What Selenium, Blood actually measures — the analyte, and the assay

Which fluid was run matters more here than anywhere else in this set. Serum or plasma selenium integrates recent weeks. Whole-blood selenium is different, because erythrocyte selenium is largely glutathione peroxidase-1 loaded at erythropoiesis, so a whole-blood value integrates months. This page's test is named ‘Selenium, Blood’, and a reader comparing it to a serum reference range is comparing two different pools.

Method is ICP-MS, or hydride-generation atomic absorption. There is no colorimetric routine method for selenium, which is why the large serum-mineral method comparisons that cover sodium, potassium, calcium, magnesium, iron, zinc and copper do not cover it Kojo 2024. Sample volume is a real constraint: a population survey running multi-element ICP-MS on 0.25 mL of whole blood from 758 healthy participants found several elements below the limit of detection and recommended at least 0.5 mL for future work Syversen 2021.

The functional marker, and the fact that makes this page different: the pool saturates. Selenium's job is done by 25 genetically encoded selenoproteins, and selenoprotein P (SELENOP) is the transport form that carries selenium to peripheral tissue and the main plasma selenoprotein. SELENOP represents a saturable pool, maximized at a plasma selenium concentration of about 110 µg/L, or a selenomethionine intake of about 1.2 µg per kg body weight in adults Alexander 2023. Above that, total selenium keeps rising and the functional pool does not.

The two markers are related but not interchangeable. In 1500 participants of the Berlin Aging Study II, serum selenium and SELENOP correlated at r = 0.59, p < 0.001 — and gave different answers about disease: SELENOP was positively associated with hypertension (OR 1.15 [1.01, 1.32] per mg/L) while total selenium was non-linearly associated, the third quartile carrying an OR of 0.66 [0.45, 0.96] against the lowest Demircan 2023. A correlation of 0.59 is not a substitute.

Selenium, Blood: what changes the blood, and what only changes the reading

What changes the selenium in your body:

  1. Geology. This is the dominant term and it is unlike any other nutrient on the site: soil selenium varies by orders of magnitude, so intakes across the Nordic and Baltic region run from 39 to 88 µg/day in men and 22 to 68 µg/day in women, with Finland highest because fertilizer is supplemented and Norway supplied partly by imported selenium-rich wheat Alexander 2023. Brazil nut content follows the same rule, which is why ‘two nuts a day’ is a dose you cannot actually specify.
  2. Supplementation, and the form matters: selenomethionine is incorporated non-specifically into body protein wherever methionine would go, so it raises total selenium whether or not anything functional needs it Alexander 2023.
  3. Restrictive diets. Median urinary selenium was 13 µg/L across 205 Norwegian vegans, lacto-ovo vegetarians and pescatarians Henjum 2023.
  4. Malabsorption and critical illness, where micronutrient status alterations are associated with worse outcomes and selenium is one of the most studied de Man 2024.

What changes only the reading:

  1. Inflammation. Selenium and SELENOP behave as negative acute-phase reactants and fall in illness independent of status de Man 2024. Draw an hs-CRP or the number is uninterpretable.
  2. Serum versus whole blood, which sample two pools with different time constants.
  3. Hemolysis, because erythrocyte selenium exceeds serum selenium.
  4. The supplemented form, since selenomethionine parked in albumin reads as selenium and does nothing catalytic Alexander 2023.
  5. Day-to-day intake, though less than you would fear: across 947 spot urines collected over four seasons the log-transformed intra-individual variation for selenium was 10.5% against 14.7% between individuals — the tightest of the three elements measured Fuse 2023.

Reference interval or decision threshold — which kind of number Selenium, Blood is

A reference interval at the bottom, a toxicity ceiling at the top, and no defensible optimum in between. Each of those three claims has a source and they do not agree with the way the range is usually read.

The bottom. Observational data suggest inverse associations with cardiovascular disease, cancer and all-cause mortality at intakes below 55 µg/day or plasma/serum below 100 µg/L — but the same review states that a lack of quantitative data and inconsistency between studies precludes using them to derive dietary reference values Alexander 2023. That is a guideline body explicitly declining to turn an association into a target.

The top is not theoretical. Selenium causes toxic effects on liver, peripheral nerves, skin, nails and hair at intakes above 330 to 450 µg/day, and EFSA set a tolerable upper level of 255 µg/day for adults Alexander 2023. Put those two numbers beside each other and the gap is small: the tolerable ceiling sits barely below the intake at which liver, nerve, skin, nail and hair toxicity has been described, and both are within reach of ordinary supplementation. That is what makes selenium one of the few micronutrients where the upper limit is a practical concern rather than a footnote — and why a Brazil-nut habit, whose selenium content follows the soil the tree grew in Alexander 2023, is a poor way to dose it.

The middle is where the mortality data get misused. A meta-analysis of 20 cohort studies found that one standard deviation higher selenium biomarker concentration was associated with 13% lower all-cause mortality (RR 0.87 [0.83, 0.90]), 11% lower cardiovascular mortality (0.89 [0.84, 0.94]) and 15% lower cancer mortality (0.85 [0.78, 0.94]) — and its own conclusion is that trials in selenium-deficient populations are still needed Cui 2025. An association across cohorts is not a dose instruction, particularly when the functional pool stops responding around 110 µg/L Alexander 2023.

How you would know your Selenium, Blood was wrong — and when to redraw

Twelve weeks for serum, longer for whole blood — and the reason is which pool you sampled. Serum selenium responds over weeks. Erythrocyte and therefore whole-blood selenium is fixed at erythropoiesis and turns over on the red cell's 120-day lifespan, so a whole-blood value needs four months to fully reflect a change.

What would have to change for the second value to mean anything. If the first result was already near 110 µg/L, SELENOP is already maximal and a higher second number tells you that selenomethionine went into body protein, not that anything works better Alexander 2023. That is the falsification test for the whole exercise, and it is why the plateau is worth knowing before you buy anything.

Conditions that must match: same fluid, same laboratory, same supplement form and timing, no hemolysis.

How you would know your selenium was wrong. For the thyroid claim — the reason most people order this — draw free T3, free T4, TSH and thyroid antibodies on the same sample, and hold the evidence honestly. An overview of 6 systematic reviews containing 75 randomized trials found TPO antibodies fell at 3 and 6 months on selenium, in both levothyroxine-treated and untreated groups — at very low to low certainty of evidence, with only one of the six reviews rated high quality Wang 2023. A meta-analysis of 23 trials found the same antibody reductions, no significant change in TSH, a higher rate of reported adverse effects, and concluded that current evidence does not justify the use of selenium supplementation in autoimmune thyroiditis Qiu 2021. So the prediction is: antibodies should fall by three months. If free T3 and TSH do not move with them, the antibody drop has not bought you anything the trials could measure. Add hs-CRP, because selenium falls in inflammation de Man 2024.

What Selenium, Blood cannot tell you

It cannot tell you selenoprotein activity. That is SELENOP's job, and total selenium correlates with it at r = 0.59 — strongly enough to be related, weakly enough that the two markers gave different associations with hypertension and diabetes in the same 1500 people Demircan 2023.

It cannot distinguish functional selenium from parked selenium. Selenomethionine incorporated into albumin in place of methionine raises the number without joining the selenoprotein pool Alexander 2023.

It cannot be read as ‘more is better’. The functional pool saturates near 110 µg/L and the tolerable upper intake is 255 µg/day Alexander 2023; the mortality benefit people quote is an observational association awaiting trials Cui 2025.

It cannot be interpreted during illness without an inflammatory marker beside it de Man 2024.

The wrong inference is specific and it is common in the thyroid world: an already-adequate selenium pushed upward on the strength of a cohort association and an antibody effect of very low certainty Wang 2023 Qiu 2021, with a toxic ceiling only two shelf products away.
