Apolipoprotein A-1

Also known as: ApoA-1

The main structural protein of HDL particles — a direct count of your HDL particles, the way ApoB counts atherogenic ones.

The ApoB:ApoA-1 ratio is one of the strongest lipid predictors of heart attack risk in large international data (INTERHEART). Particularly relevant to anyone using androgens, which suppress HDL hard.

Standard — male
94–178 mg/dL
★ Optimal — male
>140 mg/dL, with an ApoB:ApoA-1 ratio <0.6 (lower is better).
Standard — female
101–199 mg/dL
★ Optimal — female
>150 mg/dL, same ratio target.
🔍 Why it happensLow: androgen use (especially orals), insulin resistance, sedentary behavior, smoking, genetics. High: aerobic exercise, moderate alcohol, estrogen, favorable genetics.
▲ If Apolipoprotein A-1 is highGenerally favorable.
▼ If Apolipoprotein A-1 is lowReduced reverse cholesterol transport and elevated cardiovascular risk — the classic androgen-user profile.

The plan of attack

In this order. Most people start at step four, which is why they change five things at once and learn nothing.

  1. Confirm the number is real
Androgens, and oestrogen in the other direction. Testosterone lowers ApoA1; oestrogen raises it. Both are real drug effects rather than changes in cardiovascular risk of the same size. Record your hormone status on the result — otherwise a low ApoA1 on TRT reads as a risk finding rather than a drug effect.
  2. Read it with its partner
Non-fasting acceptable. Draw it alongside: ApoB (Apolipoprotein B), Lipid Panel (Cholesterol, HDL, LDL, Triglycerides), hs-CRP (High-Sensitivity C-Reactive Protein).
  3. Work out which direction is yours
If it's high — Generally favorable.
If it's low — Reduced reverse cholesterol transport and elevated cardiovascular risk — the classic androgen-user profile.
  4. Fix it in this order
Nutrition. Olive oil and monounsaturated fats, oily fish, reduce refined carbohydrate and trans fats.
Lifestyle. Aerobic exercise is the most reliable HDL/ApoA-1 raiser. Stop smoking, lose visceral fat.
Supplements. Omega-3s, niacin (raises HDL but no proven outcome benefit). Honestly, exercise beats every supplement here.
Hormones. Minimize oral androgens — they are uniquely damaging to HDL. Injectables are far gentler. Estrogen raises ApoA-1 (part of why premenopausal women have lower cardiovascular risk).
Compounds. Run this alongside ApoB if you're on androgens — the ratio tells you far more than either alone.
Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail.
  5. Retest
Every 3–6 months on androgens. Change one thing at a time, or the retest can't tell you which thing worked.

How to fix it

🥩 Nutrition: Olive oil and monounsaturated fats, oily fish, reduce refined carbohydrate and trans fats.
💊 Supplements: Omega-3s, niacin (raises HDL but no proven outcome benefit). Honestly, exercise beats every supplement here.
🏃 Lifestyle: Aerobic exercise is the most reliable HDL/ApoA-1 raiser. Stop smoking, lose visceral fat.
⚕️ Hormones / medications: Minimize oral androgens — they are uniquely damaging to HDL. Injectables are far gentler. Estrogen raises ApoA-1 (part of why premenopausal women have lower cardiovascular risk).
🧬 Peptides: Run this alongside ApoB if you're on androgens — the ratio tells you far more than either alone.
⚡ Testing tip / TRT noteNon-fasting acceptable.
Retest: Every 3–6 months on androgens.

What Apolipoprotein A-1 is usually tested alongside

On its own, one marker is a data point. These panels include Apolipoprotein A-1 plus the markers that make it interpretable — each names every test, why it is on the list, and loads the whole set into your cart in one click at 10% off.

includes this + 8 more markers — Anyone over 30, anyone with a family history of early heart disease, or anyone who's been told their cholesterol is 'fine' and wants to know what that actually means.

Would you feel it? Symptoms Apolipoprotein A-1 helps explain

People rarely search for a marker — they search for how they feel. These are the complaints where this marker is worth checking, and whether it is a first-line test or a follow-up once the obvious causes are ruled out.

Why your Apolipoprotein A-1 might be wrong

Most abnormal results are interference, not disease. These are the things that measurably move this specific marker — check them before you change anything. Each one says whether the number is wrong and should be repeated, badly timed and should be redrawn, or real with a cause worth explaining.

💊 Androgens, and oestrogen in the other directionA real change — account for the cause

Testosterone lowers ApoA1; oestrogen raises it. Both are real drug effects rather than changes in cardiovascular risk of the same size.

Record your hormone status on the result — otherwise a low ApoA1 on TRT reads as a risk finding rather than a drug effect.

🏃 Acute illness or infectionA real change — retest once it passes

This is an acute-phase reactant — it moves substantially during any infection, injury or inflammatory episode, and the change has nothing to do with your baseline.

Wait two weeks after any illness, vaccination or injury before drawing or interpreting.

What Apolipoprotein A-1 means in combination

A single marker tells you a little. Combinations tell you the story — these are the named patterns this marker takes part in, and what each one points at.

Enhanced athlete — the lipid profile nobody looks at
HDL crashed (<30) · ApoB elevated · Apolipoprotein A-1 low · On orals or high-dose androgens

The single most under-monitored consequence of androgen use. Androgens — especially oral 17-alpha-alkylated compounds — suppress HDL dramatically and raise ApoB. There are no symptoms; the cost is cumulative arterial damage measured in decades.

Prefer injectables over orals and minimize oral duration. Add cardio, soluble fiber, omega-3s. Many long-term users treat ApoB pharmacologically (statin/ezetimibe) with a physician — that's a rational trade if you're committed to being on hormones for years. Track ApoB every 3 months, not just a standard lipid panel.

What to test next
