovarian tumors all raise the same antigen Scara 2015.

The wrong inference people actually draw is that a normal CA 19-9 excludes pancreatic disease and a raised one implies it. Between 5 and 10% of people are physiologically incapable of producing the antigen, and among those who can, the commonest reason for a high value is a blocked bile duct Indellicato 2020 Zhao 2021.

Most abnormal results are interference, not disease. Check these before you change anything. Each says whether the number is wrong (repeat it), badly timed (redraw it), or real with a cause.

One marker tells you a little; combinations tell you the story. These are the named patterns this one takes part in.

Tumor markers are monitoring tools, not screening tests, and every one of them rises in benign conditions. CA-125 with endometriosis, fibroids or any peritoneal irritation. CEA with smoking. CA 19-9 with gallstones and any biliary obstruction. Calcitonin with chronic PPI use and kidney impairment.

Interpret only in context, and never as a screen. The trend across serial measurements matters far more than one value.

CA 19-9 near zero — the false reassurance

Roughly 5–10% of people genetically cannot produce CA 19-9 at all. In them the marker reads near-zero even with advanced disease — which makes this the rare tumor marker whose failure mode is false reassurance rather than a false alarm.

A very low CA 19-9 does not exclude anything if you are Lewis-negative. Any real clinical suspicion needs imaging regardless of the number.
