This is protection against glucose damage rather than glucose lowering, which is a distinction worth understanding — it will not move HbA1c but may reduce what a given HbA1c costs you. Trial evidence in diabetic neuropathy is decent. It is also the better thiamine form for anyone with impaired absorption from alcohol use or bariatric surgery.
How Benfotiamine actually works
A fat-soluble thiamine derivative with far better tissue penetration than thiamine hydrochloride. It raises thiamine pyrophosphate, which activates transketolase — and that is the key step: transketolase diverts glycolytic intermediates into the pentose phosphate pathway and away from the three routes that cause hyperglycemic damage, being AGE formation, the hexosamine pathway and protein kinase C activation.

What Benfotiamine actually does
Benfotiamine is not a lipid-soluble vitamin, and the correction matters because it changes what the molecule can do. It is S-benzoylthiamine O-monophosphate: an open-ring, neutral, S-acylated derivative. Being uncharged, it crosses the intestinal membrane without needing the saturable thiamine transporters, which is the entire point of the molecule. It is then dephosphorylated by alkaline phosphatase at the brush border to S-benzoylthiamine, and the benzoyl group is removed by thioesterases in blood and tissue to release thiamine Volvert 2008.
Where it goes is not where the marketing says. The careful comparison of benfotiamine against genuinely lipid-soluble allithiamine derivatives found different mechanisms and a different pharmacological profile: benfotiamine raises thiamine and thiamine diphosphate strongly in blood, liver and kidney, and much less in brain, because the released thiamine still has to use a transporter to cross the blood-brain barrier Volvert 2008. Sulbutiamine and the thioester series do reach brain Bettendorff 2023. Benfotiamine's advantage is peripheral.
The disease mechanism it was designed for is the transketolase-hyperglycemia hypothesis. Excess intracellular glucose in cells that cannot downregulate their uptake — endothelium, mesangium, neurons — overflows into four damaging routes: the polyol pathway, hexosamine flux, protein kinase C activation by diacylglycerol, and advanced glycation end product formation from methylglyoxal. Activating transketolase diverts fructose-6-phosphate and glyceraldehyde-3-phosphate back into the pentose phosphate pathway and starves all four Beltramo 2021.
That is an elegant hypothesis with a specific enzyme at its center. It predicts that a compound which raises tissue thiamine diphosphate should reduce methylglyoxal-derived advanced glycation end products, and in cell and animal models it does.
The published critique of the mechanism deserves equal billing. Whether transketolase in human tissue is actually thiamine-limited at ordinary status, and whether the enzyme activation achieved in vitro is reproduced at achievable plasma concentrations, has been questioned in the same reviews that describe the therapeutic potential Bozic 2023. An enzyme that is already saturated cannot be activated by more cofactor.
Cell, rodent, human — and where it stops
This page has something almost nothing else in the cohort has: a large, long, well-designed trial that measured the outcome instead of the marker.
In cells and rodents the chain is complete. Benfotiamine raises transketolase activity, reduces hexosamine and protein kinase C flux and prevents experimental retinopathy and neuropathy in diabetic rodents. It is one of the better animal packages in this catalog Beltramo 2021.
In human pharmacokinetics the advantage is real and measured. Benfotiamine produces substantially higher blood thiamine and erythrocyte thiamine diphosphate than an equal oral dose of thiamine hydrochloride, because it bypasses the saturable carrier Volvert 2008. The surrogate the product is sold on genuinely moves, and moves more than the cheap alternative does.
Then somebody ran the trial properly. The BOND study was designed as a 12-month randomized, double-blind, placebo-controlled trial in people with type 2 diabetes and symptomatic polyneuropathy, with morphometric, neurophysiological and clinical measures as endpoints — corneal confocal microscopy nerve fiber measures and intraepidermal nerve fiber density alongside symptom scores Bonhof 2022. That is a design that cannot be satisfied by a biomarker.
The result is the reason this page is in a cohort about surrogates. Over 12 months, benfotiamine did not produce the improvement in the morphometric and neurophysiological measures the trial was built around Ziegler 2026. The compound raised the cofactor, as designed, and the nerves did not respond over a year.
The obstacle to transfer is probably the target, not the delivery. Earlier positive results came from shorter trials with symptom scores as the endpoint, where a subjective measure in a painful condition is vulnerable to placebo response. When the endpoint became a nerve fiber count, the effect did not survive. That is the sequence this whole file is about, and here it happened inside a single well-run program Bonhof 2022 Ziegler 2026.
Benfotiamine — which form, and does it matter
The form IS the drug here, and it is worth being precise about what it is not. Benfotiamine is frequently described as fat-soluble; it is not, particularly. Its aqueous solubility is reasonable and its absorption advantage comes from being uncharged rather than from partitioning into lipid Volvert 2008. The genuinely lipophilic thiamine derivatives are the allithiamines and the synthetic thioesters, and they have a different tissue distribution Bettendorff 2023.
The exposure comparison is the whole commercial case and it is well documented. Against an equal dose of thiamine hydrochloride, benfotiamine gives severalfold higher peak plasma thiamine and markedly higher erythrocyte thiamine diphosphate, with peak plasma concentration at roughly 1 to 2 hours and a longer apparent terminal phase because the S-benzoyl intermediate acts as a slow-release reservoir. Elimination of the released thiamine is by rapid renal clearance once tissue capacity is met Volvert 2008 Bozic 2023.
Two enzymes stand between the tablet and the cofactor, and both can be rate-limiting. Alkaline phosphatase at the brush border removes the phosphate; a thioesterase then hydrolyzes the S-benzoyl bond. Neither step involves a cytochrome, so the usual drug interaction list does not apply, but intestinal alkaline phosphatase activity varies with blood group secretor status, fasting and zinc status, which is an unstudied source of between-person variation.
The doses on the shelf and the doses in the trials are far apart. The BOND program used 300 mg three times daily, 900 mg a day, for twelve months Bonhof 2022. Most retail products are 150 or 300 mg once daily. Somebody buying benfotiamine on the strength of the diabetic neuropathy literature is usually taking a third of the studied dose — and the studied dose is the one that returned null on the hard endpoints.
What benfotiamine is not a substitute for. Because it reaches brain poorly, it is not the derivative to reach for where a central effect is wanted, and it is not appropriate as a replacement for parenteral thiamine in acute Wernicke encephalopathy Volvert 2008.
What would have to be true, and how you would know it was not
1. Predict the cofactor rises, and rises more than on an equal dose of the cheap salt. On 300 mg daily, predict erythrocyte thiamine diphosphate and whole blood vitamin B1 (thiamine) rise measurably within 4 weeks, and by more than 300 mg of thiamine hydrochloride would achieve Volvert 2008. This is the prediction the product will win.
2. Predict the nerve does not change over a year. In symptomatic diabetic polyneuropathy, predict no improvement in intraepidermal nerve fiber density, corneal nerve fiber measures or nerve conduction over 12 months at 900 mg a day, because that is what the trial designed to detect it returned Ziegler 2026 Bonhof 2022. A replication showing morphometric improvement would falsify this page.
3. Predict a symptom score improves and predict you will not be able to interpret it. Neuropathic symptom scales in painful conditions carry large placebo responses over 6 to 12 weeks. Predict an improvement in a self-rated score that is not accompanied by any change in a measured nerve parameter. That dissociation is the content of this page.
4. Predict the glycemic markers do not move. The mechanism is about handling intracellular glucose overflow, not about lowering glucose. Predict HbA1c and fasting insulin unchanged at 12 weeks Beltramo 2021. If HbA1c falls, something else in the regimen changed.
5. The measurement that would rescue the hypothesis. Predict that plasma methylglyoxal-derived advanced glycation end products fall on 900 mg a day. If they do not, the mechanism never engaged and the null endpoint is explained; if they do fall and the nerves still do not recover, the hypothesis was engaged and wrong. Nobody has published that measurement alongside the morphometric endpoints, and it is the single most informative thing left to do Bozic 2023.
What nobody has tested yet
Nobody has tested it early enough. Every trial has enrolled people with established symptomatic neuropathy, where nerve fibers are already lost. Whether benfotiamine prevents neuropathy in people with diabetes and normal nerve function is the question the mechanism actually predicts, and it requires a prevention trial that has never been funded Beltramo 2021.
Nobody knows whether human transketolase is thiamine-limited at all. The activation coefficient in a replete person is small, which implies the enzyme is mostly saturated. If it is, raising thiamine diphosphate further cannot increase flux, and the entire rationale fails at the first step Bozic 2023.
The Alzheimer's signal is a pilot and has stayed one. Small open-label and pilot randomized work with high-dose benfotiamine in mild cognitive impairment has been reported, and the compound reaches brain poorly by design Volvert 2008. Whether a brain-penetrant thioester would do better is an open question with a candidate molecule attached Bettendorff 2023.
And nobody has compared the derivatives head to head in people. Benfotiamine, sulbutiamine and fursultiamine differ in tissue distribution in animals and have never been randomized against each other for any human endpoint. A reader choosing between them is choosing on rodent biodistribution data.
Benfotiamine — its own safety story, not its category's
Tolerability across a year at 900 mg a day was good, and that is a real finding rather than an absence of data. The 12-month randomized program provides safety information at a dose three times what most products supply, with no signal that changed the risk-benefit reading Ziegler 2026 Bonhof 2022. Mild gastrointestinal upset and occasional rash are the usual reports.
The benzoyl group is the part that is not thiamine. Each 300 mg dose delivers a benzoate moiety that is cleaved and handled by conjugation with glycine to hippuric acid for renal excretion. That is an ordinary metabolic route and unremarkable at these doses, but it is the reason benfotiamine is not simply thiamine with better absorption, and it is not characterized in people with impaired renal clearance.
The clinical hazard is what it displaces. Painful polyneuropathy in a person with diabetes warrants an assessment of glycemic control, alcohol intake, B12 status and other causes. A supplement that produces a symptom-score improvement without changing the nerve can delay that work, and the trial evidence says that is the likely pattern Ziegler 2026.
One specific interaction to be aware of. Metformin, the drug most people in this population are taking, both lowers B12 over years and has been reported to interfere with thiamine transport Beltramo 2021. Anyone taking benfotiamine for diabetic neuropathy has a specific reason to have their B12 and methylmalonic acid checked rather than assumed.
Who this is not for. It is not a treatment for acute thiamine deficiency, which needs the parenteral route. It has no safety data in pregnancy or lactation at supplemental doses. And it is not interchangeable with the thiamine on a Supplement Facts panel, in either direction. How you would know if it worked
The thing this is sold to reduce cannot be measured directly: there is no advanced glycation end-product assay on this or any consumer menu, so the transketolase mechanism — diverting glycolytic intermediates away from the pathways that build AGEs — has to be judged through proxies. HbA1c is the best of them and it is a fair one, because HbA1c is itself a glycation product; fructosamine covers the 3 to 4 week window, which is the right scale for a 150-300 mg trial. The B1 level is here because this is a thiamine derivative reaching roughly 5 times the tissue concentration of thiamine hydrochloride, so it is the marker that confirms the dose landed. Be honest about the ceiling: the mechanism is clean, the clinical translation is mixed, and diabetic neuropathy is scored on symptoms and nerve testing by the physician already managing it.
