What Ovarian & Hormone Support actually does
Start with the filed label, because the card on this site describes a product this is not. Thorne's Ovarian Care, as recorded with the NIH Dietary Supplement Label Database, is a mixed-berry powder, one 3.57 g scoop containing: a 40:1 inositol blend of 2,000 mg myo-inositol and 50 mg D-chiro-inositol, 400 mcg of L-5-methyltetrahydrofolate (667 mcg DFE), 150 mg of a coenzyme Q10 phytosome, and 50 mg wild blueberry, 50 mg decaffeinated green tea extract and 25 mg pomegranate NIH 2024. There is no vitex, no dong quai, no black cohosh and no red clover — no estrogenic botanical of any kind. Everything below is written against that list.
Inositol is not a vitamin and not a hormone. It is a second messenger precursor, and the mechanism is specific. Myo-inositol is a cyclohexanehexol; an epimerase converts it to D-chiro-inositol, and that conversion is insulin-dependent. Both isomers are built into inositol phosphoglycans, the messengers released from glycosylphosphatidylinositol anchors when the insulin receptor is engaged — and the two do different jobs. The myo-inositol-containing messenger mediates glucose uptake; the D-chiro-containing one mediates glycogen synthesis. Different tissues therefore need different ratios of the two, which is the single fact that makes the number on this label meaningful.
Why the ovary specifically wants myo-inositol. Myo-inositol is the head group of phosphatidylinositol, and therefore the precursor of PIP2 and of inositol trisphosphate — the messenger that releases calcium from intracellular stores. FSH receptor signaling in granulosa cells runs through that arm, and follicular fluid is a myo-inositol-rich compartment. An ovary short of myo-inositol is an ovary whose FSH signal is degraded, whatever the FSH concentration in the blood.
And that gives the paradox the 40:1 ratio exists to solve. In insulin-resistant polycystic ovary syndrome, the epimerase is proposed to run too fast in ovarian tissue, converting myo-inositol to D-chiro-inositol and depleting the pool the FSH signal needs — which is why loading D-chiro-inositol alone can help the liver and hurt the oocyte. 2,000 to 50 is 40 to 1 NIH 2024, and 40:1 is roughly the ratio the two isomers hold in human plasma. That number is a mechanistic decision rather than a marketing one, and it is the most defensible thing on the label.
The hormonal chain, in the order it has to happen. Insulin suppresses hepatic transcription of sex hormone-binding globulin. Lower the insulin and SHBG rises; raise SHBG and more testosterone is bound, so free testosterone falls; and the androgen excess that drives anovulation eases. Nothing in this product touches an androgen receptor, an aromatase or an ovary directly. It is an insulin intervention with a hormonal read-out, and every prediction below follows that order.
Methylfolate, and why it is here rather than as decoration. L-5-MTHF is the substrate methionine synthase needs to remethylate homocysteine, and it enters the folate cycle without passing through dihydrofolate reductase or through MTHFR — the enzyme whose C677T variant, present on roughly 38% of unselected chromosomes, produces a thermolabile protein with reduced activity and raises plasma homocysteine in homozygotes Frosst 1995. This is a product bought by women trying to conceive, so the folate is doing preconception work as well as metabolic work.
The rest is an antioxidant argument and should be labeled as one. Coenzyme Q10 is the mobile electron carrier between complexes I and II and complex III of the respiratory chain, and the oocyte case for it is mitochondrial: an oocyte carries more mitochondria than any other human cell and its ATP supply is proposed to limit meiotic competence. Blueberry, green tea and pomegranate are polyphenols NIH 2024. None of the four has a named receptor here, and none of them appears in the trials below.
Cell, rodent, human — and where it stops
The founding trial used the other isomer, and its numbers are the largest in this file. Forty-four obese women with PCOS, half given D-chiro-inositol: the insulin area under the curve after glucose fell from 13,417 ± 11,572 to 5,158 ± 6,714 µU/mL per minute, serum free testosterone from 1.1 ± 0.8 to 0.5 ± 0.5 ng/dL, triglycerides from 184 ± 88 to 110 ± 61 mg/dL, and 19 of 22 women ovulated against 6 of 22 on placebo Nestler 1999. That is an enormous effect, it was published in 1999, and it is the reason anybody has heard of inositol.
The trial that matches this product's isomer is much more modest. Ninety-two women randomized, 45 to treatment and 47 to placebo, for 14 weeks, on 4 g of myo-inositol plus 400 mcg of folic acid: ovulation frequency 25% against 15%, and time to first ovulation 24.5 days (95% CI 18 to 31) against 40.5 days (95% CI 27 to 54) Gerli 2007. Set that beside 19-of-22 and the difference in scale is the honest picture of this field: the spectacular result belongs to a different isomer in a smaller, older, unreplicated trial.
Now the dose arithmetic, which is the point of this page. That trial gave 4 g of myo-inositol a day Gerli 2007. One scoop of this product is 2 g NIH 2024. One scoop is half the trial dose, and the trial's co-treatment — 400 mcg of folate — is matched exactly, in a different molecule. To take the dose the evidence belongs to you take two scoops, which also doubles the coenzyme Q10 to 300 mg, the folate to 800 mcg and the green tea to 100 mg. Nobody on the label tells you that, and nobody has tested 2 g/day against placebo.
The pooled evidence, and it is genuinely encouraging. A systematic review of 26 randomized trials and 1,691 women — 806 on inositol, 311 on placebo, 509 on metformin — found women on inositol 1.79 times as likely to have a regular menstrual cycle (95% CI 1.13 to 2.85), with free testosterone down (mean difference −0.41, CI −0.69 to −0.13), total testosterone down (−20.39, CI −40.12 to −0.66), fasting glucose down (−3.14, CI −5.75 to −0.54), insulin area under the curve down (−2,081.05, CI −2,745.32 to −1,416.78) and SHBG up (+32.06, CI 1.27 to 62.85) Greff 2023. Note that the SHBG interval very nearly touches zero: a lower bound of 1.27 on a point estimate of 32 is a wide, imprecise estimate rather than a confident one.
And now the finding that the marketing for this category leaves out. A second systematic review, commissioned specifically to inform the 2023 update of the International Evidence-based PCOS Guidelines, searched to August 2022, included 30 trials and 2,230 women (1,093 intervention, 1,137 control), pooled 19 of them across 13 comparisons — and concluded that “the evidence supporting the use of inositol in the management of PCOS is limited and inconclusive”, that inositol may have no effect on outcomes beyond some metabolic measures, and that metformin may be better for waist-hip ratio and hirsutism with likely no difference for reproductive outcomes Fitz 2024.
Two systematic reviews, one year apart, largely the same trials, opposite verdicts — and the reason is methodological rather than mysterious. The independent review pooled broadly and reported effect sizes Greff 2023; the guideline review graded certainty and pooled only where the evidence justified it, which is why 30 included trials produced only three meta-analyses Fitz 2024. Both are honest readings of a literature made of small, short, heterogeneous trials. The obstacle to translation on this page is therefore not a species gap or a gut wall: it is that the underlying trials are not good enough for two careful teams to agree about them, and any page that quotes only one of the two is selling rather than describing.
The last obstacle is the population. Every number above comes from women with polycystic ovary syndrome and insulin resistance Nestler 1999 Gerli 2007 Greff 2023 Fitz 2024. A woman with regular cycles and a normal fasting insulin taking this for ‘hormone balance’ is outside every trial that has ever been run on it.
Ovarian & Hormone Support — which form, and does it matter
The 40:1 is the specification, and it is the one number to check against a competitor. 2,000 mg myo-inositol to 50 mg D-chiro-inositol NIH 2024. Products selling D-chiro-inositol alone, or at ratios like 10:1 or 4:1, are making a different mechanistic bet — and the trial with the biggest ovulation numbers did use D-chiro alone Nestler 1999, so the bet is not absurd. It is simply a different one from the 40:1 physiological-ratio argument this product is built on, and a label that gives a combined ‘inositol’ milligram figure with no split has told you nothing.
Powder rather than capsules, and that is forced by the dose. Two grams of myo-inositol is roughly four large capsules before excipients; four grams is eight. No inositol product at a trial dose can be a capsule, which is why this arrives as a 3.57 g flavored scoop with stevia and medium-chain triglyceride oil NIH 2024. If you find an inositol capsule product, check the milligrams before the price.
The folate form has a pharmacokinetic study behind it, and it is a clean one. Sixteen women homozygous for MTHFR 677TT and eight wild-type took a single oral dose of 400 mcg folic acid and 416 mcg [6S]-5-MTHF in a randomized crossover. AUC and Cmax were significantly higher and tmax significantly shorter for the methylfolate in both genotypes — and unmetabolized folic acid appeared regularly in plasma after folic acid but rarely after [6S]-5-MTHF Prinz-Langenohl 2009. The [6S] designation is the part to read: the racemic [6R,S] mixture delivers half its mass as a diastereomer the body cannot use.
The coenzyme Q10 is a phytosome, which is a claim about solubility. 150 mg of a CoQ10-phospholipid complex NIH 2024. Ubiquinone is a large, entirely lipophilic quinone with famously poor and variable absorption, so a phospholipid complex is a sensible answer to a real problem. Whether 150 mg of this complex matches a larger dose of plain ubiquinone is a formulation claim, and no head-to-head appears in any trial on this page.
And what this product is not, said plainly: it is not a prenatal. It carries 400 mcg of folate and no iron, no iodine, no vitamin D and no B12 NIH 2024. Sold to women trying to conceive, with the folate a preconception dose, it is easy to assume it covers preconception nutrition. It covers one nutrient of the four that matter most.
What would have to be true, and how you would know it was not
1. fasting-insulin moves first, and it moves before anything hormonal. The mechanism is insulin, then SHBG, then free testosterone, in that order. The pooled insulin area under the curve fell by 2,081 µU/mL per minute Greff 2023. Predict fasting insulin down by 12 weeks in a woman whose baseline is raised — and predict nothing at all if fasting insulin was normal to begin with, because then the mechanism has nothing to act on and the ovulatory problem is a different one.
2. shbg up, then free-testosterone down, in that sequence. Pooled SHBG rose 32.06 nmol/L and free testosterone fell 0.41 Greff 2023. Retest at 12 weeks, in the early follicular phase both times if cycles are occurring at all — comparing a follicular draw against a luteal one manufactures changes that are not there. And read the confidence interval before celebrating: 1.27 to 62.85 on SHBG Greff 2023 means the real effect could be trivial.
3. Cycles, on the timescale the trials used rather than the one the label implies. Pooled odds of a regular cycle were 1.79 times higher Greff 2023; in the myo-inositol trial ovulation frequency was 25% against 15% and first ovulation came at 24.5 days instead of 40.5 Gerli 2007. Predict cycles that shorten and steady somewhat, over three to six months, not a restored 28-day cycle in a fortnight. Keep a dated log from before the first scoop, because nobody remembers last quarter's cycle lengths accurately once they are hoping.
4. The prediction that cuts against the product, and it has two halves. One scoop is 2 g against the trial's 4 g NIH 2024 Gerli 2007, so predict a smaller effect than any number quoted above, and possibly none — nobody has tested this dose. And the review commissioned to inform the 2023 PCOS guidelines concluded the evidence is limited and inconclusive, with metformin likely no different for reproductive outcomes and possibly better for hirsutism and waist-hip ratio Fitz 2024. So predict that a substantial share of women see nothing, and that the honest comparison for anyone who wants ovulation rather than a supplement is a prescription conversation.
5. hba1c, with a warning about how to read a flat one. Pooled fasting glucose fell 3.14 mg/dL Greff 2023 — small. Predict HbA1c essentially unchanged at 12 weeks, and do not read that as failure: PCOS-associated insulin resistance frequently sits behind a completely normal HbA1c, which is exactly why fasting-insulin is the marker to draw and HbA1c is the one to ignore.
6. amh and lh-fsh, predicted flat, and named to stop them being misused. AMH is high in PCOS because of the antral follicle count, not because of insulin, and nothing in these trials moved it. Predict AMH unchanged, predict the LH-to-FSH ratio unchanged, and treat anyone selling AMH as a response marker for a supplement with suspicion. If cycles are still absent at six months, that is a gynecology referral rather than a second scoop — prolonged anovulation leaves the endometrium under unopposed estrogen, and that is a risk with a name, not a fertility inconvenience.
What nobody has tested yet
Nobody has tested 2 g a day. The myo-inositol PCOS trials run at 4 g Gerli 2007, this scoop is 2 g NIH 2024, and a dose-ranging study against placebo has never been published. It is the cheapest and most commercially consequential missing trial in this cohort, because every half-dose inositol product on the market depends on the answer.
Nobody has tested the 40:1 ratio head to head. The ratio is a mechanistic argument from tissue physiology; the trial with the strongest ovulation result used D-chiro-inositol alone Nestler 1999 and the trial matching this label's isomer used myo-inositol alone Gerli 2007. A three-arm trial — myo alone, 40:1, D-chiro alone — at adequate power would settle a question the entire category is currently priced on.
Nobody has reconciled the two systematic reviews. Twenty-six trials pooled broadly say inositol works Greff 2023; thirty trials graded for certainty say the evidence is limited and inconclusive Fitz 2024. What is missing is not another meta-analysis. It is one adequately powered, pre-registered, placebo-controlled trial with live birth as its primary endpoint, and after twenty-five years there is still not one.
Nobody has tested this blend. Inositol plus a CoQ10 phytosome plus three fruit polyphenols NIH 2024 has never been trialed as a formula, and the coenzyme Q10 and polyphenol components do not appear in any study cited on this page.
And nobody has studied a woman without PCOS. The product is sold for cycle regularity and ovarian health generally; every trial enrolled women with polycystic ovary syndrome Nestler 1999 Gerli 2007 Greff 2023 Fitz 2024. Whether inositol does anything in a woman with normal insulin and irregular cycles is not a hard study to run and it has not been run.
Ovarian & Hormone Support — its own safety story, not its category's
The first safety statement on this page is a correction. The warnings usually attached to a women's hormone blend — the contraceptive interaction, the photosensitivity, the liver signal, the estrogenic activity — belong to vitex, dong quai, black cohosh and red clover. None of those is in this product NIH 2024. There is no phytoestrogen in it, nothing that binds an estrogen receptor, and nothing with a documented cytochrome interaction. Reciting those cautions here would be borrowing another product's risks, which is exactly the failure this standard exists to remove.
What it actually does is gastrointestinal, and it is dose-dependent. Inositol at gram doses produces nausea, wind and loose stools in a minority, worse when the whole dose goes in at once — which is a practical argument for splitting two scoops rather than a reason to under-dose. The guideline review found myo-inositol likely causes fewer gastrointestinal adverse events than metformin, and that those events are typically mild and self-limited Fitz 2024. For a woman choosing between the two, that is a genuine and honestly stated advantage.
The interaction that matters most is B12, and it is specific to who takes this. The product supplies 400 mcg of methylfolate and no B12 NIH 2024. Folate can correct the anemia of B12 deficiency while the neurological damage continues, which is why the combination to watch for is folate without B12 — and the most likely woman to be taking this is a woman with PCOS, who is very often also on metformin Fitz 2024, which depletes B12. Have a B12 checked at least once, particularly if you are also vegetarian.
Two small ones, with the numbers that make them small. Green tea extract carries a recognized hepatotoxicity signal, but that signal lives at several hundred milligrams of EGCG a day and this product carries 50 mg of extract per scoop NIH 2024 — a minor contributor unless a separate green tea or weight-loss product is stacked on top, and remember that taking two scoops for the trial dose doubles it. And if there is any chance of conception, note that this is not a prenatal: it has folate but no iron, iodine, vitamin D or B12 NIH 2024, and it does not replace one.
Who should not rely on it. Anyone whose periods have stopped and who has not been worked up. Absent or very infrequent periods have causes — thyroid disease, hyperprolactinemia, premature ovarian insufficiency, hypothalamic amenorrhea from under-eating or over-training — that a myo-inositol powder does not touch and will delay finding. Every trial on this page enrolled women who already had a diagnosis Nestler 1999 Gerli 2007. Getting the diagnosis first is not a formality; it is what makes the product either appropriate or beside the point.

How you would know if it worked
Myo-inositol's PCOS trials are insulin trials with a hormonal endpoint attached: where it works, it works by improving insulin sensitivity, and the hormone changes follow from that. That gives you an order to expect. Fasting insulin moves first. SHBG rises next, because insulin is what suppresses the liver's production of it. Free testosterone comes down as more of it gets bound. Cycle regularity is the last thing to arrive, not the first, and HbA1c is the slow confirmation that the metabolic change was real. If fasting insulin was normal before you started, the ovulatory problem is not the one inositol addresses — and cycles that stay irregular past six months belong with a gynecologist, because absent periods have causes that matter well beyond fertility.
- Fasting Insulin Retest: Every 3–6 months, or 8–12 weeks after an intervention.
- SHBG (Sex Hormone-Binding Globulin) Retest: Every 3–6 months alongside testosterone.
- Free Testosterone Retest: With every total T retest — they must be interpreted together.
- HbA1c (Hemoglobin A1c) Retest: Every 3 months (matches red cell lifespan).
