Apolipoprotein A-1
The main structural protein of HDL particles — a direct count of your HDL particles, the way ApoB counts atherogenic ones.
The ApoB:ApoA-1 ratio is one of the strongest lipid predictors of heart attack risk in large international data (INTERHEART). Particularly relevant to anyone using androgens, which suppress HDL hard.
The plan of attack
In this order. Most people start at step four, which is why they change five things at once and learn nothing.
- Confirm the number is real
Androgens, and oestrogen in the other direction. Testosterone lowers ApoA1; oestrogen raises it. Both are real drug effects rather than changes in cardiovascular risk of the same size. Record your hormone status on the result — otherwise a low ApoA1 on TRT reads as a risk finding rather than a drug effect. - Read it with its partner
Non-fasting acceptable. Draw it alongside: ApoB (Apolipoprotein B), Lipid Panel (Cholesterol, HDL, LDL, Triglycerides), hs-CRP (High-Sensitivity C-Reactive Protein). - Work out which direction is yours
If it's high — Generally favorable.
If it's low — Reduced reverse cholesterol transport and elevated cardiovascular risk — the classic androgen-user profile. - Fix it in this order
Nutrition. Olive oil and monounsaturated fats, oily fish, reduce refined carbohydrate and trans fats.
Lifestyle. Aerobic exercise is the most reliable HDL/ApoA-1 raiser. Stop smoking, lose visceral fat.
Supplements. Omega-3s, niacin (raises HDL but no proven outcome benefit). Honestly, exercise beats every supplement here.
Hormones. Minimize oral androgens — they are uniquely damaging to HDL. Injectables are far gentler. Estrogen raises ApoA-1 (part of why premenopausal women have lower cardiovascular risk).
Compounds. Run this alongside ApoB if you're on androgens — the ratio tells you far more than either alone.
Work down the list, not across it. Adding a compound on top of an unfixed diet is why generic protocols fail. - Retest
Every 3–6 months on androgens. Change one thing at a time, or the retest can't tell you which thing worked.

A single marker tells you a little. Combinations tell you the story — these are the named patterns this marker takes part in, and what each one points at.
The single most under-monitored consequence of androgen use. Androgens — especially oral 17-alpha-alkylated compounds — suppress HDL dramatically and raise ApoB. There are no symptoms; the cost is cumulative arterial damage measured in decades.
Prefer injectables over orals and minimize oral duration. Add cardio, soluble fiber, omega-3s. Many long-term users treat ApoB pharmacologically (statin/ezetimibe) with a physician — that's a rational trade if you're committed to being on hormones for years. Track ApoB every 3 months, not just a standard lipid panel.
