How Horny Goat Weed actually works
Icariin, the principal flavonoid, is a genuine PDE5 inhibitor — the same enzyme target as sildenafil and tadalafil. Blocking PDE5 prevents breakdown of cyclic GMP, so nitric oxide signalling produces a longer vasodilatory response. The difference is entirely one of potency: icariin's activity is orders of magnitude weaker than the pharmaceuticals, and the quantity of icariin in a typical extract is far below what the in-vitro work used. Icariin also has weak oestrogenic activity and some bone-density data in postmenopausal models.

The important safety point follows from the shared mechanism: combining it with nitrates is the same theoretical risk, and it shouldn't be stacked casually with prescription PDE5 inhibitors.
