People rarely search for a marker — they search for how they feel. These are the complaints where this marker is worth checking, and whether it is a first-line test or a follow-up once the obvious causes are ruled out.
Why your D-Dimer might be wrong
Most abnormal results are interference, not disease. These are the things that measurably move this specific marker — check them before you change anything. Each one says whether the number is wrong and should be repeated, badly timed and should be redrawn, or real with a cause worth explaining.
D-dimer rises with infection, surgery, injury, pregnancy, cancer and simply with age. It has excellent negative predictive value and poor specificity — a raised D-dimer in a well person usually means nothing.
This is a rule-OUT test. A raised value in someone with no symptoms is not a diagnosis. Chest pain or breathlessness is an emergency, not a lab order.
Clotting assays depend on an exact blood-to-citrate ratio, so a tube that did not fill to the line gives an unreliable result in either direction.
If the tube was short or the draw clotted, repeat it rather than interpreting the number.
What D-Dimer means in combination
A single marker tells you a little. Combinations tell you the story — these are the named patterns this marker takes part in, and what each one points at.
Several independent contributors to clot risk stacking together. Each is unremarkable alone; the combination is not, particularly alongside androgen use, high haematocrit, oestrogen therapy or long-haul travel.
A raised D-dimer with leg swelling, chest pain or breathlessness is an emergency, not a retest. Otherwise: address homocysteine with B12/folate/B6, treat the inflammation driving fibrinogen, and review anything oestrogenic or androgenic you are taking with a clinician.
