Real mechanism, almost certainly insufficient dose. It's an honest example of how a compound can be legitimately active in a test tube and still do very little in a capsule, and it's worth saying that plainly rather than borrowing credibility from the drug class. If erectile function is the actual goal, the prescription PDE5 inhibitors work and are cheap. The important safety point follows from the shared mechanism: combining it with nitrates is the same theoretical risk, and it shouldn't be stacked casually with prescription PDE5 inhibitors.
How Horny Goat Weed actually works
Icariin, the principal flavonoid, is a genuine PDE5 inhibitor — the same enzyme target as sildenafil and tadalafil. Blocking PDE5 prevents breakdown of cyclic GMP, so nitric oxide signaling produces a longer vasodilatory response. The difference is entirely one of potency: icariin's activity is orders of magnitude weaker than the pharmaceuticals, and the quantity of icariin in a typical extract is far below what the in-vitro work used. Icariin also has weak estrogenic activity and some bone-density data in postmenopausal models.

What Horny Goat Weed actually does
The named target is real, and naming it is what makes this page checkable. Icariin is a prenylated flavonol glycoside from Epimedium, and it inhibits phosphodiesterase type 5 — the enzyme that hydrolyzes cyclic GMP in corpus cavernosum smooth muscle. Recent work identifies PDE5A specifically as an icariin target and traces downstream effects from it Liao 2025. That is the same enzyme the prescription drugs act on Boolell 1996, which is the reason this herb is more interesting than most, and also the reason the comparison below is fair to make.
The pathway is worth stating precisely, because it explains both the effect and the interaction. Nitric oxide from nitrergic nerves and endothelium activates soluble guanylate cyclase, which makes cyclic GMP, which activates protein kinase G, which lowers intracellular calcium and relaxes the smooth muscle. PDE5 destroys the cyclic GMP. Inhibit PDE5 and the signal persists — but only where nitric oxide put a signal there in the first place. A PDE5 inhibitor amplifies an existing signal; it does not create one.
Icariin is a glycoside, and the sugars are the problem. Icariin carries rhamnose and glucose on a prenylated flavonol core. Removing them gives icariside II and then icaritin, and the whole published effort around this molecule is about overcoming poor water solubility and poor membrane permeability, which together produce low bioavailability Szabó 2022. Formulation strategies, structural transformation and absorption enhancers are the three approaches the field has taken, and their existence is the admission.
Compare that with what is known about a drug aimed at the same enzyme, and the asymmetry is the whole argument. Sildenafil's absolute oral bioavailability, food effect and dose proportionality were established in healthy men in a single defined study Nichols 2002. For icariin there is no equivalent: no absolute bioavailability figure in humans, no established plasma concentration at a supplement dose, and therefore no way to say whether an oral capsule ever reaches a PDE5-inhibiting concentration in cavernosal tissue Szabó 2022.
Which leaves the honest mechanistic statement. The target is identified Liao 2025, the pathway is understood, the compound class has genuine actions on the male reproductive system in preclinical work Liu 2024, and the step that has never been closed is delivery. A mechanism that cannot reach its target at the dose sold is a mechanism the reader does not have.
Cell, rodent, human — and where it stops
Cell and animal work is extensive; the human step is where this stops. The review of Epimedium's bioactive compounds in the male reproductive system surveys the current status and calls out the future work needed, which is the standard shape of a literature that has not produced a definitive clinical trial Liu 2024.
The newest mechanistic work is in cells and animals, and the endpoint is not erection. Icariin targeting PDE5A was studied in spermatogonial stem cells, where it regulated viability, DNA synthesis and DNA damage and improved reproductive capacity Liao 2025. That is a real finding about a real target and it is a fertility endpoint in a preclinical model, not a sexual-function endpoint in men.
The pharmacokinetic obstacle is documented rather than suspected. Icariin and its derivatives have poor water solubility and poor membrane permeability, and the published literature consists largely of strategies — nanoformulation, structural modification, absorption enhancers — to raise the concentration reaching a target Szabó 2022. None of those strategies is what is in a 500 mg capsule of standardized extract.
The commercial layer subtracts again. An analysis of supplements marketed for erectile dysfunction scored products against the effective ingredients and minimum effective daily doses identifiable in the literature: 8% matched the higher-expected-efficacy cluster, 12% the lower, and 80% met the criterion of no expected efficacy, because such products are typically blends with individual ingredients at negligible doses Petre 2023. Epimedium is a common component of exactly that kind of blend.
So the translation obstacle here is not the usual one. It is not that the trial was in deficient people, and not that the mechanism is fictional. It is that the compound is poorly absorbed Szabó 2022, the human trial has not been done Liu 2024, and the product is usually a blend at a dose nobody chose on evidence Petre 2023. Three separate gaps, each sufficient on its own.
Horny Goat Weed — which form, and does it matter
The genus is not one plant, and the species on the label changes the risk as well as the chemistry. Epimedium sagittatum, E. brevicornu, E. koreanum and E. grandiflorum are all sold as horny goat weed and differ in prenylflavonoid profile Liu 2024. The species matters for a specific reason: the mechanistic study of Epimedium-induced liver injury was done on Epimedium koreanum Nakai Li 2022. A label that says only “Epimedium” has withheld the variable that the one hepatotoxicity study was about.
Percent icariin is the standardization number, and it measures the least active member of the family. Icariin is the glycoside; icariside II and icaritin are the deglycosylated forms, and the bioavailability literature treats icariin as the starting material whose delivery has to be improved Szabó 2022. A product standardized to 10% or 20% icariin is standardized to the form with the solubility and permeability problem.
Run the extract arithmetic anyway, because it is instructive. A 500 mg capsule of a 10% icariin extract delivers 50 mg of icariin; a 20% extract delivers 100 mg. Those are the numbers on the shelf. What is missing on the other side of the equation is the plasma concentration they produce, which has not been established in humans Szabó 2022 — so the dose can be computed and the exposure cannot, and any claim about potency relative to a drug is being made across a gap nobody has measured.
Traditional preparation and a modern extract are different materials. Classical use is a decoction of the leaf, often processed with fat before use, taken as part of a multi-herb formula. A concentrated solvent extract standardized to a single flavonoid is a twentieth-century product. The safety record of the first is not evidence about the second, and the liver-injury work was done on modern material Li 2022.
The form question that should worry a buyer most is whether the product is a herb at all. If a herbal product reliably produces the recognizable pharmacology of a PDE5 inhibitor — facial flushing, headache, nasal congestion, a several-hour duration — that is worth taking seriously as evidence about the product rather than about the plant, because icariin's own literature says it struggles to reach a meaningful concentration Szabó 2022. A drug-like effect from a material with a documented delivery problem is a reason to stop and have the product tested.
What would have to be true, and how you would know it was not
1. The prediction that would establish the mechanism in a person, and the study design is obvious. Predict that a standardized icariin dose does not change IIEF-5 score against placebo over four weeks at commonly sold doses, because the compound's bioavailability problem is documented and unsolved in marketed products Szabó 2022 and no adequately powered trial exists to say otherwise Liu 2024. A positive blinded trial at a defined icariin dose would falsify this and would be the most valuable paper in the category.
2. Predict no endocrine change at all. PDE5 inhibition is a hemodynamic mechanism, not a hormonal one Boolell 1996. Predict total testosterone, free testosterone, LH and SHBG unchanged at 8 weeks. If they move, something in the bottle is not Epimedium.
3. The prediction that identifies an adulterated product. Predict that an Epimedium product producing a sudden, reproducible, hours-long effect with flushing and headache contains a phosphodiesterase inhibitor that is not on the label — because sildenafil's pharmacokinetics were characterized precisely so that profile is recognizable Nichols 2002, and 80% of marketed erectile dysfunction supplements were scored as having no expected efficacy from their declared contents Petre 2023. The reader's own experience is the assay here, and a strong effect is the warning.
4. Predict liver enzymes are the marker to watch, with a defined window. A mechanistic study of Epimedium koreanum-induced liver injury exists Li 2022. Predict ALT and AST unchanged at 8 weeks in most users, and treat any rise, or new fatigue, dark urine or right upper quadrant discomfort, as a reason to stop immediately and get tested rather than to reduce the dose.
5. Predict a blood pressure interaction is dose-limited by absorption, and do not rely on that. Sildenafil potentiates the hypotensive effect of nitric oxide donor drugs in men with stable angina, which is why the nitrate contraindication exists Webb 2000. Whether an icariin extract reaches a concentration capable of the same additive effect is unknown Szabó 2022. Predicting “probably not” is not a basis for combining it with a nitrate, and the asymmetry of consequences settles it.
What nobody has tested yet
Nobody has published icariin's absolute bioavailability in humans. This is the single missing number that would let anyone reason about a dose. The field has instead published strategies for improving bioavailability Szabó 2022, which is a way of working around a measurement rather than making it.
Nobody has run the trial the mechanism deserves. A randomized, placebo-controlled study of a defined icariin dose with IIEF-5 as the primary endpoint, in men with mild erectile dysfunction, does not exist in a form the reviews can point to Liu 2024 Petre 2023. The mechanism has been identified at the enzyme Liao 2025 and never followed into the clinic.
Nobody has established which species should be on the label. Prenylflavonoid content differs across Epimedium species Liu 2024 and the one mechanistic hepatotoxicity study concerns one of them Li 2022. A comparative study of icariin content and hepatic signal across the four commercially used species would tell buyers which name to look for, and has not been done.
And nobody has tested the deglycosylated forms in people. Icariside II and icaritin are the more permeable relatives Szabó 2022. Whether either produces a measurable erectile or endocrine effect in humans is unexamined, which means the interesting compound in this family has never been given to a person in a controlled setting.
Horny Goat Weed — its own safety story, not its category's
The organ-specific risk on this page is the liver, and it is documented at mechanism level rather than as folklore. Epimedium koreanum Nakai-induced liver injury has been studied with untargeted metabolomics to work out how the injury happens Li 2022. That is a stronger statement than a case report: somebody found the signal reproducible enough to investigate the pathway. Anyone taking a concentrated Epimedium extract for more than a few weeks has a reason to check ALT and AST that most botanicals do not provide.
The blood pressure caution is inherited from the drug class and should be obeyed anyway. Sildenafil potentiates the hypotensive effect of nitric oxide donors Webb 2000, which is why nitrates are an absolute contraindication with prescription PDE5 inhibitors. Icariin acts at the same enzyme Liao 2025, its achievable human exposure is unmeasured Szabó 2022, and the downside of being wrong is a profound drop in blood pressure. Do not combine this with nitroglycerin, isosorbide or any nitric oxide donor, and do not stack it with a prescribed PDE5 inhibitor.
Alpha-blockers deserve their own sentence. Men taking tamsulosin, doxazosin or a relative for prostate symptoms are the population most likely to be buying this, and they are also the population in whom additive vasodilation shows up as orthostatic dizziness. That combination is managed carefully with prescription PDE5 inhibitors Boolell 1996, and it is not managed at all when one half of it is a supplement nobody was told about.
The blend problem is a safety problem, not only an efficacy one. Where 80% of marketed erectile dysfunction supplements were scored as containing many substances at negligible doses Petre 2023, a consumer cannot know what they reacted to. A single-ingredient extract naming its species and its icariin percentage is the only version of this product whose risk can be reasoned about.
Who should not take it. Anyone on a nitrate, in any form, including recreational nitrites Webb 2000; anyone with existing liver disease or on other hepatotoxic agents Li 2022; anyone on an alpha-blocker or with symptomatic orthostatic hypotension; and anyone whose erectile symptoms are new, since new erectile dysfunction is a recognized early vascular sign and belongs in front of a clinician rather than behind a supplement.

How you would know if it worked
There is no blood test for this one. That is not a criticism — it is a fact about the effect, and it changes how you should judge it.
- What to watch: Erections — firmness, and whether they last — tracked across the same weeks. The proposed mechanism is PDE5 inhibition, the same enzyme the prescription drugs block, so the observable is the one those drugs are judged on. Morning erections are the cleanest version of it, being the least influenced by what you were thinking about at the time.
- How long before it means anything: Four weeks taken consistently. The pharmaceutical version of this mechanism works within an hour; a weak version of it in a plant, at an icariin content the label may not even state, needs long enough that you are not simply scoring your mood that fortnight.
- What will fool you: The standardization. Icariin is the active compound and its content varies wildly between products, so a null may mean you bought a weak extract rather than that the mechanism is absent. Two safety lines outrank the experiment: this does not belong anywhere near nitrates for chest pain, and erectile function that has genuinely declined is one of the earliest signals of vascular disease and of low testosterone. Getting that looked at is the higher-value move, and it may find something worth treating.
Run it one variable at a time. Starting three things in one week means a result you cannot attribute, which is the same as no result.
